A Figurational Approach: Views of Communication and Awareness of Death and Dying in Ireland

Open awareness and communication in death and dying have become important aspects in caring for people who are terminally ill. This change began in the 1960s and has been driven by social and ideological factors, especially in highly individualistic societies such as the US and the UK. While this is the preferred interaction within palliative care, open awareness and communication about death and dying can be resisted in some societies where families seek to protect their relative from the truth of a terminal illness. This article considers some attitudes to open awareness and communication in Ireland from recent research and a popular radio talk programme. This suggests that while there is openness about issues of mortality in Ireland, there can be resistance when terminal illness becomes a reality within a family. Elias's figurational approach is utilised to understand these differing responses.

Chapter 5 traces how free market ideology displaced the apparent consensus on economic regulation that emerged from the Depression, the New Deal, and the Second World War. Viewed as cranks within economics through the 1960s, Milton Friedman and his supporters built an apparatus of ideas, publications, students, think tanks, and rich supporters, establishing outposts in Latin America and the UK. When developed economies faltered in the 1970s, Friedman’s neoliberal doctrine was ready. With citizens, consumers, and workers feeling worked over by monopolies, inflation, unemployment, and taxes, these strange bedfellows elected Reagan in the US and Thatcher in the UK and rolled to power in academia and in public discourse with a doctrine of privatization, liberalization, and deregulation. Friedman, Eugene Fama, and James Buchanan whose radical free market views triumphed at the end of the 1970s are profiled. A technical appendix, “Skeptics and Critics vs. True Believers” explores the economic debates.

Abstract Objective Dying is mostly seen as a dreadful event, never a happy experience. Yet, as palliative care physicians, we have seen so many patients who remained happy despite facing death. Hence, we conducted this qualitative study to explore happiness in palliative care patients at the University of Malaya Medical Centre. Method Twenty terminally ill patients were interviewed with semi-structured questions. The results were thematically analyzed. Results Eight themes were generated: the meaning of happiness, connections, mindset, pleasure, health, faith, wealth, and work. Our results showed that happiness is possible at the end of life. Happiness can coexist with pain and suffering. Social connections were the most important element of happiness at the end of life. Wealth and work were given the least emphasis. From the descriptions of our patients, we recognized a tendency for the degree of importance to shift from the hedonic happiness to eudaimonic happiness as patients experienced a terminal illness. Significance of results To increase the happiness of palliative care patients, it is crucial to assess the meaning of happiness for each patient and the degree of importance for each happiness domain to allow targeted interventions.

This article introduces the special number of French Cultural Studies commemorating the role of Brian Rigby as the journal’s first Managing Editor. It situates his contribution in the emergence of cultural history and French cultural studies during the rapid expansion of higher education from the 1960s in France, the UK, the US and other countries. It suggests that these new areas of study saw cultural activities in a broader social context and opened the way to a wider understanding of culture, in which popular culture played an increasingly important part. It argues that the study of popular culture can illuminate some of the most mundane experiences of everyday life, and some of the most challenging. It can also help to understand the rapidly changing cultural environment in which our daily lives are now conducted.

