A 32-year-old multiparous obese woman was referred to our center at 37 weeks of twin gestation. She was referred for birth planning following an accidentally discovered high international normalised ratio (INR) in routine preoperative labs. Her history was significant for recurrent pregnancy-associated deep venous thrombosis as well as two early pregnancy losses. Further work-up revealed transaminitis, mild splenomegaly and high lupus anticoagulant titre. A multidisciplinary team of physicians from the high-risk pregnancy, anaesthesiology, haematology, gastroenterology and hepatology departments put a management plan; it culminated into uncomplicated delivery of the patient by repeated caesarian section. The team was also able to figure out the cause of the patient’s high INR that is associated with thrombophilia rather than haemophilia.

Health problems for pregnant women, both physically and psychologically, have an impact on the quality of life of the mother and the baby she is carrying. Women with high-risk pregnancies need to prepare themselves with more attention to their health conditions in facing pregnancy. Several factors that influence the independence of pregnant women in early detection of high risk pregnancies include age, developmental conditions, health conditions, cultural orientation, health care systems, family system factors or family support, lifestyle, environmental factors and available resources. The output target to be achieved in the implementation of this community service activity is to increase the understanding and skills of pregnant women about high-risk pregnancies and to be published in national journals. Efforts to increase the independence of pregnant women in detecting high risks and complications of pregnancy through the use of the  health of both mother and childbook, Skor Pudji Rochyati and birth planning and complication prevention program stickers are a form of self-awareness for pregnant women and supported by social support by their families, it is hoped that pregnant women will be fully aware of the signs and dangers that occur. on himself and checked himself into a health facility and wanted to be referred to a higher level of service precisely and quickly.

The data collection of pregnant women is the realization of the Birth Planning and Prevention of Complications (P4K) Program in the Riau Provincial Health Office of the Family and Nutrition Health Section. The data collection of pregnant women aims to find out information about pregnant women scattered in 12 regencies and cities in Riau Province including: the number of pregnant women, Tabulin, transportation, blood donors, assisting pregnant women and birth assistance plans. Currently reports and data recording of pregnant women are still done manually using Excel. To make it easier for officers to monitor the health of pregnant women in Riau Province, a Web based health monitoring system for pregnant women was built. This system integrates data on pregnant women in Riau Province starting from the Puskesmas level. This system can help officials in the Health Department monitor and record pregnant women, so that they can assist in planning a safe delivery and preparing for complications for pregnant women.

Birth planning for a couple from the first and subsequent births can be arranged so that the welfare of the community increases and other undesirable things can be anticipated from afar. The methods include using contraception or prevention of pregnancy and family planning. Sterilization is to spay a man or woman by operating (in general) so as not to produce offspring. Sterilization for men (vasectomy) and women (tubectomy). Procedurally, vasectomy in men is relatively simpler than tubectomy while the procedure for tubectomy is a bit complicated and complex.
