The pace-of-life hypothesis is a socio-psychological theory postulating that citizens of different cities transact the business of life at varying paces, and this pace is associated with a number of population level variables. Here we apply the pace-of-life hypothesis to a hospital context to empirically test the association between pace and patient and staff outcomes. As pressure on hospitals grow and pace increases to keep up with demand, is there empirical evidence of a trade-off between a rapid pace and poorer outcomes? We collected data from four large Australian hospitals, inviting all staff (clinical and non-clinical) to complete a survey, and conducted a series of observations of hospital staff’s walking pace and transactional pace. From these data we constructed three measures of pace: staff perception of pace, transactional pace, and walking pace. Outcome measures included: hospital culture, perceived patient safety, and staff well-being outcomes of job satisfaction and burnout. Overall, participants reported experiencing a “fast-paced” “hurried” and “rapid” pace-of-life working in the Australian hospital sector. We found a significant difference in perceived pace across four hospital sites, similar to trends observed for transactional pace. This provides support that the pace-of-life hypothesis may apply to the hospital context. We tested associations between faster perceived pace, hospital culture, staff well-being and patient safety. Results revealed perceived faster pace significantly predicted negative perceptions of organizational culture, greater burnout and lower job satisfaction. However, perceived pace did not predict perceptions of patient safety. Different perceptions of hospital pace-of-life were found between different clinical settings and the type of care delivered; staff working in emergency departments reported significantly “faster-paced” work environments than staff working in palliative, aged care, or rehabilitation wards.

The aim of this work is to explore the working conditions of logistics personnel in hospital and provide a new approach for improving the service quality of them in the post-COVID-19 era. Eighty-four logistics personnel in an upper first-class hospital were included in the study, and their working conditions were investigated and analyzed via self-designed questionnaire. 73.8% of all respondents think their work is significantly important to the brand building of the hospital, and 67.9% of them think they are closely related to a harmonious doctor–patient relationship. The compliant rate is higher in security personnel when compared with other personnel and the difference was statistically significant ([Formula: see text]). 94% of the logistics personnel indicate a higher intensity in their work, 39.3% of them constantly face the working pressure and 57.1% come from the risk of infection which accounts for the largest. The largest demand comes for the protective equipment, which is from 69 workers. It would be better for us to pay attention to the hospital culture training and strengthen the sense of identity among logistics personnel of the hospital continuously. Meanwhile we need to enrich the connotation of professional style construction in hospital and promote the efficiency of logistics service. Targeted training is necessary to improve the service capabilities of the security personnel since they receive more complaints. Diversified and personalized support to the logistics personnel based on the grasp of their special demand is also important.

Healthcare services, in particular, are one of the areas in which Lean can be applied and benefits can be reached through it. In order to analyze the suitability of Lean implementation in healthcare units, this research focused on a case study, namely in Hospital of Santo António emergency area, in the city of Porto, Portugal. The study was conducted in the year 2020, during the pandemic of COVID-19, which forced the change of screening processes. This research analyses and compares the new and previous sorting model and discusses if Lean methodology was applied. It was concluded, through data collected in the interview to the leading nurse, that despite de fact she is not familiar with the Lean concept and methodology, as process simplification and time reduction were taken into account, the new process can be considered Lean. The flow charts that reflect both the sorting structure used in the urgencies before and after were developed. Hospital culture, lack of communication before the new process implementation, and the facilities were some of the identified barriers.

Hospital culture regarding imaging and the specialties which perform it has been resistant to change. In particular, ultrasound performed at the point of care (POCUS) has taken a while to become established within hospital systems. The COVID-19 pandemic has seemingly caused acceleration of POCUS acceptance. What is driving this rapid culture change?
