ABSTRACT Introduction In recent years, the various health benefits of meditation have been acknowledged by the scientific community as well as by the public. Apart from its physiological benefits, it can also improve the psychological and spiritual well-being. A case–control study was planned to investigate the effect of Rajyoga Meditation on cardiovascular autonomic activity in meditators and nonmeditators. Materials and methods The study was conducted on 100 subjects, randomized into two groups: meditators (n = 50; age 35.80 ± 7.69 years) and nonmeditators (n = 50; age 36.76 ± 6.38 years). The meditator group practiced meditation for 30 minutes in the morning as well as in the evening. The control group did not practice any type of meditation or relaxation techniques. The cardiovascular parameters – heart rate (HR), systolic (SBP) and diastolic blood pressure (DBP), and Valsalva maneuver tests – were performed in both the groups in the same environmental conditions. The data were compiled and analyzed using unpaired t test. Results The mean values of HR in meditators and the control group were 77.08 ± 5.39 and 80.68 ± 5.71 respectively, and the difference was statistically significant (p < 0.001). The mean values of SBP in meditators and nonmeditators were 124.60 ± 5.39 and 129.56 ± 4.30 mm Hg respectively, while those for DBP were 77.84 ± 4.65 and 80.80 ± 4.78 mm Hg respectively. The difference in both was statistically significant. In meditators, Valsalva ratio was 1.60 ± 0.20, while in nonmeditators, it was 1.33 ± 0.13, and the difference was highly significant. Conclusion Significant improvement is seen in physiological, cardiac, and parasympathetic parameters in Rajyoga meditators. A shift of the autonomic balance toward the parasympathetic side is seen. By purposefully energizing the parasympathetic system by meditation, we can combat the ill effects of stress and help heal many health conditions. We studied the effect of experimenter expectations and different instructions in a balanced placebo design. 157 subjects were randomized into a 2 × 4 factorial design. Two experimenters were led to expect placebos either to produce physiological effects or not (pro- vs. antiplacebo). All subjects except a control group received a caffeine placebo. They were either made to expect coffee, no coffee, or were in a double-blind condition. Dependent measures were blood pressure, heart rate, well-being, and a cognitive task. There was one main effect on the instruction factor (p = 0.03) with the group “told no caffeine” reporting significantly better well-being. There was one main effect on the experimenter factor with subjects instructed by experimenter “proplacebo” having higher systolic blood pressure (p = 0.008). There was one interaction with subjects instructed by experimenter “proplacebo” to receive coffee doing worse in the cognitive task than the rest. Subjects instructed by experimenter “antiplacebo” were significantly less likely to believe the experimental instruction, and that mostly if they had been instructed to receive coffee. Contrary to the literature we could not show an effect of instruction, but there was an effect of experimenters. It is likely, however, that these experimenter effects were not due to experimental manipulations, but to the difference in personalities.

