Backgraund: the researchers explain the high prevalence of metabolic syndrome among patients with psoriasis by the development of systemic infl ammation, which is considered as a single pathogenetic mechanism in these pathological conditions. The response of patients with psoriasis with metabolic syndrome to treatment with narrowband therapy is currently insuffi ciently studied. Aims: the study evaluated the effi cacy of narrowband therapy (311 nm) in patients with psoriasis associated with metabolic syndrome. Materials and methods: an objective and instrumental examination was conducted in 72 patients diagnosed with psoriasis vulgaris, after which a course of 20 procedures of narrow-cavity medium-wave ultraviolet irradiation (UFO) with a wavelength of 311 nm (B) was conducted. Patients were divided into 2 groups: patients with psoriasis and patients with psoriasis combined with metabolic syndrome. To assess the eff ectiveness of treatment, the index of the area and severity of psoriasis (PASI), lipid and carbohydrate metabolism; insulin and leptin; proinfl ammatory cytokines —IL-1β, IL-8, TNF-α and anti-infl ammatory cytokine IL-10 before and after the 10th, 20th procedure of UV-B therapy and 3 and 6 months after treatment. Results: а signifi cant decrease in PASI was observed in all patients with psoriasis after 10 procedures of UV-B therapy, but in the group of patients with metabolic syndrome, the PASI index was higher. After 20 procedures, Pasi reduction by more than 50 % was observed in the group of patients with psoriasis without manifestations of metabolic syndrome (р < 0,001). Indicators of lipid and carbohydrate blood metabolism in MS patients decreased (р < 0,05), the eff ect remained up to 6 months. The level of proinfl ammatory cytokines was increased in both groups, but in the group of patients with MS these indicators were higher. In the course of therapy cytokines decreased in both groups, the eff ect was maintained up to six months. On the contrary, IL-10 was reduced in both groups, and increased after our treatment, with the prolongation of the eff ect to 6 months. Conclusions: the appointment of UV-B therapy (311 nm) in patients with psoriasis with metabolic syndrome reduces the eff ectiveness of the course in the long term, which apparently requires repeated courses of narrowband therapy after 6 months.

Obesity causes low-grade chronic infl ammation. Adiponectin is an anti-infl ammatory cytokine. Increased waist circumference in obesity infl uences pro-infl ammatory and anti-infl ammatory cytokine levels which increase the risk of metabolic complications. This study was a cross-sectional study conducted at the Nutrition and Metabolic Diseases Clinic of Children's Hospital Dr. Soetomo, Surabaya. Subjects were chosen consecutively as many as 59 obese adolescents with inclusion criteria: obesity, aged 13-16 years, and having parents / guardians who were willing to follow the research and exclusion criteria: taking drugs, such as dyslipidemia, getting hormone therapy, suff ering from secondary obesity , suff ers from endocrine abnormalities, and has an infection or infl ammatory disease. Anthropometric examination was performed on subjects, including body weight, height, and waist circumference. Body Mass Index (BMI) is measured by weight calculation (kg) divided by height squared (meters) and plotted into the CDC 2000 curve. Obesity is established if a Body Mass Index/Age (BMI/A) >P95 is obtained according to age and sex on the CDC 2000 curve. Adiponectin levels are examined using venous serum with ELISA. The relationship between waist circumference and adiponectin levels was analyzed by Pearson correlation. 59 subjects were included in this study. The mean of waist circumference and adiponectin were 100.18 ± 10.63 cm and 7,84 ± 3,81 μg / ml. Statistic test showed that there is no correlation between waist circumference and adiponectin levels in obese adolescents (p>0,05). There is no correlation between waist circumference and adiponectin levels in obese adolescents. Factors that aff ect the correlation between waist circumference and adiponectin include race, diet and physical activity

The superoxide radical (O2·̅), hydrogen peroxide (H2O2) and nitric oxide (NO) are pleiotropic infl ammatory mediators which play an important role in infl ammatory joint diseases. They are overproduced during rheumatoid arthritis and its experimental model - adjuvantinduced arthritis in rodents - and may be detected both systemically and intra-articularly. Their secretion is up-regulated by proinfl ammatory cytokines such as IFN-γ, IL-12, IL-6 and TNF-α, and they are responsible for the destruction of joint tissue. In this work, the effect of superoxide dismutase (SOD) from a thermotolerant yeast strain, Kluyveromyces marxianus, on the production of proinfl ammatory cytokines, reactive oxygen and nitrogen species was studied. Mice received three intraperitoneal injections of yeast SOD at a dose of 10 mg/ kg body weight (30,000 U/kg) on consecutive days starting on the day after arthritic induction. On days 3, 8 and 14 post induction peritoneal macrophages were isolated and both spontaneous and stimulated production of reactive oxygen and nitrogen metabolites were measured. Early in arthritic development yeast SOD treatment did not infl uence the O2·̅ production, but on day 14 both spontaneous and PMA-induced secretion were dramatically reduced. Spontaneous H2O2 release was inhibited on day 14, while PMA-stimulated production was decreased from the beginning of the arthritic development. Yeast SOD treatment effectively suppressed the spontaneous and recombinant mouse IFN-γ + LPS induced release of NO as well. Serum levels of proinfl ammatory cytokines, IL-12, IFN-γ, IL-6 and TNF-α, were also signifi cantly reduced. The obtained results show some of the mechanisms of action of SOD in reducing the severity of arthritic infl ammation. Besides direct inhibition of joint tissue destruction exogenous SOD substantially limits the existing positive feedback between secretion of reactive oxygen species and infl ammatory cytokine production.
