Abstract Case Presentation A 42-year-old man was transferred to our institution after a high-energy accident (an explosion). He presented second-degree burns on 20% of the total body surface area (TBSA), affecting the lower hemiabdomen and the lower right limb. He also presented a supraintercondylar open fracture of the right femur (Gustilo I). The burns were treated with debridement and coverage with mesh graft, while the fracture required an early transitory transarticular external fixation with delayed definitive osteosynthesis. Our patient presented good local and systemic evolution. We can offer a eleven-month follow-up. Discussion The literature supports that the combination of trauma and burn injuries is a relatively rare pattern, which may explain the lack of knowledge and studies on this subject. This double injury has demonstrated a synergistic effect on mortality. The management of soft tissues in the coexistence of an open fracture and a burn in the same limb is a challenge. The method and timing of the treatment of the fracture directly impacts the treatment of the burn (and vice versa), and most authors tend to treat the fracture first. All of the studies reviewed emphasized the importance of the multidisciplinary approach. Conclusions We have presented a complex case combining major trauma and severe burns. Although there is a lack of studies in the literature on this subject, the papers state that this is an unusual pattern with a synergistic effect on mortality. In our experience, the management of soft tissues and the multidisciplinary approach play a central role, as it is also stated in the literature. The management of these patients is still controversial, and more studies are needed.

AbstractCommunicating bronchopulmonary foregut malformations (CBPFM) are extremely rare. We present a complex case of type IB CBPFM with esophageal atresia and distal tracheoesophageal fistula (EA/TOF), duodenal atresia/annular pancreas (DA/AP), and intestinal malrotation who underwent primary repair for EA/TOF on day 3. Bilious aspirates on day 8 prompted an upper gastrointestinal (GI) contrast revealing a duodenal obstruction and communication between the right lung lower lobe and the esophagus (T8-T9 level). DA/AP and malrotation were repaired by a gastrojejunostomy and Ladd's procedure. A repeat contrast swallow identified a 2nd communication from the esophagus into the right lower lobe (T5-T6 level) raising the suspicion of a recurrent TOF. Computed tomography (CT) thorax confirmed above findings with an anomalous blood supply to right lung. An exploratory thoracotomy identified a three-lobed lung. However, the lower lobe was enlarged and connected in two separate locations to the esophagus. The child recovered after the disconnection of the esophageal connections and partial right lower lobectomy. CBPFM are extremely rare anomalies requiring a high index of suspicion, use of an upper GI contrast series, and CT scans for diagnosis. The treatment of choice is resection of the affected lung and disconnection of the esophageal communications.

Abstract Urethral obstruction is a very important problem in small ruminants` flocks, as it affects breeding rams and so invaluable genetic reserves of the population. A three-years-old breeding ram referred to the teaching hospital with the owner`s compliant of lack of urination in previous two days. The ram was the only one in the flock which it had unlimited access to fat-tailed ewes. The other signs consisted of anorexia, reluctance to walking and final lateral recumbency. Severe depression, anuria, dribbling of blood drops from the glans and firmness of the enlarged penis in palpation were detected in the clinical examination. But noticeably there was no local subcutaneous edema and even swelling outside of the penis as it could be usually observed in urolithiasis. It was noticeable that the animal had remarkable bleeding tendency. An obvious splashing sound was heard using stethoscope auscultation on the right flank along. Abdominal paracentesis demonstrated a blood-tinged peritoneal fluid. Ultrasonography showed a completely distended urinary bladder. The animal died because of severe obstructive azotemia. The distended urinary bladder was opened and large amounts of blood-tinged urine flashed out. The urethra was incised but no urinary stones were detected in the lumen. Otherwise, an intra-luminal blood clotting at full length was detected following to the proximal internal hemorrhage. Obviously, this complex case may be confused with urolithiasis and should be exactly differentiated. There is no available reported data concerning the entity in literature. This case report is discussing the matter.

Assuring healthy streams in the urban environment is a major goal for restoration scientists, urban planners, and city practitioners around the globe. In South Korea, many urban stream restoration efforts are designed to provide safe water to society and enhance ecological functions. We examined the extent to which the individual interests and different values of multiple stakeholders were considered in previous decision-making in two urban stream restoration projects. The relevant data on stream restoration were collected through the nominal group technique (NGT) and the analytic hierarchy process (AHP) for the two stream cases of a populated inland area and a coastal region in South Korea. The AHP results provide information about the comparative weights of the values of ecological restoration (priority score: 0.487), social restoration (priority score: 0.231), and landscape revitalization (priority score: 0.279) of the Ahn-Yang stream and ecological restoration (priority score: 0.527), social restoration (priority score: 0.182), and landscape revitalization (priority score: 0.290) of the Sahn-Jee stream. The stakeholders of the populated metropolitan area had a relatively high awareness of their role in environmental restoration, thus it was natural for them to place a high value on social restoration.

