Aims: The aim of this study was to investigate the prevalence of astrovirus infections and associated risks factors. Methods: A prospective study was undertaken from May 2009 to March 2010, covering the rainy and dry seasons, at the Saint Camille Medical Center in Ouagadougou, Burkina Faso. A total of 213 non hospitalized children less than 5 years of age with diarrhea were enrolled and examined for astrovirus, others enteropathogens, and clinico-epidemiological aspects. Results: Astroviruses prevalence among the enrolled children was 14.6%. Astrovirus infections were common during the cold dry season from December to February (38.7%), during the rainy season from June to September (54.8%), also during dry season in March (3.2%) and May (3.2%). Children younger than 11 months of age were most affected by astroviruses (16%). Moderate and severe malnutrition influenced more severe symptoms of astrovirus related diarrheas. Conclusion: The present study shows that astroviruses have an important role in pediatric viral-associated diarrhea in Burkina Faso. Diarrhea is more severe in malnourished children.

The New Misola consists of millet soybean, peanut, vitamins, minerals, and industrial amylase. Our objective is to demonstrate that porridge made from local grains and legumes restores the nutritional balance of malnourished children. The study was carried on 304 malnourished children aged 6–48 months including 172 girls and 132 boys from Saint Camille Medical Centre. At the beginning, these malnourished children had a WHZz-score of −3.10 and a WAZz-score of −3.85, which reflected, according to WHO, a severe malnutrition. After eight weeks of nutritional rehabilitation, a normal WHZ of −1.41 was obtained. These children recovered more than those in a similar study performed in 2006 with the old formula of Misola. This study shows that malnutrition remains a public health problem in Burkina Faso. It should be necessary that public health services and the epidemiologists work in synergy with nutritionists and “nutrigenetics” in order to combat malnutrition efficiently.

In karst regions, due to the heterogeneous features of karst medium, the characteristics of the groundwater flow turn to be of high complexity. Researchers have been seeking proper forecasting methods for karst water dynamic for many years. This paper, taking the spring in Jinan as an example, using regression analysis, analyzed the factors influencing spring water dynamic, and quantitatively evaluated the influencing coefficients of spring water level concerning rainfall, exploitation and recharge as well as the natural decay coefficient of spring water in dry seasons. The prediction model coupling multiple factors was built by investigating natural and anthropogenic factors influencing groundwater level, which could be used for forecasting dynamic of spring water in Jinan. The calculated value of model was highly coincided with the observed value. In consideration of the characteristics of uneven precipitation in Jinan, the suitable zones and volume of artificial recharge were investigated finally, which could help to sustain the spewing of Jinan springs significantly.

AbstractNow there is no clinical scale for early prediction of refractory Mycoplasma pneumoniae pneumonia (RMPP). The aim of this study is to identify indicators and develop an early predictive scale for RMPP in hospitalized children. First we conducted a retrospective cohort study of children with M. pneumoniae pneumonia admitted to Children’s Hospital of Nanjing Medical University, China in 2016. Children were divided into two groups, according to whether their pneumonia were refractory and the results were used to develop an early predictive scale. Second we conducted a prospective study to validate the predictive scale for RMPP in children in 2018. 618 children were included in the retrospective study, of which 73 with RMPP. Six prognostic indicators were identified and included in the prognostic assessment scale. The sensitivity of the prognostic assessment scale was 74.0% (54/73), and the specificity was 88.3% (481/545) in the retrospective study. 944 children were included in the prospective cohort, including 92 with RMPP, the sensitivity of the prognostic assessment scale was 78.3% (72/92) and the specificity was 86.2% (734/852). The prognostic assessment scale for RMPP has high diagnostic accuracy and is suitable for use in standard clinical practice.

Objective: To estimate the recommended lifespan of 223 peripheral intravenous accesses in pediatric services. Method: In this cohort study, we monitored the time of intravenous catheter between insertion and removal in children aged up to 15 years old in a Hospital from Bogotá-Colombia. The routine catheter observations was registered in questionnaires during nursing shifts. Survival analyses were performed to analyze the lifespan of the catheter free of complications. Results: The median lifespan of peripheral intravenous catheters without complications was 129 h (IQR 73.6–393.4 h). This median time free from complications was much lower for children ⩽1 year 98.3 h (IQR 63–141 h), than for participants aged >1 year 207.4 h (IQR 100–393 h). Catheters of 24 G (gauge) caliber had a median complication free time of 128 h (IQR 69–207 h) and 22 G calibers 144 h (IQR 103–393 h). Conclusions: In this study, 75% of peripheral indwell catheters remained free from complications for 74 h, the other extreme 25% of these patients could remain up to 393 h.

