This case report describes the positive outcome of a therapeutic intervention that integrated an intensive, residential component with follow-up telepractice for a 21 year old male who stutters. This therapy utilized an eclectic approach to intensive therapy in conjunction with a 12-month follow-up via video telepractice. The results indicated that the client benefited from the program as demonstrated by a reduction in percent stuttered syllables, a reduction in stuttering severity, and a change in attitudes and feelings related to stuttering and speaking.

A urinary tract infection is the most common bacterial infection met in day to day practice. It is more common in females than males due to shorter urethra which allows the bacteria quick access to the bladder. In Ayurveda symptoms of UTI has close resemblance with Mutrakruchha. Acharya Charaka has described eight types of Mutrakrichha. In Mutrakrichha, the vitiated Pitta dosha along with Apana Vayu reaches the Vasti (bladder) and afflicts the Mutravaha Srotas due to which the patient feels painful and burning micturition. In this case report a 45 years old female patient presenting with symptoms of urinary urgency, increased frequency and burning micturition on and off since 10 years was diagnosed as a case of recurrent UTI, managed effectively with chandraprabha vati, gokshuradi guggulu shwetparpati for 21days and uttarbasti procedure for 3days. The assessment were done before and after treatment showed significant changes in sign, symptoms and urine examination report. No recurrence of UTI was noticed in follow up visit. Ayurvedic management offers a good approach to manage Mutrakrichha.

Purpose: This report describes the development of focal choroidal excavation (FCE) and recurrent central serous chorioretinopathy (CSCR) following the treatment of choroidal neovascularization (CNV) years earlier. Methods: A case report is presented. Results: A 30-year-old man previously treated for an active CNV returned several years later with subacute metamorphopsia. Optical coherence tomography and angiography demonstrated no recurrence of the CNV but instead found an FCE and associated CSCR in its place. Conclusion: Longitudinal follow-up with multimodal imaging demonstrated FCE with recurrent CSCR as possible sequelae of treated CNV.

The authors describe a rare complication of ureteral stenting is the case study of a patient admitted to the hospital for congestion in the outlet system of both kidneys due to external ureteral compression by tumorous mass in the retroperitoneum. Histology confirmed the B-lymphoma of the retroperitoneum as a cause of the patient‘s problems. The ureteral stent was perforated in the course of inserting the stent into the ureter and the end of the splint was introduced into the inferior vena cava. The patient was asymptomatic, and this complication was detected as late as on day 12 on the follow-up CT scan. Stent extraction was without complications and without bleeding.

Chronic spontaneous urticaria (CSU) is a benign skin disorder usually responsive to treatment; however, at times it can be difficult to control and become very debilitating. We discuss the case of a woman with CSU that was unresponsive to H1-antihistamines who was treated with omalizumab and became pregnant during omalizumab treatment. We also considered the follow-up of the mother and newborn for 4 years after delivery. Our case report confirms that omalizumab is a safe and effective therapeutic option, after careful evaluations in terms of cost-effectiveness, in pregnant and lactating women with severe chronic urticaria. Assessment throughout follow-up confirmed a regular progression of pregnancy parameters and no adverse reaction was documented in the child from birth to 4 years of age.

Cyclic vomiting syndrome is an insufficiently understood disorder which manifests itself in stereotypical episodes of vomiting with no detectable organic cause. Considering its unknown aetiology, drugs borrowed from various medication classes are applied in the therapy of this disorder, with variable success. Among other medicaments, erythromycin is also used in treatment of cyclic vomiting syndrome. This is a case study in which the application of erythromycin led to the prevention of attacks of cyclic vomiting syndrome. Our case report presents how periodical erythromycin therapy in two-week intervals at expected attack periods in a girl led to disappearance of cyclic vomiting. Adverse effects of erythromycin did not show up.

Osteochondromas are benign bone tumors that arise from divergent cartilage formation most commonly in childhood versus adulthood. We report the case of a 42-year-old healthy female who presented with a unusual solitary posterolateral ankle mass with associated pain and ankle impingement with 6 weeks follow up. The patient was successfully treated with open surgical excision with bone with pathology diagnosis of benign osteochondroma. The patient returned to normal baseline function with no pain at 6 weeks follow up. Open posterior ankle incision approach performed to remove suspicious enlarged bony growth from posterior talar process sent to pathology. Pathology report returned benign osteochondroma of the posterior talar process and patient subsequently had routine healing of post op incision site and return to full function without pain or disability at 6 weeks follow up. This case study adds to the current understanding, incidence, occurrence, and treatment of rare osteochondromas occurring in the posterior talar process.

We report a unique case of the total loss of the intermediate cuneiform by posttraumatic avascular necrosis resulting from a left foot open fracture and dislocation of the navicular bone and medial and intermediate cuneiforms at the Chopart and Lisfranc joints in a 64-year-old woman. The injury was managed with open reduction and internal fixation with Kirschner wires and cannulated screws. During postoperative follow-up, we observed avascular necrosis of the intermediate cuneiform and the total loss of the bone. An extensive English literature search revealed only one case report published on this topic. Thus, we provide this case study to help guide clinical decision making in the future.
