The purpose of this study is to determine the socio-economic effectiveness of seasonal influenza vaccine prophylaxis among health professionals based on the analysis of the morbidity with temporal disability (MTD) and the financial benefits of this preventive measure. The study included the collection and analysis of information on MTD and direct and indirect costs of influenza treatment compared to vaccine prophylaxis in “Dnipropetrovsk Regional Perinatal Center with Hospital” Dnipropetrovsk Regional Council” (ME “DRPC with Hospital” DRC”) during 2017-2020 (3 epidemic seasons). To assess the consequences of vaccine prophylaxis, the analysis of MTD indicators was performed, and the index and coefficient of anti-epidemic effectiveness were used. The economic effect was determined on the basis of the cost-benefit method using the analysis algorithm in the form of a “decision tree”. The information base for the analysis was the accounting and reporting and financial and economic documentation of the health care institution (HCI). The analysis of influenza MTD during the observation period revealed that all studied morbidity rates (number of cases per 100 employees; number of days per 100 employees; average case duration) were statistically significantly lower in vaccinated compared to unvaccinated (p<0.001). Precautionary number of cases of MTD for influenza in the last year of observation (2019-2020) was 11.07 (95% CI 6.68-15.46) per 100 employees; days – 96.23 (95% CI 81.86-110.60) per 100 employees. The index of anti-epidemic efficiency during the study increased by 2.37 (95% CI 1.95-2.79), the coefficient of anti-epidemic effectiveness – by 15.16% (95% CI 11.17-18.01). The economic efficiency of vaccination of 69.44% of employees for one epidemic season (2019-2020) amounted to a total of UAH 248,976 or UAH 494 (56%) benefits per employee. The study proved the high medical, social and economic effectiveness of vaccine prophylaxis in HCI. Medical efficiency consists in a significant reduction in the morbidity with temporary disability, high levels and a tendency to increase the indicators of anti-epidemic efficiency. The economic effect is defined as the available monetary benefit from the vaccine prophylaxis. The resulting savings are the basis for optimizing the use of resources of the medical institution, including labor.

The costs with the safety and health of employees are essential to guarantee the integrity of the workforce and maintain the productivity of the organization, nevertheless, the procedures and bureaucracy encountered by small entrepreneurs are a challenging factor for the class supporting a Management disproportionate risk for the size of the activities. That said, the NR-1 is a standard responsible for noting general actions which employers must comply with related to the Safety and Health of their employees in the most diverse work environments. In 2019, the Standard underwent changes through Ordinance No. 915 directly reflecting MEI’s, micro and small businesses covering matters such as PPRA and PCMSO. This article seeks, through a case study, to analyze the economic impact obtained by altering the NR-1 in three companies from different sectors located in the city of Manaus. Through analysis, it was possible to prove the economic effectiveness of the standard in the companies studied, providing speed and ease in the legal processes, in addition to facilitating the managerial routine.

Abstract The aim of the present study was to evaluate the effects of reducing dietary crude protein (CP) levels and supplementing rumen-protected methionine (RPM) on production, apparent digestibility of nutrients and economic effectiveness of lactating Holstein cows. A total of 96 (60 ± 7 d in milk; 34.39 ± 5.74 kg/d of milk production; mean ± SD) lactating Holstein dairy cows were randomly assigned to 1 of 2 treatments: diet containing 17.3% CP without RPM (control group; CON); diet containing 16.4% CP with supplementing 15.0 g/d of RPM (treatment group; RPM). All the data were analyzed using one-way analysis of variance (ANOVA) of SAS (SAS version 9.2, SAS Institute Inc., Cary, NC.). No effects were observed of reducing dietary CP on somatic cell counts, milk yield and milk composition including fat, protein, lactose, total solid, solids-not-fat and milk urea nitrogen. The total apparent digestibility of nutrients was similar between treatments, while RPM group had higher calcium digestion (44.24% vs. 42.75% ± 10.13%) and phosphorus digestion (48.50% vs. 45.96% ± 12.90%) numerically. In addition, supplying the diet of 16.4% CP with RPM supplementation to cows could reduce feeding cost by 0.5 $/d per cow and boost net profits. In conclusion, lower dietary CP with RPM supplementation did not limit milk yield, milk composition and apparent digestibility of nutrients, and also could improve the economic benefits of the dairy farms.

Objective: to review clinical and economic studies on the pre-dialysis stage of chronic kidney disease (CKD) and the cost-effectiveness of medical interventions used to detect CKD and prevent its progression.Material and methods. Articles were collected from the bibliographic databases PubMed, Cochrane Library and eLibrary for 2010–2020. Keywords for the search: ‘chronic kidney disease (CKD)’, ‘cost-effectiveness’, ‘economic evaluation’, ‘economic burden’, ‘cost’, ‘healthcare’. After the selection and elimination of the repeated articles, we reviewed 36 Russian and foreign studies on the economic burden of CKD and the clinical and economic effectiveness of various medical interventions of the CKD pre-dialysis stage.Results. We found 21 studies on the economic burden of CKD. A significant part of them (n=6) was performed in the United States. The methodology for assessing the economic burden varied significantly from study to study. Studies on medical interventions at the CKD predialysis stage can be divided into three groups: Group 1 (n=9) – studies on the clinical and economic effectiveness of screening programs; Group 2 (n=3) – studies on the clinical and economic effectiveness of nephroprotectors and other drugs that slow down the course of CKD; and Group 3 (n=3) – studies on the clinical and economic effectiveness of multidisciplinary care. We concluded on the clinical and economic feasibility of most interventions that were used to detect CKD and prevent its progression.Conclusion. We did not find any scientific data that could currently serve as a justification for the clinical and economic feasibility of CKD control programs in the Russian Federation. At the same time, studies conducted in foreign countries on the screening for CKD and some interventions aimed at slowing the progression of CKD suggest that in the Russian Federation, such approaches can be justified not only clinically, but also economically. It is necessary to conduct domestic research on the clinical and economic feasibility of interventions aimed at identifying and treating CKD at an early stage.

The article discusses the establishment of the actual costs of land surveying expertise in the Russian Federation nowadays. The standard indicators of the cost in a typical land management examination for state forensic institutions of the Russian Federation were determined in this paper. The actual calculation of the cost estimate for the land management examination was made here as well by example of the division of a land plot for land share allocation in accordance with the title documents of the owner. The potential economic benefit of expert institutions (commercial allowance of organizations) was calculated in this paper as well.

This article presents main proposals on the concept of development of the Bilyar State Historical, Archaeological and Natural Museum-Reserve. The high potential of cultural and natural heritage in the territory of the Bilyar settlement and in its surroundings, which is currently not fully used, is noted. The identified heritage of the "Great City", the most important political and administrative, craft, commercial and cultural center of Volga Bulgaria, allows us to talk about it as one of the largest archaeological sites in Europe, and the Bilyar Museum-Reserve itself can become one of the interesting and promising museum centers of the Republic of Tatarstan. Both general directions of promising development of the Bilyar Museum-Reserve and specific museum and infrastructure projects are proposed. Proposals are formulated for the museumification of archaeological heritage, for the use of natural and landscape potential, for the creation of an ethnographic exposition, for the development of events' tourism, the formation of the "Craftsmen City " as well as proposals for the development of infrastructure on the territory of the museum-reserve and the Alekseevsky district of the Republic of Tatarstan. An assessment of the socio-economic effectiveness of the implementation of the intended concept was given.
