Health problems in a population registered at the family health strategy: the community health agent as the key informant

OBJECTIVE: To analyze the strengths and limitations of the Family Health Strategy from the perspective of health care professionals and the community. METHODS: Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was used to evaluate the Family Health Strategy (ESF) with 77 healthcare professionals and 293 caregivers of children under five. Health care professional training, community access to health care, communication with patients and delivery of health education and pediatric care were the main points of interest in the evaluation. Logistic regression analysis was used to obtain odds ratios and 95% confidence intervals as well as to assess the statistical significance of the variables studied. RESULTS: The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caregivers and professionals identified similar weaknesses (services not accessible to the community, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caregiver satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caregivers reported being generally satisfied with the ESF services. CONCLUSIONS: Identifying the limitations and strengths of the Family Health Strategy from the healthcare professional and caregiver perspective may serve to advance primary community healthcare in Brazil.

Resumo: Objetivou-se refletir e discutir sobre o papel potencial do enfermeiro no enfrentamento do problema da tuberculose junto ao Agente Comunitário de Saúde no Programa de Controle da Tuberculose. Trata-se de pesquisa descritiva, de abordagem quantitativa, com desenho de estudo transversal. A amostra constituiu-se de 121 Agentes Comunitários de Saúde (ACS) da Estratégia Saúde da Família (ESF) - zona urbana de Teresina. Evidenciou-se que 71,1% dos sujeitos admitiram procurar o enfermeiro da ESF em caso de dúvidas frente a casos suspeitos de tuberculose. The aim of this study was to evaluate the knowledge of Community Health Agents (CHA) about the oral health/disease process, and compare the results between those who work in Family Health Teams (FHT), with and without Oral Health Teams (OHT). The participants in this study were 162 subjects, representing 66% of the total number of CHA in the municipality, and 81 of them are connected with Family Health Units (FHU) with OHT, and 81 CHS with FHUs without OHT. Data were collected from October to December 2013, by application of a questionnaire, containing personal and professional data, and 12 validated questions about knowledge of the oral health/disease process. The Mann-Whitney test was applied to verify differences between the groups. Statistically significant differences were verified between the groups of CHA when the total scores of questions about the oral health/disease process were analyzed (p < 0.0021). It was concluded that knowledge about the oral health/disease process, of the CHAs connected with a FHU with an OHT was better when compared with that of CHAs connected with a FHU without an OHT.

ABSTRACT Objectives: To identify social and environmental determinants of health in the perception of community health agents, and to develop a health education process to contribute to the improvement of health promotion. Methods: A qualitative action research based on Freire's Research Itinerary (Culture Circles), using talking maps and integrated panels, with 29 community health agents of the Paraisópolis II Basic Health Unit, in the community of Paraisópolis, Sao Paulo. The analysis of the results was carried out by the Method of Triangulation. Results: Sanitation, housing, behaviors, health services, and other social facilities were identified, which, along with the representation of blaming, were codified and decoded by the integration of technical and traditional knowledge. Conclusions: Knowledge, values and abilities related to social and environmental determinants and their applicability in the Family Health Strategy, as well as the importance of community strengthening for health promotion and for coping with health vulnerabilities and inequities, were critically unveiled.

