Backgrounds: A plug-and-play standardized algorithm to identify the ischemic risk in patients with coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI) could play a valuable step to help a wide spectrum of clinic workers. This study intended to investigate the ability to use the accumulation of multiple clinical routine risk scores to predict long-term ischemic events in patients with CAD undergoing PCI.Methods: This was a secondary analysis of the I-LOVE-IT 2 (Evaluate Safety and Effectiveness of the Tivoli drug-eluting stent (DES) and the Firebird DES for Treatment of Coronary Revascularization) trial, which was a prospective, multicenter, and randomized study. The Global Registry for Acute Coronary Events (GRACE), baseline Synergy Between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX), residual SYNTAX, and age, creatinine, and ejection fraction (ACEF) score were calculated in all patients. Risk stratification was based on the number of these four scores that met the established thresholds for the ischemic risk. The primary end point was ischemic events at 48 months, defined as the composite of cardiac death, nonfatal myocardial infarction, stroke, or definite/probable stent thrombosis (ST).Results: The 48-month ischemic events had a significant trend for higher event rates (from 6.61 to 16.93%) with an incremental number of risk scores presenting the higher ischemic risk from 0 to ≥3 (p trend < 0.001). In addition, the categories were associated with increased risk for all components of ischemic events, including cardiac death (from 1.36 to 3.15%), myocardial infarction (MI) (from 3.31 to 9.84%), stroke (3.31 to 6.10%), definite/probable ST (from 0.58 to 1.97%), and all-cause mortality (from 2.14 to 6.30%) (all p trend < 0.05). The net reclassification index after combined with four risk scores was 12.5% (5.3–20.0%), 9.4% (2.0–16.8%), 12.1% (4.5–19.7%), and 10.7% (3.3–18.1%), which offered statistically significant improvement in the performance, compared with SYNTAX, residual SYNTAX, ACEF, and GRACE score, respectively.Conclusion: The novel multiple risk score model was significantly associated with the risk of long-term ischemic events in these patients with an increment of scores. A meaningful improvement to predict adverse outcomes when multiple risk scores were applied to risk stratification.

Abstract The risk for venous thromboembolism (VTE) is considered to be low in the general paediatric intensive care unit (PICU) population, and pharmacological thromboprophylaxis is not routinely used. PICU patients considered at high-risk of VTE could possibly benefit from pharmacological thromboprophylaxis, but the incidence of VTE in this group of patients is unclear. This was an observational, prospective study at a tertiary multi-disciplinary paediatric hospital. We used comprehensive ultrasonography screening for VTE in critically ill children with multiple risk factors for VTE. Patients admitted to PICU ≥72 hours and with ≥two risk factors for VTE were included. Patients receiving pharmacological thromboprophylaxis during their entire PICU stay were excluded. Since pharmacological thromboprophylaxis has not been proven effective for central venous catheter(CVC)-related VTE, the primary outcome was VTEs not related to the use of a CVC. Ultrasonography screening of the great veins was performed at PICU discharge. Seventy patients with median (interquartile range) 3 (2-4) risk factors for VTE were evaluated. Median age was 0.34 years (0.03-4.3) and median PICU length of stay 9 days (5-17). Regarding the primary outcome, no symptomatic VTEs occurred and no asymptomatic VTEs were found on ultrasonography screening. The observed proportion of VTE, 0 in 70 patients, corresponds to a VTE incidence of 0-5.1%. Conclusion: No VTEs where pharmacological thromboprophylaxis could potentially have been an effective prophylactic measure were found. This indicates that VTE is an uncommon event even in a selected group of severely ill small children considered to be at high risk of VTE.

AbstractIn August of 2021, China imposed severe restrictions on children’s online gaming time. We argue that such a policy may seem useful on the surface but does not reflect the current evidence concerning prevention of disordered gaming. Videogame play is normal for children worldwide, and like other leisure activities can lead to benefits for the majority and problems for a minority. Problematic or disordered play results from the interaction of multiple risk factors that are not addressed by draconian policy measures. Identifying these factors through stakeholder-engaged research and current evidence will be much more likely to succeed in preventing disordered gaming and promoting youth wellbeing.

The metatheoretical ecosystem construct requires integrating the virtual dimension of the internet in the assessment of human living space. Objective: The main goal of the present study is to identify possible relationships between loneliness, risk behavior in the use of psychoactive substances of adolescents, based on the ecosystem meta-frame of living space (which also reflects virtual space), as one of the essential factors in the emergence and development of addiction. Based on this, an effort is made to compile predictive models of multiple risk behaviors concerning the use of psychoactive substances. Methods: Loneliness is determined using the OESL questionnaire (Gierveld & Tilburg, 2021). The RSi questionnaire examines risk behavior (Kahan et al., 2021 in press; Lovašová, 2021). Multiple linear regressions construct predictive models, and subcategories are subjected to correlations and parametric tests. At the same time, data on adolescents' loneliness are compared with data on the loneliness of addicted adult respondents and data on abstainers from another research. File: The representative sample consists of 744 respondents in the second and third years of secondary schools in the Slovak Republic, of which 263 are men and 481 women. The research file of addicted respondents consists of 138 respondents (100 men, 38 women), and the research group of abstainers is 94 (58 men, 36 women). Results: Statistically significant gender differences inexperienced social and emotional loneliness are found in adolescents and addicted respondents. For both male and female adolescents, excessive internet use is a significant predictor of psychoactive substance use. The prediction is more robust if, in adolescents, at least one parent suffers or has suffered from some kind of addiction. Conclusions: Aspects of virtual space act as significant risk factors which, together with loneliness, create multiple risk conditions for risk behavior in the field of psychoactive substance use.
