A male patient aged 43yrs. came to surgery OPD Majeedia Unani Hospital Jamia Hamdard New Delhi, in September 2018 with the complain of ulcer in the right foot posteriorly along the necrosis of Tendon Achilles. He was unable to walk a distance of about 100 meter due severe pain in the ulcer and calf muscle. limping on movement was very prominent. After required investigations and local examination of wounds, patient was planned and treated by local application, and combination of Unani Medicines formulations along with leech therapy as described in texts of Unani System of Medicine. With this Unani treatment non healing ulcer completely healed without any locomotor disturbances. The condition of leg totally resolved without any complications and side effects. The incidence of non-healing ulcer is 1-2 percent in developed countries. In Indian community its prevalence reported in year 2004 was 4.8 per 1000 populations. It may increase many folds in future because of increase in number of old persons, diabetes mellitus and vascular diseases A male patient aged 63yrs. came to surgery OPD Majeedia Unani Hospital, Jamia Hamdard, New Delhi, in October 2019, with the complain of ulcer on antero-medial aspect of right foot. The ulcer was painful and movement was limited on affected side.  After required investigations and local examination of wound, treatment was started by local application of meshed papaya and combination of oral Unani drugs formulations along with leech therapy as described in texts of Unani System of Medicine. It was observed that Unani treatment of non-healing ulcer with papaya and leech therapy is an option to avoid amputation and its complications. The ulcer completely healed without any locomotor disturbances. Recovery was very fast and the condition of leg totally resolved without any complications and side effects. Hematoma is generally defined as a collection of blood outside of blood vessels. Most commonly, hematomas are caused by an injury to the wall of a blood vessel, prompting blood to seep out of the blood vessel into the surrounding tissues. A hematoma can result from an injury to an artery, vein, or small capillary. A patient with 12x12 cm Hematoma of left gluteal region associated with severe pain and local tenderness was taken for Jalaukavacharana (leech therapy) as primary line of management.

Abstract Background The right and left lung anatomy are similar but asymmetrical. The right lung consists of three lobes, and the left lung consists of two lobes. Our study is unique because of discovering a very rare morphological feature of the left lung which has not been reported yet. By the way, we compared two different available chemical agents for pleurodesis (talc and bleomycin) according to side effects, complications, and pneumothorax recurrence. Case presentation We reported a case of bilateral primary spontaneous pneumothorax, who underwent talc slurry and bleomycin pleurodesis at right and left side retrospectively, and then complicate with left-sided recurrent spontaneous pneumothorax, so underwent open thoracotomy and was surprisingly and accidentally found to have 4 lobes and 3 fissures in left lung. Conclusion In our case report, there were one main oblique fissure and two accessory fissures which divided the lung into 4 separated lobes, and this discovery in human’s and other animals’ lung anatomy has not been previously reported. In our case study, the talc slurry was more effective in preventing spontaneous pneumothorax recurrence, but with more side effects than bleomycin. We could hypothesize that the morphological variation of the lung might affect spontaneous pneumothorax development and recurrence.

Amavata is a very painful condition in day-to-day life. It is a chronic disorder related to especially sandhisthanas (joints). It can be compared with Rheumatoid Arthritis (R.A.). In modern medicine, R.A. is treated with steroids and various NSAIDS which has multiple side – effects. In Ayurveda, Amavata is caused due to impairment of Agni (indigestion) and vitiation of Vata dosha which form ‘Aama - The untransformed, non-assimilated compo- nent in the digestive process.' Amavata is a madhyama rogamarga disease and causes severe pain, stiffness and is difficult to treat. Hence, there are chances of recurrence. For this, in Ayurveda, both Shamana and Shodhana treatments are explained very well. In this case study, the patient was dependent on steroids and her physiological functioning body was hampered. By Ayurvedic intervention (Shodhana and Shamana Chikitsa) we have effec- tively manage the disease by gradually tapering the steroids and ultimately stopped. As sedentary life style is increasing there is increase in the incidence of low backache of which one commonest condition is sacroiliitis. Sacroiliitis means inflammation of sacroiliac joint which causes symptoms as severe low backache, tingling numbness, difficulty in walking etc. In allopathy pain killers, steroidal injection in sacroiliac joint etc are the only line of treatment followed which has many side effects and gives only temporary relief. In Ayurveda Jalaukavacharana is very helpful in reducing pain and swelling. In Ashtang hruday Acharya has mentioned to do raktamokshan if patient do not get relief after snehan, swedan and other treatment. Jalauka was choosen for raktamokshan as it has many anti-inflammatory enzymes in it saliva and can be used in all patients. Three settings of Jalaukavacharana at interval of 15 days was done and results were observed.