Introduction: Cigarette smoking is a prime risk factor for cardiovascular morbidity and mortality. Chronic smoking results in autonomic dysfunction leading to increased cardiovascular risk in smokers. The present study was planned to study the effect of smoking on the Cardiovascular Autonomic Functions among smokers. Materials and Methods: Fifty male subjects who were in the age group of 25 to 45 years. They were grouped into 25 smokers and 25 non-smokers. The participant subjects were selected among the staff members, residents and the patients from the routine OPD in SVIMS. Prior to study, they were informed about the procedure and the purpose of the study tests and written consents were obtained from them. The Cardiovascular Autonomic Function Tests were assessed by using a POLYGRAPH which was available in the department. Results and Conclusion: After applying the ‘t’-test for the difference between the two sample means, it was observed that there was a highly significant difference between the mean values of the BMI(i.e.p<0.01) and the para-sympathetic function tests among the smokers and the non – smokers(i.e.p<0.00). The Resting Heart Rate had significantly increased and the Deep breathing difference, the postural tachycardial index (Response to standing) and the Valsalva Ratio had significantly decreased in the smokers as compared to those in the non – smokers. After applying the ‘t’-test for the difference between the two sample means, it was observed that there was no significant difference between the mean values of the Postural hypotension test (i.e. p>0.05) and that there was a highly significant difference between the mean values of the Sustained handgrip test in the smokers and the non – smokers (i.e. p<0.00). Haemodynamic stability is an integral and essential goal of any anaesthetic management plan. Laryngoscopy and intubation can cause striking changes in haemodynamics. Increase in blood pressure and heart rate occurs most commonly from reflex sympathetic and vagal discharge in response to laryngotracheal stimulation, which in turn leads to increased plasma norepinephrine concentration. This study was designed to compare efficacy of esmolol and lignocaine for attenuating haemodynamics response due to laryngoscopy and endotracheal intubation. The aim of this study was to compare the effects of Esmolol with that of Lignocaine to attenuate the detrimental rise in heart rate and blood pressure during laryngoscopy and tracheal intubation. One hundred and twenty adult patients randomized into group-L and group-E, were received lignocaine 1.5 mg/kg and Esmolol 1.5 mg/kg I.V. respectively. Heart rate and blood pressure in each minutes for the 10 minutes after intubation was recorded. Time span around intubation up to 4 minutes has been looked specifically to isolate the effect of the study drugs at the time of intubation. For statistical analysis Student's 't' test was used for comparing means of quantitative data and chi-square test was used for qualitative data. Difference was considered statistically significant if p<0.05. The mean heart rate, systolic, diastolic, and mean blood pressure, and rate-pressure product before starting anesthesia were similar in group-L (Lignocaine group) and in group-E (Esmolol group) (p>0.05). The mean values of heart rate, systolic, diastolic, and mean blood pressure, and rate-pressure product at 2, 3 and 4 minutes after intubation were significantly lower in group-E than group-L (p<0.05). In conclusion, esmolol 1.5 mg/kg is superior to lignocaine (1.5 mg/kg) for attenuation of haemodynamic response to laryngoscopy and endotracheal intubation. Objective: To compare the effect of 0.5% and 0.75% hyperbaric Bupivacaine on haemodynamic stability in terms of mean systolic blood pressure and heart rate recorded at 4 min in patients undergoing caesarian section in spinal anesthesia. Study Design: Quasi experimental study. Place and Duration of Study: Department of Anaesthesiology, Combined Military Hospital, Malir, from Jul to Dec 2018. Methodology: The patients were assigned in two groups (A and B) using lottery method. Group A received 0.5% hyperbaric Bupivacaine solution. Group B received 0.5% hyperbaric Bupivacaine solution. Spinal anaesthesia was given, blood pressure and heart rate were recorded. Data were analyzed in SPSS version 23. Both groups were compared for mean systolic blood pressure and heart rate by using independent sample t-test. Results: The mean age of patients was 29.62 ± 6.21 years in 0.75% Bupivacaine group while 29.31 ± 6.20 years in 0.5% Bupivacaine group. The mean systolic blood pressure of patients was 111.63 ± 5.96 mmHg in 0.75% Bupivacaine group while 117.16 ± 7.12 mmHg in 0.5% Bupivacaine group. The difference was significant in both groups (p-value <0.05). The mean heart rate of patients was 92.27 ± 4.71 beats per min (bpm) in 0.75% Bupivacaine group while 97.68 ± 4.58 bpm in 0.5% Bupivacaine group. The difference was significant in both groups (p-value <0.05). Conclusion: 0.5% hyperbaric Bupivacaine was better than 0.75% hyperbaric Bupivacaine solution in spinal anaesthesia during caesarean section.