OBJECTIVES: follow-up of children exposed to oxacillin during hospitalization focusing on adverse reactions. METHODS: patients were selected from the pediatric wards of two hospitals in Fortaleza (Hospital Universitário Walter Cantídio-HUWC and Hospital Infantil Albert Sabin-HIAS) from the first oxacillin prescription with a prospective cohort study between October, 2000 and July, 2001 (HUWC) and July/2001 and March, 2002 (HIAS). Patients' follow-up was performed by daily visits to the wards and medical charts and prescription analysis. Suspected oxacillininduced adverse reactions (OxAR cases) were notified and classified according to causality and severity. Related statistic tests were completed. RESULTS: of the 130 patients exposed to oxacillin, 27 had OxAR (20.8%). Fever was the most frequent reaction (50%) followed by rash (35.7%). The majority of reactions were considered Probable, for oxacillin was the only medication involved and 92.6% of the cases had Moderate severity with the need of therapeutic interventions caused by OxAR. A significant relation between oxacillin exposure time and OxAR was determined as well as hospitalization time and the appearance of adverse reactions. Exposure time over 14 days to oxacillin was established as a risk factor for OxAR (relative risk = 5.49). CONCLUSIONS: careful administration of oxacillin in children is recommended with established treatment duration. Empiric and prolonged use must be avoided.

Abstract The timing and length of the dry season is a key factor governing ecosystem productivity and the carbon cycle of the tropics. Mounting evidence has suggested a lengthening of the dry season with ongoing climate change. However, this conclusion is largely based on changes in precipitation (P) compared to its long-term average (P ̅) and lacks consideration of the simultaneous changes in ecosystem water demand (measured by potential evapotranspiration, Ep, or actual evapotranspiration, E). Using several long-term (1979-2018) observational datasets, we compared changes in tropical dry season length (DSL) and timing (dry season arrival, DSA, and dry season end, DSE) among three common metrics used to define the dry season: P < P ̅, P < Ep, and P < E. We found that all three definitions show that dry seasons have lengthened in much of the tropics since 1979. Among the three definitions, P < E estimates the largest fraction (49.0%) of tropical land area likely experiencing longer dry seasons, followed by P < Ep (41.4%) and P < P ̅ (34.4%). The largest differences in multi-year mean DSL (> 120 days) among the three definitions occurred in the most arid and the most humid regions of the tropics. All definitions and datasets consistently showed longer dry seasons in southern Amazon (due to delayed DSE) and central Africa (due to both earlier DSA and delayed DSE). However, definitions that account for changing water demand estimated longer DSL extension over those two regions. These results indicate that warming-enhanced evapotranspiration exacerbates dry season lengthening and ecosystem water deficit. Thus, it is necessity to account for the evolving water demand of tropical ecosystems when characterizing changes in seasonal dry periods and ecosystem water deficits in an increasingly warmer and drier climate.

Figs are a remarkable food resource to frugivores, mainly in periods of general fruit scarcity. Ficus calyptroceras Miq. (Moraceae) is the only fig species in a type of dry forest in western Brazil. In this study I examined the fruiting pattern as well as fig consumption by birds in F. calyptroceras. Although rainfall was highly seasonal, fruiting was aseasonal, since the monthly proportion of fruiting trees ranged from 4% to 14% (N = 50 trees). I recorded 22 bird species feeding on figs. In the wet season 20 bird species ate figs, while in the dry season 13 did. Parrots were the most important consumers. This group removed 72% and 40% of the figs consumed in the wet and dry seasons, respectively. No bird species increases fig consumption from dry to wet season. However, a group of bird species assumed as seed dispersers largely increases fig consumption from wet to dry season, suggesting the importance of this resource in the period of fruit scarcity. The results of this study points out the remarkable role that F. calyptroceras plays to frugivorous birds, in such a dry forest, since its fruits were widely consumed and were available all year round.

SUMMARYThe effects of phosphorus fertilizer and the insecticide carbofuran on the growth and N2-fixation of Azolla pinnata and on the growth, grain yield and nitrogen uptake of intercropped rice were examined in a wet and a dry season. Treatment with phosphorus or carbofuran increased the biomass of Azolla and the amount of nitrogen fixed (nitrogen yield) in both seasons, but the response was much better in the dry season. Azolla inoculation at 1.0 t ha−1 resulted in a greater bio mass and nitrogen yield than inoculation at 0.5 t ha−1. In the dry season, a combination of phosphorus and carbofuran enhanced the growth and N2-fixation of Azolla more than either treatment alone. Carbofuran treatment slowed the rate of decomposition of Azolla, particularly in the dry season. The plant height, leaf area index and dry matter production of rice at flowering time were increased in the plots treated with phosphorus or carbofuran in the wet season and these treatments increased rice grain yield and nitrogen uptake in both the wet and dry seasons.
