ABSTRACT Context Chylothorax is an uncommon complication after penetrating trauma to the chest and the neck. Only 25% of all chylothorax cases are related to penetrating trauma. We report such a case on a transmediastinal stab wound, managed by video-assisted thoracic surgery. Case report A 48-year-old male presented to our level I trauma center after sustaining a stab wound to the right chest. Initial workup revealed a left side pneumothorax. After chest tube placement 700 ml of blood and milky fluid were recovered. The patient was managed initially with TPN, without success. On day 3, he was taken to the OR for a left VATS. The thoracic duct was dissected and the injury identified and controlled with hemoclips. The intervention was successful and the patient was discharged on POD #4. Discussion Chylothorax are classified and managed according to the daily output of the fistula. While low output chylothorax are more frequent, more likely related to malignancy of the mediastinum and lymph nodes, and more likely to close with nonsurgical management; traumatic chylothorax are generally high output and more likely to require surgical intervention. Bochdalek hernia is the most common congenital diaphragmatic hernia that typically presents in childhood. However, right sided diaphragmatic hernia is relatively rare. We review the case of 21 years old female with incidentally detected congenital diaphragmatic hernia on the right side. There are fewer than 20 cases of right sided diaphragmatic hernia reported in adults in literature.

Impalement injuries are a unique form of penetrating trauma and are typically associated with a fall onto the object (Steele, 2006). We present the case of a 45-year-old man who reportedly slipped in his bathtub and fell onto a broomstick. Radiographic examination revealed a slender mass extending from his rectum to the right side of his neck. A review of English literature suggests that this is the second reported case in the last 100 years describing the successful management of an impalement injury traversing the pelvic, abdominal, and thoracic cavities. The management of this case is described.

Chronic granulomatous disease is a rare phagocytic disorder with recurrent, severe bacterial and fungal infections. We describe an unusual case of chronic granulomatous disease manifesting as an invasive pulmonary aspergillosis with an obstructive necrotic mass at the right middle bronchus. The patient was successfully treated with a bronchoscopic intervention for the removal of the obstructive mass and a medical therapy.

Adrenal sarcomatoid carcinomas are extremely rare tumors presenting with extensive locoregional spread at the time of diagnosis. Patients succumb to metastases within a couple of months. As a result, very few cases are reported in the literature until now. We present a case of a 62-year old female with non-functional sarcomatoid carcinoma of the right adrenal gland. There was no radiological evidence of locoregional metastases. Patient underwent right adrenalectomy. Follow up after 3 months showed para-aortic lymphadenopathy and similar left adrenal mass on computed tomography. Patient refused further treatment and succumbed to the disease. A brief case report with review of literature is presented.

Salmonella Osteomyelitis

Abstract Salmonella osteomyelitis is a rare entity, typically associated with hemoglobinopathies or other underlying disorders. We report a case of Salmonella osteomyelitis of the right knee in an otherwise healthy 1-year-old boy. He received 8 weeks (2 weeks intravenous and 6 weeks oral) of antibiotic treatment, with full recovery.

ABSTRACT Introduction Leiomyoma arising from the vagina is a rare entity with varied presentations. Case Report A woman 44 years of age presented with complaints of something coming out vaginally, polymenorrhea, and pain in abdomen. A mass arising from the right posterolateral wall of vagina was seen. Ultrasound reported it to be cervical fibroid. The mass was enucleated through vaginal route. Histopathology confirmed it to be a leiomyoma. Review of literature revealed that it has a varied presentation. Diagnosis is often missed.