AbstractIntroduction The purpose of the six-minute walk test (6MWT) is to evaluate cardiopulmonary capacity using a low-cost test that is easy to administer, generally well tolerated by different populations and reflects one’s performance on activities of daily living. However, few studies have been conducted to determine the difference between performing the 6MWT indoors and outdoors.Objective The aim of the present study was to compare the distance covered on the 6MWT performed indoors and outdoors and evaluate the following physiological variables: heart rate, blood pressure and the subjective sensation of shortness of breath, using the Borg perceived exertion scale.Materials and methods A prospective, randomized, clinical trial was conducted involving eight healthy females not engaged in regular physical activity, with mean age 23.75 ± 1.67 years. Each subject performed the 6MWT indoors and outdoors with a 30-minute interval between tests. The order of the tests was determined randomly.Results The mean distance traveled was 578 ± 50.07 m on the outdoor trial and 579.95 ± 45.35 m on the indoor trial (p = 0.932). The mean physiological variables were 82.25 ± 11.02 bpm (indoors) versus 84.38 ± 9.42 bpm (outdoors) for heart rate, 121.88 ± 10.28 mmHg (indoors)versus 118.75 ± 19.40 mmHg (outdoors) for systolic blood pressure, 81.88 ± 9.74 mmHg (indoors) versus 80.50 ± 7.89 mmHg (outdoors) for diastolic blood pressure and a mean score of 12 on the perceived exertion score in both environments.Conclusions The present data demonstrate no differences in the distance walked on the 6MWT or the physiologic variables of participants between the indoor and outdoor trials.

Background and objective: We aimed to investigate the effect of yoga on cardiovascular autonomic reactivity in essential hypertensive patients. Methods: The study was conducted on 14 essential hypertensive patients, who were on salt-reduction and similar antihypertensive drugs. They were randomized into two groups; control (n=7; age 42.2±11.9 years) and yoga (n=7; age 44.9±10.8 years). The yoga group practiced yoga for ½ h/d, 6 d/ week for 6 weeks. The control group did not practice any type of yogic exercises or relaxation techniques. Autonomic function tests consisting of deep breathing, Valsalva maneuver, handgrip (HGT), and head-up tilt (HUT) tests of all patients were assessed at 0 week and every two weeks for 6 weeks. The data were analyzed using Friedman test followed by multiple comparisons. Results: In yoga group significant reduction in resting heart rate, SBP, and DBP were found after 4 weeks of yogic practices as compared to baseline recording; HR [84(78-94.5) Vs 72(67.5-77); p=0.028], SBP[144(140-165) Vs 130(125- 144); p=0.018], and DBP[98(94-101) Vs 88(78-90); p=0.018]. Similarly, significant reduction in SBP was found after yogic practices in response to HGT and HUT although the magnitude of changes was increased or comparable to control. There was significant increase in Valsalva ratio after yogic practices [1.17(1.12-1.4) Vs 1.33(1.29-1.55); p=0.018]. Conculsion: Yoga combined with anti-hypertensive drugs is effective in reducing BP in resting condition and increasing parasympathetic reactivity. It is also found to normalize cardiovascular autonomic function in hypertensive patients. Keywords: Yoga; essential hypertension. Abstract Introduction: Massage can be defined as the rhythmic and smooth manipulation of body tissues, with the aim to promote health and well-being. Objective: To assess the influence of classic massage on cardiac autonomic modulation. Methods: Cross-sectional study that evaluated healthy participants, with mean age between 18 and 25 years, divided into two groups: test group (TG, n=11) and control group (CG, n=10). The TG had their heartbeat recorded for 5min before receiving a classic massage for 40min and during three periods after this procedure: 0-5min, 5-10min and 10-15min. The CG had their heartbeats recorded at the same time; without receive massage. Cardiac autonomic modulation was investigated by heart rate variability (HRV). Results: The mean values of HRV rates were: pNN50, respectively, for the TG: before massage (10.5 ± 9.5%), and after massage: 0-5min (11.6 ± 7.2%), 5-10min (12.1 ± 8.0%) and 10-15min (11.1 ± 7.9%), with no significant statistical difference. The same result was found for the mean values of rMSSD index of the TG; before massage: 52.1 ± 46.2 ms, and after massage: 0-5min (50.0 ± 21.6ms), 5-10min (52.0 ± 27.4 ms) and 10-15min (48.2 ± 21.1 ms). Also, the values of LFnuand HFnu indexes did not change significantly before and after massage, and they were not statistically different from the values presented by the control group. Conclusion: The study results suggest that one session of classic massage does not modify cardiac autonomic modulation in healthy young adults.