C.G. Jung’s work has had a noticeable impact on conceptions about death and the dying experience, as well as on the therapeutic work methods that deal with anxiety, depression or terminal illnesses. This article analyses the reception of C.G. Jung’s work in the United States during the time period 1960-80. It examines ways in which Jung’s concepts were discussed and applied by psy practitioners who worked in fields related to death and dying studies (thanatology, palliative care, suicide and near- death studies). Following an examination of Jung’s ‘Americanisation’ in the 1950s and the reception of his commentaries on death, discussed in relation to the reception of The Tibetan Book of the Dead, I will analyse four examples: 1) a psychiatric interpretation of Jung’s account of a near- death experience and its comparison with William James’ mystical states of consciousness; 2) psychedelic therapies conducted with LSD, in which ‘symbolic dying processes’ are provoked; 3) suicide studies done on suicide survivors; 4) parapsychological investigation of near-death experience. The examples show that Jung’s work was pivotal, allowing psychologists to link it to concepts and approaches to terminal illness and positive or transpersonal psychology. Within the period under consideration, Jung’s reception has to be read and understood in relation to the more general reception of James’ work, in particular his psychology of religion. Background: The increment of terminal health problems causes the demand for palliative care to increase. Nurses’ attitudes toward palliative care have critical implications on the quality of care for terminally ill patients. This study aimed to investigate non-palliative care nurses’ attitudes towards death and dying patients in palliative care in Indonesia.Method: A cross-sectional design was performed in four general hospitals, five primary health centers, and three nursing homes in Yogyakarta. Systematic random sampling was the method employed. The research instrument used Frommelt Attitudes towards Care of the Dying (FATCOD). FATCOD is a 30-item tool using a five-point Likert scale to indicate respondents' attitudes toward caring for dying patients. Attitudes had two categories, namely favorable (≥ mean score of the total score of (FATCOD) Scale) and unfavorable ( mean score of the total score of (FATCOD) Scale). Two hundred seventeen (217) non-palliative care nurses completed the FATCOD and a ten-item demographic questionnaire. The data analysis used descriptive statistics, chi-square, and linear regression.Results: The total FATCOD score achieved in this study was 111.29±9.44 (range 30–150). Concerning nurses’ attitudes towards the family members of terminally ill dying patients, the score was 39.85 (±4.12) out of a possible 50. Besides attitudes towards patient care provision, the score was 71.43 (±6.39) out of a possible 100. The nurses' attitudes towards the dying patients showed that the favorable and unfavorable attitudes had almost the same percentage. Of significance, working experience variables were associated with attitudes in the total FATCOD and patient FATCOD. Working experience and level of education were related to attitudes towards the family FATCOD.Conclusion: Non-palliative care nurses had a favorable attitude towards palliative care. The ministry of health should give awareness by providing various training, workshop, formal and informal education about PC for nurses.

End of life communication includes both verbal and nonverbal messages that transpire following a diagnosis of a terminal illness and death. The circumstances that occur at the end of life create opportunities for unique and important communication. Specifically, communication at the end of life is impacted by numerous and complicated factors: First, cultural views on death and dying often determine what is talked about, when it can be talked about, and who is included in the conversations. Second, the fears, desires, and needs of the terminally ill must be taken into account at the end of life as it is their personal end of life journey. Third, the nature of the relationships between the terminally ill and their family and friends have tremendous influence on the nature and topics of conversations that will be shared. Fourth, interactions with healthcare professionals (preferably with palliative care specialists) tend to be more task focused, emphasizing end of life decision making and comfort care for the terminally ill. Fifth, as people are tending to live longer with terminal illness and often doing so far from their family, professional caregivers and hospice volunteers are also engaging in meaningful and significant communication with the terminally ill. Communication at the end of life often determines whether or not the dying are allowed to die with dignity, with some control over their final wishes, and whether they are ultimately able to obtain some peaceful closure. Within close relationships communication at the end of life has the potential for authentic conversations that bring people closer, heal old wounds, and allow the terminally ill and close others to create some final memories and to say goodbye to one another. Communication at the end of life with health professionals has the potential for both the terminally ill and their family members to have greater satisfaction with end of life decisions and control of pain for the terminally ill, as well as better outcomes regarding grief and bereavement following the death for family members. For hospice volunteers and professional caregivers, communication at the end of life teaches the necessity and complexities of interactions at the end of life for the larger society.

The American due process revolution that began in the 1960s was slow to reach Britain. The limited power of judicial review and the removal of procedure from the academic study of law left the field oddly barren just as it sprung to life in defining the relation between citizens and the state in the US. Almost single-handedly, Professor Zuckerman sought to reframe the legal understanding of procedural order in the UK. His use of a cost-benefit matrix to define the governmental interest in a particular course of conduct, and his use of the same metric to weigh the costs to private parties and the risks of legal error were transformative. Beginning with his work on the incentives to error created by the Mareva injunction, and continuing to his crusade over the exorbitant costs of British procedure, the result was a scholarly transformation of a field largely abandoned since the time of Bentham. This chapter chronicles the efforts of one far-sighted scholar to drag British procedure into the modern era.
