Abstract: The proactive maintenance of pipelines through condition based monitoring, plays an essential role in improving their overall reliability and availability. Their criticality can also be assessed by conducting reliability analysis such as FMECA, which helps in identifying relevant failure modes and averting catastrophic failures to sustain economic growth. This paper will focus on an integrity assessment set up for pipelines and the potential failure modes associated are evaluated and mitigated by determining the risk triggers. The outcome of this research has shown the various threats associated with pipelines, having an effective integrity assessment program will help mitigate such threats. The National Collegiate Athletic Association NCAA (2009) defines the term “risk” as a “student-athlete’s likelihood of graduation”. The ability of athletic departments and athletic academic professionals to identify which of their student-athletes might be considered at risk is less straightforward. While many departments use their own tools to assess risk, there has been little research on the subject. This exploratory study sought to generate a collective understanding of how athletic academic units across the country define the term “at-risk” and assess which of their student-athletes are at-risk in order to begin creating a framework for use across the industry. A survey was completed by 43 member of the National Association of Academic and Student-Athlete Development Professionals (N4A) who serve in learning specialist roles, within athletic academic units. The results showed that academic units, across NCAA institutions, do not have one clear definition of risk, but rather the definition falls into four distinct categories. The study also found that there are three initial risk assessments used most frequently to determine student-athletes’ risk upon arrival at the institution, while the continued risk assessments fall into six distinct categories of assessments. As this is an exploratory study, the researchers acknowledge that we are only scratching the surface in regards to the breadth` and depth of assessment usage to determine risk of student-athletes at NCAA institutions. Installation of precast concrete in construction of multi-storey buildings at risk of workplace accidents due to be carried out on the altitude. Installation of precast concrete must also use heavy equipment, because of the weight of the material to be install. Accidents on the installation of precast concrete can be very fatal and cause of death. Risk assessment is an attempt to calculating risk arising from hazards, take into account the adequacy of any existing controls, and deciding whether or not the risk is acceptable. The purpose of this research is to studies and perform  risk assessment level onprecast concrete installation at apartment project in Surabaya with hazard, risk, risk control, and residual risk observation. Primary data were collected through interview and observation. Object of this research is installation proses of precast concrete wall at Puncak Bukit Golf Apartment project by PT. Wijaya Karya Bangunan Gedung  in Surabaya. The data was analyzed descriptively using table and naration. Results showed that hazard identify in precast concrete installation at apartment project in Surabaya include: fall from height, squeezed,  fallen object, struck by precast concrete, crush by precast concrete, and electro shock. Risk assessment has obtained 21 hazards. Risk control with was done: installing safety net for falling objects, safety induction, work instruction, wear PPE, and wear fall protection. The result of risk assessment showed19% risk are high risk, 38% with moderate risk category, and 43% with low risk category. Keywords: risk assessment, precast concrete, installing

A project always has risks that can lead to project failure. In the project, a risk analysis is required to provide an evaluation for the project to proceed as planned. In the event of inadequate planning and ineffective control, it will result in irregularities identified as a risk to the project. This study aims to analyze the qualitative risk on Fiber Optic Installaion project in Sukabumi, West Java, Indonesia. In addition, risk assessment is undertaken on project implementation. Assessment of risk using the impact and probability to measure the impact of risk occurrence. The impacts are more detailed by classified by time impact, cost impact, quality impact, safety and security impact, proximity. The result is there are 36 risk that may occur and mostly risks are associaated by quality and safety&security impact. Keywords—  risk assessment, qualitative analysis, Project, fiber optic.

ABSTRACTPT. PAL Surabaya operates Overhead Crane Double Girder tool every day. Those have some risks, for example squeezed, material crushed, fracture and passed away. The objective of the research was to value the level of risks on kapal niagadivision PT. PAL Surabaya. The type of this research was descriptive observational research with cross sectional approach. The research focuses on overhead crane double girder operation at kapal niaga division PT PAL Surabaya, consists ofoperation phase itself. Primary data collection by doing interview and observation, it was presented in a table, analyzed, and explained in descriptive way. The result showed there were 8 potential hazards on operation phase. Controlling type that most implementated was providing personal protective equipment (PPE) as 75%. That is quite significant and effective although there were still residual risks on 7 low potential hazards on preparation phase and 1 medium potential hazards on operation phase. PT PAL Surabaya has not done measurement the level of risks and suggested to do identification in details by noticing any hazards and risks in every job phase. Besides, the operator from the whole overhead crane doublegirder operation should have K3 (SIO) license and specific experience in operating overhead crane double girder, this suitable with Permenaker No 09/MEN/VII/2010.Keywords: risk assessment, overhead crane double girder, level of risks
