<p>Our research examines the problem of multiple lines of reasoning reaching the same conclusion, but only through different and unrelated arguments. In the context of non-monotonic logic, these types of conclusions are referred to as floating conclusions. The field of defeasible reasoning is divided between those who claim that floating conclusions ought not to be accepted through a prudent or skeptical point of view, whereas others argue that they are good enough conclusions to be admitted even from a conservative or skeptical standard. We approach the problem of floating conclusions through the formal framework of Inheritance Networks. These networks provide the simplest and most straightforward gateway into the technical aspects surrounding floating conclusions in the context of non-monotonic logic and defeasible reasoning.  To address the problem of floating conclusions, we construct a unifying framework of analysis, namely, the Source Conflict Cost Criterion (SCCC), that contains two basic elements: source conflict and cost. Both elements are simplified through a binary model, through which we provide a comprehensive understanding of the floating conclusions as well as the problematic nature of the debate surrounding this type of inferences. The SCCC addresses three key objectives: (a) the assessment of floating conclusions and the debate surrounding its epistemological dimension, (b) the construction of a general and unified framework of analysis for floating conclusions, and (c) the specification of the normative conditions for the admission of floating conclusions as skeptically acceptable information.</p>

Often cognitive function is affected adversely during prolonged, high intensity exercise. We assessed whether hydrothermally modified corn starch (HMS) ingestion minimizes cognitive decline with soccer play. 11 men (177.7 ± 6.8 cm, 77.3 ± 7.9 kg, 22 ± 3 y, 12.8 ± 4.9% body fat, [Formula: see text]O2max = 57.1 ± 3.9 ml•kg BM−1•min−1; mean ± SD) completed 60 min simulated soccer matches with HMS (8% CHO; 0.7 g•kg BM−1•h−1; 2.8 kcal·kg BM−1•h−1) vs isoenergetic dextrose (DEX) consumed 30 min prior to the match and at half time. Compared to DEX, blood glucose was lower (p < 0.001) with HMS at 15 (5.3 ± 0.6 vs 7.7 ± 1.4 mmol•L−1) and 30 min post ingestion (5.6 ± 0.6 vs 8.3 ± 1.0 mmol•L−1), and greater (p = 0.004) after 15 min of play (5.8 ± 0.5 vs 5.1 ± 0.6 mmol•L−1). With HMS, perceived exertion ratings were reduced throughout the match (p = 0.025 at 15 min). Flanker test incongruent trial reaction time (p = 0.040) and conflict cost (p = 0.019) were both better with HMS. These data suggest that HMS ingestion minimizes cognitive decline during soccer play. More study is warranted to elucidate fully the cognition benefits of HMS ingestion for sport performance.

AbstractThe surgical complexities of our current population have pushed the technological limits of healthcare, urging for minimally invasive approaches. For ventral hernias, in particular, robotic assisted laparoscopic repairs have been met with conflict. Cost and longer operative times are among the arguments against robotic surgery, although thorough evaluation of patient outcomes could potentially advocate for use of this tool. We attempted to approach this by retrospectively reviewing our own data. We reviewed charts between September 2016 and February 2017 of patients receiving complex hernia repairs, either a standard open repair (SOR) or robotic-assisted repair (RAR). Data collected included preoperative, perioperative, and postoperative care. Of the 43 patients reviewed, 16 were SOR, versus 27 RAR. Patients were comparable in age, gender, BMI, diabetes as a comorbidity; average hernia defect size was similar between the two groups. Although operative times were longer in the RAR group, estimated blood loss (EBL) was less. Hospital stay was also shorter in the RAR group, at 3.0 ± 1.9 days versus 9.6 ± 8.4 days for the OAR group. Of those requiring critical care management, only one patient had a robotic assisted repair, versus half of the patients who received an open repair. Of the patients who presented to the emergency department within 30 days of surgery, each group had four patients, and two from the OAR group required admission. Our data is consistent with other literature supporting shorter lengths of stays. Although the robotic approach did required a longer operative time, the resulting improved patient outcomes support this technique for complex ventral hernia repairs.

Abstract Background Relationships between dietitians and other healthcare providers can impact the degree to which patient care is collaborative; inefficient communication can lead to suboptimal care. It takes time for multidisciplinary team members to build collaborative, trusting relationships. For this reason, frequent dietitian turnover is of concern. Consequences include fewer referrals to clinical dietetic services and limited provider continuity. The characteristics of clinical dietetic jobs associated with high turnover have not been identified. We predicted that managers would identify disease prestige as having an impact. In this study, we aimed to explore: 1) characteristics of clinical dietetic jobs associated with the highest turnover, and 2) consequences of high turnover on patients and managers of clinical dietitians. Methods Research assistants conducted semi-structured interviews with ten managers of clinical dietitians in the Canadian public healthcare system. We employed a constant comparative approach to thematic analysis. We classified themes related to turnover as either avoidable or unavoidable. Results Sub-themes under avoidable turnover included lack of manager support, growth opportunities, burnout/workload, tension/conflict and hours of work. Sub-themes under unavoidable turnover included life-stage/life-events and geography. We also identified themes related to consequences of turnover, including: burnout/workload, client/patient impact, tension/conflict, cost and gap-specific. As predicted, prestige was perceived as playing a role in triggering dietitian turnover. Managers observed high turnover resulting in low provider continuity and limiting patient access to dietitians. Conclusions Managers of publicly-employed dietitians identified many factors as contributing to high turnover. Future prospective research, incorporating the objective measure of turnover and multi-method analysis of work characteristics and work setting, would be of value in the identification of characteristics of clinical dietetic jobs associated with high turnover and the consequences of high turnover on patients and managers of these staff.

This chapter traces the history of cost–benefit analysis in the United States bureaucracy from the 1920s until about 1960. It is not a story of academic research, but of political pressure and administrative conflict. Cost–benefit methods were introduced to promote procedural regularity and to give public evidence of fairness in the selection of water projects. Early in the century, numbers produced by the Army Corps of Engineers were usually accepted on its authority alone, and there was correspondingly little need for standardization of methods. About 1940, however, economic numbers became objects of bitter controversy, as the Corps was challenged by such powerful interests as utility companies and railroads. The really crucial development in this story was the outbreak of intense bureaucratic conflict between the Corps and other government agencies, especially the Department of Agriculture and the Bureau of Reclamation. The agencies tried to settle their feuds by harmonizing their economic analyses. When negotiation failed as a strategy for achieving uniformity, they were compelled to try to ground their makeshift techniques in economic rationality. On this account, cost–benefit analysis had to be transformed from a collection of local bureaucratic practices into a set of rationalized economic principles.

Recent studies suggest that females and males show different levels of susceptibility to neuropsychological disorders which might be related to sex differences in executive control of behaviour. Music, as a cognitively salient factor, might influence cognitive functions; however, it is unclear how sex and music interact in influencing executive control of behaviour in a dynamic environment. We tested female and male participants in a computerized analogue of the Wisconsin Card Sorting Test (WCST) while listening to music or in silence. We found that music decreased the percentage of correct trials in both sexes. While music decreased response time in females, it had an opposite effect in males. Response time increased in error trials (error slowing), and music sex-dependently influenced error slowing. Conflict between potential rules adversely influenced performance in the current trial (conflict cost) in both sexes and listening to music increased conflict cost. These findings suggest that music shows both adverse and beneficial effects on various behavioural measures in the WCST, some of which are sex-dependent. Our findings suggest that in using music as an adjunct for rehabilitation of neuropsychological disorders, both adverse and beneficial effects and sex dependency need to be considered.