Karna (ear), the organ of hearing is considered as one among the Navadwaras which is predominant in Akasha Mahabhuta. Karna Srava is one among the 28 Karna Rogas explained by Acharya Sushruta. The present article emphasises the importance of Sthanika Chikitsopakramas used to treat the disease Karna Srava successfully. A 24-year male patient approached Shalakya tantra OPD of SPSAMCH and RC, Arju-nabettahalli, Nelamangala with the complaints of right ear pain for 3 days associated with itching. On ex-amination, pus and fungus mass were found in the right ear and tympanic membrane was not visible. It was treated accordingly using appropriate Sthanika Chikitsopakramas like Karna Prakshalana and Karna Dhupana and Shamanoushadhis which gave complete relief to the patient. Based on the Lakshanas in the present case, Karna Srava is corelated to otomycosis which is having the similar symptoms. Otomycosis is a fungal infection of the ear canal with the symptoms of pain, itching, discharge and ear block. Any ail-ment connected with the ear should be treated as early as possible due to the importance of organ of hear-ing and also due to its delicacy and inaccessibility.

Good skin is an integral part of health. In the present era, skin diseases are becoming a major hazard for mental health more than physical harm as it attributes the cosmetic harmony. Skin diseases are the outcome of improper food habits and lifestyles. Ayurveda has described all the skin diseases under the heading of Kushta. It is further divided into Maha Kushta and Kshudra Kushta. Vipadika is a common disorder of skin mentioned under Kshudraroga characterized by fissures and cracks in the hand and feet with severe pain.1Vata Kapha Dosha is involved in this disease. The main features are Panipada Sputana along with Kandu, Thivravedana and Raga.2 It can be compared with palmoplantar psoriasis in modern science. It is an auto-immune condition, characterized by red scaly patches on palm and soles often with fissures and bleeding. Ayurvedic treatment is very effective in these conditions. A 49-year-old male patient came to KVG Ayurveda Medical College, KC, OPD with complaints of itching, scaly lesion and pain in palms and soles, for 4 months. According to Lakshanas, it is diagnosed as Vipadika and treated with Shodhana, Shamana and Rasayana Oushadis and the patient got good relief. Ayurvedic treatment is very effective in the management of skin diseases because Ayurveda removes Doshas from its root. The present case study proves the Ayurvedic management of Vipadika Kushta is very effective with promising results. Keywords: Vipadika Kushta, palmo plantar psoriasis, Shodana, Shamana, Rasayana.

The aim of the case study is to evaluate the efficacy of Leech therapy along with ayurvedic treatment in Alopecia areata (INDRALUPTA). It is prospective interventional case report on alopecia areata treated with leech therapy and ayurvedic formulations. Using a subjective evaluation, an improvement in growth of hair on bald patches over scalp region was observed. An 18-year-old female patient, resident of Delhi, presented with complaints of two bald patches (right parietal & vertex) over the scalp without any secondary changes. With these complaints, the patient visited Shalya Tantra, Outpatient department (OPD No 16) at Chaudhary Brahm Prakash Ayurved Charak Sansthan, New Delhi. She took treatment for six weeks and got complete remission in her symptoms. Line of treatment adopted was tridosha shamaka, rakta-shodhaka and raktamokashana (Leech application). Leech therapy supplemented with ayurvedic formulations have definite role in Alopecia Areata (Indralupta) especially of recent origin. A case series or pilot study may further strengthen above case report and open new treatment arena for such cases. This case report can also be used to formulate hypothesis for management of baldness from an ayurvedic perspective.

Ayurveda can provide effective and harmless relief for certain diseases which sometimes can’t be cured with modern medicine. Nimesha is a condition, pertaining to Shalakya Tantra in which eye lids starts blinking involun- tary due to influence of Vata Dosha. In this condition patients can’t be able to focus things properly and having lots of discomfort. It can be correlated to blepharospasm i.e., an abnormal contraction of eye lid muscles, which is also refers to benign essential blepharospasm. It is thought to be rare, affecting about 16–133 cases per million. There is no cure for blepharospasm in modern practice. Generally, doctors inject botox injection into eyelid mus- cles which need to get injected in every 4-6 month, rather than permanent benefits it has lots of side effects like diplopia ectropion etc. Ayurveda can be useful in the treatment of blepharospasm and its complication that it is a Vataj Vyadhi all Vatashamak therapies could be effective for its treatment. A 61 years old male patient known case of bilateral blepharospasm was visited in OPD of Government Autonomous Dhanwantari Ayurveda Hospi- tal, Ujjain. He had complained of excessive blinking with tired eye muscles, heaviness around eyes, headache, anxiety and irritation etc. for 3 years. He was on regular allopathic medicine along with botox injection around each eye which is repeated in every 6 months, but after few shots of treatment pt. didn’t receive any benefits.Therefore, he came to us and after Ayurvedic treatment patient showed marked improvement in symptomatology, with limited abnormal eye lid movement and with negligible side effects. Gangrene is a type of tissue death caused by a lack of blood supply. In Unani system of medicine gangrene described in the term of Ghangrana/Shafaquloos the main cause of ghangrana is Sudda (obstructrion) in blood vessels results to inadequate blood supply which leads to Nuqs-e-Taghziya (nutrients) and Unani Scholars suggest many modes of treatment including Amal-e-kai for preventing further putrefaction. Eminent unani scholars used Irsal-e-alaq for skin disorders and non healing ulcer, in this case study we applied leaches on wounded toe to prevent amputation, which successfully treated in 30 days of intervention.
