A male patient aged 43yrs. came to surgery OPD Majeedia Unani Hospital Jamia Hamdard New Delhi, in September 2018 with the complain of ulcer in the right foot posteriorly along the necrosis of Tendon Achilles. He was unable to walk a distance of about 100 meter due severe pain in the ulcer and calf muscle. limping on movement was very prominent. After required investigations and local examination of wounds, patient was planned and treated by local application, and combination of Unani Medicines formulations along with leech therapy as described in texts of Unani System of Medicine. With this Unani treatment non healing ulcer completely healed without any locomotor disturbances. The condition of leg totally resolved without any complications and side effects. The incidence of non-healing ulcer is 1-2 percent in developed countries. In Indian community its prevalence reported in year 2004 was 4.8 per 1000 populations. It may increase many folds in future because of increase in number of old persons, diabetes mellitus and vascular diseases A male patient aged 63yrs. came to surgery OPD Majeedia Unani Hospital, Jamia Hamdard, New Delhi, in October 2019, with the complain of ulcer on antero-medial aspect of right foot. The ulcer was painful and movement was limited on affected side.  After required investigations and local examination of wound, treatment was started by local application of meshed papaya and combination of oral Unani drugs formulations along with leech therapy as described in texts of Unani System of Medicine. It was observed that Unani treatment of non-healing ulcer with papaya and leech therapy is an option to avoid amputation and its complications. The ulcer completely healed without any locomotor disturbances. Recovery was very fast and the condition of leg totally resolved without any complications and side effects. Gangrene is a type of tissue death caused by a lack of blood supply. In Unani system of medicine gangrene described in the term of Ghangrana/Shafaquloos the main cause of ghangrana is Sudda (obstructrion) in blood vessels results to inadequate blood supply which leads to Nuqs-e-Taghziya (nutrients) and Unani Scholars suggest many modes of treatment including Amal-e-kai for preventing further putrefaction. Eminent unani scholars used Irsal-e-alaq for skin disorders and non healing ulcer, in this case study we applied leaches on wounded toe to prevent amputation, which successfully treated in 30 days of intervention.

Abstract. Background: The purpose of this observational study is to report the six-month clinical outcomes with a new multiple stent delivery system in patients with femoro-popliteal lesions. Patients and methods: The LOCOMOTIVE study is an observational multicentre study with a primary endpoint target lesion revascularization (TLR) rate at six months. Femoro-popliteal lesions were prepared with uncoated and/or paclitaxel-coated peripheral balloon catheters. When flow limiting dissections, elastic recoil or recoil due to calcification required stenting, up to six short stents per delivery device, each 13 mm in length, were implanted. Sonographic follow-ups and clinical assessments were scheduled at six months. Results: For this first analysis, a total of 75 patients 72.9 ± 9.2 years of age were enrolled. The majority of the 176 individually treated lesions were in the superficial femoral artery (76.2 %, 134/176) whereas the rate of TASC C/D amounted to 51.1 % (90/176). The total lesion length was 14.5 ± 9.0 cm with reference vessel diameters of 5.6 ± 0.7 mm. Overall 47 ± 18 % of lesion lengths could be saved from stenting. At six months, the patency was 90.7 % (68/75) and all-cause TLR rates were 5.3 % (4/75) in the overall cohort. Conclusions: The first clinical experience at six months suggests that the MSDS strategy was safe and effective to treat femoro-popliteal lesions of considerable length (14.5 ± 9.0 cm). Almost half of the lesion length could be saved from stenting while patency was high and TLR rates were acceptably low.
