Introduction. The experience of organization of medical activity in the medical institutions subordinate to the FMBA of Russia in the Siberian Federal District during the I-III epidemic waves of COVID-19, the procedure for making and implementing organization and managerial decisions on building the health care system; the analysis of the experience gained in the formation and operation of consolidated mobile emergency response teams was made.Materials and methods. The study was performed on the basis of the statistical data, regulations on the organization of medical care for patients diagnosed with COVID-19, including medical care provision in the extreme conditions. Results and Discussion. The rational distribution of infectious diseases hospitals, human resources and equipment made it possible to create bed capacity reserve, as a result of which, even during the peak episodes of the pandemic process, the average occupancy of beds in the serviced area was no more than 92-93% and there was always the possibility to receive new patients in hospitals; the capacity of the laboratory was gradually increased 25 times: from 200 tests per day in the pre-pandemic period to 5,000 tests per day. Thanks to the introduction of the new management solutions, digitalization of medical processes has been ensured: accounting of results in the medical information system, transmission of results directly to medical institutions within 12-14 hours, which is 4 times faster than the standard time. The legal regulations of interaction with the rescue centres of the Ministry of Emergency Situations of Russia, the Ministry of Defense and other power structures, the improvement of the material and technical equipment of basic mobile hospitals and emergency medical teams were proposed for discussion.Conclusion. A scheme has been worked out for the involvement of the MOF of the Federal Medical and Biological Agency of Russia in the timely organizational measures for mobilization of forces and means in order to ensure the re-profiling of medical activities to work on providing medical care in the COVID-19 pandemic conditions.

Aim research: to develop and implement a patient registration register after relaparotomies in operation on the basis of a clinical oncological dispensary. The premise of this study was the need to obtain statistical data to analyze the effectiveness of vacuum therapy during relaparotomies. By automating the process of recording relaparotomy data, it will be possible to analyze the impact of various factors on patient mortality. Designed the interface and database structure. Based on the described methods and tools, a specialized module of the medical information system for recording relaparotomies has been developed. Since the development took into account all the requirements of medical specialists in a specific field of knowledge, it was possible to develop a specialized software tool that can be used to obtain specific statistics. A patient registration register after relaparotomy was developed and implemented as a specialized module of the medical information system. The register collects, analyzes and interprets data on surgical methods used, including the use of vacuum therapy. To achieve the goal set in this work, the methods and tools presented in the article were used, which were applied in practice and can be similarly used in other medical institutions.

Developed medical information system (MIS) that enables you to automate and organize the processing of clinical database in medical institutions (MIS). In a study using IIAs analyzed data of dynamic observation of patients with various types of exudative edematous macular degeneration on the background of peripheral uveitis 376 people (406 eye). For professional computer analysis of the data used in the statistical package SPSS for Windows», which enabled to evaluate the effectiveness of a new method of treatment of patients with age-related macular degeneration.

The purpose of the study was to analyze the state of satisfaction of the population in the provision of medical services and to observe the changes taking place in this area in connection with the start of the national project “Lean Polyclinic”. The studies were conducted in the form of a questionnaire survey of patients of two medical institutions, namely: the Regional Public Health Institutions “Children’s City Hospital No. 2” and “B. I. Alperovich City Clinical Hospital No. 3”.The material of the study was the responses on paper of the interviewed patients. The average indicators of patient satisfaction with the received medical services were determined by means of statistical calculations. The survey was conducted at the start of the project in May‑July of 2017 and again in October‑December of 2017.Results. The initial survey revealed the presence of such problems as insufficient number of automated workplaces; heavy workload of the doctor when working in the Medical Information System of the Tomsk Region; imperfection of the card filing cabinet; insufficient number of racks for storing outpatient cards, front-office and back-office were located in the same room and did not have a partition, which created noise and interfered with the work of call-center operators; the need to contact the registry to get the outpatient card when the visit was previously assigned; difficult orientation of patients in the clinic; long-term preventive examination of children aged 1 year; mixed flows of healthy and sick patients; and lack of personnel. The repeated survey reflected the changes that occurred in connection with the start of the “Lean Polyclinic” project: the number of detected violations tended to decrease, which indicated the improvement in the quality of services and the improvement in the organization of medical personnel work.Conclusion. Thus, the present study established that the use of lean production technologies contributed to the quality of medical services and it had a beneficial effect on the processes of organizing the work of medical institutions.