Abstract This is a report of a 31-year-old male refugee, who was admitted to Intensive Therapy Unit after being found in severe chest pain after escaping extreme torture from his home country. He was found to have four nails in his thorax. These were removed using a subxiphoid video-assisted thorascopic surgery (VATS) technique. This technique allowed excellent visualization of the right, left and anterior mediastinal part of the chest and therefore preventing damage or injury to surrounding structures. This was particularly useful in a complex case such as this. By avoiding an intercoastal incision and intercostal manipulation, our patient had limited pain post-procedure facilitating an earlier aggressive mobilization program with potential benefit in terms of improved lung expansion, reduction of atelectasis and lung infections. With the right training, the technical challenges of using the technique should be overcome and thus the benefits of subxiphoid VATS will be offered to a larger portion of thoracic surgical patients.

Ghana has had a long-standing problem of illegal gold mining that has led to the destruction of the environment. The government of Ghana is taking steps to not only curb illegal mining but also to restore destroyed lands that resulted from illegal mining. The government intends to spend financially in the area of ecological restoration to returned disturbed lands to their natural states possible, but the question remains whether restoring those disturbed lands will be beneficial to the country. The study was undertaken in Bekwai Municipal Area in the Ashanti region of Ghana where most locals are farmers. The research studies whether the benefits of ecological restoration outweigh the cost of ecological restoration? The research deployed a quantitative data collection. The data collected was analyzed using benefit-Cost ratio. The result shows that the benefit of ecological restoration outweighs the cost incurred as dependent on the land use as a carbon sequestration project. In conclusion, investment in ecological restoration is a step in the right direction for a country endowed with gold resources. This will spur growth and at the same time improve and protect the country’s natural resources and environment.

Variation in patterns of mating within and among plant populations can be impacted by habitat disturbance and have significant implications for offspring fitness. An understanding of this variation will inform predictions of seed genetic quality, benefiting ecological restoration through better seed-sourcing guidelines. We assessed mating system variation in six populations of tuart (Eucalyptus gomphocephala DC.), an iconic tree of significance to ecological restoration in Western Australia. A mixed mating system was observed with predominant outcrossing (tm = 0.76 ± 0.05) and low biparental inbreeding (tm–ts = 0.03 ± 0.02). We detected some evidence of increased inbreeding in a naturally fragmented population (tm–ts = 0.10 ± 0.04) and in a disturbed urban remnant (tm = 0.52 ± 0.12), including a family with complete selfing. However, most variation in outcrossing rate occurred among individuals within populations (82%), rather than among populations (2.6%) or among groups of populations defined by fragmentation or disturbance (15.4%). Genetic diversity was not consistently lower in offspring from fragmented, smaller and/or disturbed populations. These data reinforce the importance of sourcing seeds from multiple trees for ecological restoration, and emphasise that tuart’s mating system and the genetic diversity of offspring is robust to some habitat disturbance and/or fragmentation.

Complex periprosthetic cases are considered as challenges by clinicians. Clinical and radiographic parameters should be considered separately to make the right choice between an endodontically or periodontally compromised treated tooth and implant. Therefore, in order to decide whether the tooth is safe or not, data that have to be collected are specific parameters of both the patient and the clinician. In addition, the presence of periodontal, prosthetic, and orthodontic diseases requires patients to be set in multidisciplinary approach. The aim of this case report is to describe how the multidisciplinary approach could be the best way to manage difficult cases of implant-prosthetic rehabilitation. How to rehabilitate with fixed prosthesis on natural teeth and dental implants a smoker patient who presents with active periodontitis, multiple edentulous areas, dental malocclusion, and severe aesthetic problems was also described.

The case of a newborn with a post-ductal interrupted aortic arch combined with severe hypoplasia of the thoracic descending aorta and intracardiac defects is described. Extraanatomic bypass surgery was performed in the neonatal period to connect the ascending and descending parts of the aorta. Closure of a ventricular septal defect and excision of hypertrophied bands in the right ventricle with subsequent ventriculotomy were delayed beyond neonatal age.

A 50-year-old man with no medical history of note presented with new onset of confusion and dyspnoea. He tested positive for coronavirus (COVID-19), and subsequently, was admitted to the intensive care unit due to severe sepsis and acute renal failure requiring haemodialysis. Shortly afterwards, he was intubated due to haemodynamic instability. His blood culture was positive for Staphylococcus aureus bacteraemia, and echocardiogram showed evidence of vegetation in the aortic valve area. He was commenced on intravenous antibiotics for infective endocarditis (IE). Following extubation, he underwent an MRI of the spine due to increasing back pain. This was suggestive of L5–S1 discitis, likely secondary to septic emboli from IE. A few days later, he developed acute ischaemia of the left toes and extensive thrombosis of the right cubital and left iliac veins. Following a prolonged hospital admission, he was discharged home and later underwent an elective forefoot amputation from which he made a good recovery.
