Implementing proactive steps to control workplace hazards is the key to an effective Injury and Illness Prevention Program (IIPP). A model for an IIPP that incorporated error reduction techniques was developed by Rutgers through an OSHA Susan Harwood grant. A “user’s manual” outlining various strategies, techniques, and materials necessary to successfully administer and facilitate a sustainable IIPP and a one-day training course to assist practitioners implement the program, was developed as part of this program. A follow-up study with the 118 participants who attended the training showed that they were able to more effectively organize onsite safety committees, engage workers in the Job Hazard Analysis (JHA) cycle, and develop peer mentorship programs. Latent issues including the lack of resources (time, staff, and funds) were seen as impediments to effective program deployment.

This chapter focuses on conducting worksite investigations, including walkthrough surveys, and provides occupational health and safety personnel, employees, and employers the opportunity to identify and assess current workplace conditions and employee health concerns and make recommendations on how to reduce or eliminate any identified workplace hazards. The methods described cover ways to implement corrective actions necessary for preventing future adverse incidents and to identify shortcomings in safety and health management programs. Various specific examples are provided. The National Institute for Occupational Safety and Health Hazard Evaluation Program, which has experience with all types of workplace hazards, is described. Preparing for and conducting workplace investigations is described in detail.

Health management systems played a crucial role in hazard reduction and prevention as well as in improving employee performance. The importance of a hospital health and safety management system and there has been no study in Indonesia with the same objective based on the OSHA indicator. The purpose of this study is to evaluate the performance indicators of Hospital Safety and health management systems in hospitals in the Gombong region, Indonesia, in terms of a nurse perspective. The research method used was descriptive-analytic. The sample size consisted of 450 nurses. The questionnaire used a standard questionnaire from hospital safety and health management systems which consisted of 53 questions including 6 domains; Leadership management, Hazard identification, employee participation, Hazard prevention, evaluation and improvement program, and Education and training. Using the 1-4 Likert scale of strongly disagree, disagree, agree, and strongly agree. The highest score was 4 and the lowest score is 1. Data analysis used SPPS, t-test, and analysis of variance (ANOVA) with a significant value 0.05. Based on the research results, poor conditions occurred in several domains except in the domain of employee participation. There were significant variables between hazard identification and gender, hazard prevention and improvement program and evaluation also. Conclusion of this study, hospital administrations need to be improved and pay more attention to occupational safety and health in the workplace. To get rid of weaknesses and a mistake, we need to continuous improvement and effective evaluation of staff in training programs.

As an institution that is committed to implementing an Occupational Safety and Health Management System (SMK3) in accordance with Government Regulation Number 50 of 2012, the PLN Education and Training Implementation Unit (UPDL) Semarang continuously makes continuous improvements both to work processes, infrastructure, competence of personnel, work methods, and other resources that can support the achievement of Occupational Safety and Health (K3) performance and Company performance. One of the tools that can help smooth OSH monitoring and reporting activities at PLN is an information technology-based application called Inspekta. The purpose of this study is to determine and evaluate the use of information technology in relation to unsafe conditions, unsafe behavior (unsafe action), near misses, accidents and occupational diseases. The research method used in this research is observation, interviews and data analysis (documents). From the results of the research that has been done, it is found that the Inspecta application provides benefits in building awareness and monitoring as well as OHS reporting. The Inspekta application still needs to be optimized and developed again to make it more efficient and effective.