O Agente Comunitário de Saúde (ACS) é o principal elo entre a comunidade e a Unidade de Saúde, no território onde está inserido. Sua dinâmica laboral pode gerar sobrecarga emocional e física que podem impactar negativamente na sua saúde, tornando-se necessária a realização de ações direcionadas à saúde desse trabalhador. Diante disso, o objetivo do presente trabalho é relatar a experiência de sessões de Ginástica Laboral (GL) com ACS de uma Unidade de Saúde da Família no município de Itabuna-BA. As sessões de Ginástica Laboral promoveram bem-estar, valorização da saúde do trabalhador e interação grupal. Ações continuadas de promoção à saúde são imprescindíveis para esses profissionais, que muitas vezes têm sua saúde negligenciada pelo serviço de saúde.      Palavras-chave: ginástica laboral; promoção à saúde; agente comunitário de saúde; saúde do trabalhador; unidade de saúde da família.     Abstract The Community Health Agent (CHA) is the main link between the community and the Health Unit, in the territory where it is inserted. Their work dynamics can generate emotional and physical overload, which can negatively impact their health, making it necessary to carry out actions directed to the health of this worker. Therefore, the objective of the present work is to report the experience of Labor Gymnastics (GL) sessions with CHW from a Family Health Strategy, in the municipality of Itabuna- BA. The Labor Gymnastics sessions promoted well-being, valuing the health of the worker and group interaction. Continued actions to promote health are essential for these professionals, who often have their health neglected by the health service. Objetivo: Descrever a experiência vivenciada por enfermeiras em conexão com Agentes comunitários de saúde para enfrentamento local da pandemia COVID-19. Método: Relato de experiência desenvolvido a partir de ações de enfermeiras que atuam na Estratégia Saúde da Família do município de Icó, Estado do Ceará, destacando o importante papel que os Agentes comunitários de saúde desenvolvem junto à população nesse cenário. Resultados: A partir da experiência vivenciada, pode-se observar o importante papel que os Agentes Comunitários de Saúde têm apresentando frente à pandemia da COVD-19, mesmo apresentando medo e insegurança. Evidenciou-se ainda durante esse período que seu papel de educador em saúde é imprescindível nas ações desenvolvidas dentro dos territórios. Além disso, esses profissionais têm sido protagonistas importantes nas atividades realizadas dentro das Estratégias de Saúde da Família, fortalecendo assim o trabalho da equipe bem como o controle e combate da infecção pelo novo Coronavírus. Conclusões: Contar com o trabalho desses profissionais nos territórios tem sido estratégico e representa um diferencial para o cuidado, por auxiliarem no controle da disseminação e no monitoramento de grupos de risco, bem como das pessoas infectadas pela doença.Descritores: Enfermagem; Atenção Primária à Saúde; Agente Comunitário de Saúde; Infecção pelo Coronavírus. COMMUNITY HEALTH AGENTS IN FRONT OF COVID-19: EXPERIENCES WITH NURSING PROFESSIONALSObjective: To describe the experience lived by nurses in connection with community health workers to face the pandemic COVID-19 locally. Method: Experience report developed from the actions of nurses working in the Family Health Strategy in the municipality of Icó, State of Ceará, highlighting the important role that community health agents play with the population in this scenario. Results: Based on the experience, it is possible to observe the important role that Community Health Agents have playing in the face of the COVID-19 pandemic, even with fear and insecurity. It was also evident during this period that his role as a health educator is essential in the actions developed within the territories. También fue evidente durante este período que su papel como educador de salud es esencial en las acciones desarrolladas dentro de los territorios. Además, estos profesionales han sido protagonistas importantes en las actividades llevadas a cabo dentro de las Estrategias de salud familiar, fortaleciendo así el trabajo del equipo y controlando y luchando contra la infección por el nuevo Coronavirus. Aim: to analyze the scientific and popular knowledge of nurses and community health agents (ACS – from the Portuguese Agentes Comunitários de Saúde) of  the  Family  Health  units  of  the  State  of  Rio  de  Janeiro.  Method:  study  from  a hermeneutic-dialectic  perspective,  where  16  nurses  were  interviewed  and  17  ACS  of Health  Family  Units  participated  in  focal  groups  from  October/2014  to  March/2015. Results  and  discussion:  professionals  use  popular  and  scientific  knowledge  in  health care,  although  not  always  in  an  articulated  way  to their  professional  practice. Some  of them  do  not  recognize  popular  knowledge  as  legitimate  knowledge  with  power  to dialogue and transform their professional practices in dialogue with scientific knowledge. Conclusion:  the  articulation  among  the  forms  of  knowledge  is  presented  as  a  powerful path in the perspective of development of health practices more complete and efficient among nurses and ACS in Family Health Strategy.
