Conjoined Twins with Thoracoabdominopagus Anomaly in the Third Trimester

ABSTRACT Conjoined twinning is rare; occurrence is about 1% of monochorionic twins. Here, we report a case of 26-year-old multigravida with 29 weeks pregnancy with conjoined twinsthoracoabdominopagus. Two live female babies joined at the chest and abdomen were delivered by cesarean section. The parents refused a separation operation and despite resuscitation the twins died of cardiopulmonary arrest after 16 hours. A review of the literature suggests that early diagnosis by a combination of ultrasound and magnetic resonance imaging is essential for predicting prognosis, management and success of surgical separation. Despite the rapid technological advance, the expansion of prenatal ultrasound diagnosis, as well as the accumulation of experience by both domestic and foreign experts, prenatal recognition of true knots of the umbilical cord causes significant difficulties. Three cases of successful ultrasound diagnosis of true knots of the umbilical cord and brief review of the literature are presented.

А case of ultrasound diagnosis of the giant fetal cervical tumor in the third trimester of gestation is presented. The diagnosis of a cervical teratoma was supposed by ultrasound examination and magnetic resonance imaging. The tumor resection was successfully performed at 6 days of life. Histopathological diagnosis was mature teratoma.

A patient at her 38+ wks of pregnancy as a ultrasonographically diagnosed case of conjoined twin admitted in our department with ruptured membrane. Two live female babies joined at the chest were delivered by caesarean section. The parents refused a separation operation and the mother and the babies were discharged from hospital at 6th post natal day. A review of the literature suggests that early diagnosis by a combination of ultrasound and MRI is essential for management as it provides prognosis for viability and process of surgical separation and also the opportunity for early counseling of parents and termination if indicated. Meningiomas are known to be more common in females than males. They are also known in rare cases to grow in size during pregnancy, which can complicate its management. We describe a 31-year-old Caucasian woman who presented with blurring of her vision and diplopia during the third trimester of her pregnancy. Magnetic resonance imaging (MRI) showed a small left cavernous sinus meningioma. The patient was treated conservatively until her uncomplicated delivery. A postpartum MRI scan showed complete regression of the suspected meningioma. Currently the patient is contemplating a further pregnancy.

Accurate calculation of the estimated fetal weight is necessary for the choice of a correct approach to management of labour. Th e existing methods are not universal and require complex application. Th is article presents a review of literatures published in PubMed and Google Scholar databases in 1955-2021 and devoted to historical aspects in development of the existing clinical and instrumental approaches to calculation of estimated fetal weight. The paper presents existing methods for calculation of estimated fetal weight at diff erent gestational ages as well as methods making it possible to predict fetal weight before gestation onset. Data on their informative value during the third trimester and alterations in their accuracy depending on the pregnant patient’s BMI are presented. Th e topic of application of magnet-resonance imaging for fetometry is considered with comparison of this approach to a more common method of ultrasonography.