AbstractBackgroundThe vital signs reflect the physiological state of patients in various clinical conditions. The purpose of this study was to compare the effects of cold compress and Xyla-P cream on hemodynamical changes during venipuncture in hemodialysis patients.Methods and MaterialIn this clinical trial study, 50 patients under hemodialysis were selected by simple random sampling. The patients were then randomly assigned to either Xyla-P cream, cold compress or placebo groups. The vital signs (blood pressure and pulse) were measured upon two intermittent hemodialysis sessions before and after venipuncture. Data were analyzed using repeated measures analysis of variance.ResultsThe mean alternation in systolic blood pressure was significantly different comparing the placebo and cold compress groups before and after intervention (p<0.001). However, the difference was not significant between the Xyla-P cream group and either placebo (p=0.402) or ice compress (p=0.698) groups. The difference of the mean diastolic blood pressure was significant comparing the placebo group with either the Xyla-P cream group (p=0.003) or cold compress group (p<0.001) before and after intervention. In addition, there was a significant difference in the mean number of heartbeats comparing the control group with either the Xyla-P cream group (p<0.001) or cold compress group (p<0.001) before and after the intervention.ConclusionsConsidering the beneficial effects of ice compress and the Xyla-P cream on reduction of cardiovascular parameters, it is recommended to use these methods in hemodialysis patients during venipuncture.

ABSTRACT Objective To evaluate the difference in orthodontic root resorption between root-filled and vital teeth. Material and Methods Sixteen individuals who required bilateral premolar tooth extraction due to orthodontic treatment and had a previously root-filled premolar tooth on one side were included in the study. The experimental group consisted of root-filled premolar teeth, and the control group consisted of contralateral vital premolar teeth. A 150-g buccally directed force was applied to these teeth using 0.017 × 0.025-inch TMA cantilever springs. The premolars were extracted 8 weeks after the application of force. Images were obtained using micro–computed tomography. Resorption measurements were obtained using the Image J program. Results The mean values for resorption were 0.08869 mm3 for the root-filled teeth and 0.14077 mm3 for the contralateral teeth, indicating significantly less resorption for the root-filled teeth compared with the contralateral teeth after the application of orthodontic force (P = .003). In both groups, the most resorption was seen on the cervical-buccal and apical-lingual surfaces. The mean resorption value of the cervical region was 0.06305 mm3 in the control group and 0.0291 mm3 in the experimental group, and the difference was statistically significant (P = .002). Conclusions Root-filled teeth showed significantly less orthodontic root resorption than vital teeth.

Fifty-five determinations of cardiovascular functions were made on eight dogs under chloralose-urethane anesthesia and forty-one on six dogs under 21-hydroxypregnane-3,20 dione sodium succinate (Viadril). No preanesthetic medication was used. Cardiac output was determined by the dye dilution technique, using the cuvette oximeter. Blood pressure was determined by strain gauge. The mean values found under chloralose-urethane were: cardiac index 4.08, mean blood pressure 146 mm Hg, peripheral resistance 5709 dynes/sec/cm–5, heart rate 149 beats/min. and stroke index 27 cc. The mean values found under 21-hydroxypregnane-3, 20 dione sodium succinate were: cardiac index 5.15, mean blood pressure 104 mm Hg, peripheral resistance 3997 dynes/sec/cm–5, heart rate 172 beats/min. and stroke index 29 cc. When successive determinations were made within a short period of time, the cardiac index and stroke index increased, peripheral resistance decreased and blood pressure and heart rate remained fairly steady.
