Modified calcium silicate cement had previously been developed as a dental retrograde filling. It had great sealing ability, good biocompatibility, and anti-bacterial properties. However, its clinical application was limited by a long setting time and poor handling property. In previous study,1, 2 the setting property of PSC was greatly improved due the sol-gel process, without the addition of transition metal element. In this study, different ratio of starting material of PSC was prepared. The composition of each group was also altered with the ratio of the starting materials according to the result of XRD pattern. There is no significant difference in pH variation profile between each group. Even though low C 3 S / C 4 AF ratio (DX631 and DX541 groups) possessed high initial strength at first 24 hours after hydration, there was no increase in strength ever since according to the result of microhardness. However, the groups with high C 3 S / C 4 AF ratio (DX811 and DX721 groups) possessed relatively low initial strength at 24 hours after hydration, but the strength increased rapidly and continuously at least for the next six days of hydration. Moreover, the peak intensity of hydration product (Portlandite) on XRD pattern in DX811 group was much higher than other three groups, and this was also in agreement with the result of microhardness. DX811 group is determined to be the optimal one for the further development in the application as root-end filling materials.

The retrograde filling is a critical step to a successful outcome of the endodontic surgery. Despite the progress in the technology of novel root-end filling materials, zinc oxide-eugenol cement superEBA is still preserving its clinical value on long-term basis. The study aimed to reconsider the tissue response to the initial irritating effect of this material. Silicon tubes filled with superEBA were subcutaneously implanted for 120 days in white Wistar rats which were afterwards sacrificed. The connective tissue surrounding the superEBA implants revealed fibroblast proliferation and a definite reparatory process without inflammatory reaction. A non-specific tissue healing in progress around the implants, without calcifications, necrosis, and apoptosis was also described after 4 months. SuperEBA proved on animal model that its cytotoxicity is reducing gradually in time until no adverse reaction is observed. The reduced content in eugenol compared to other surgical zinc oxide cements and the benefic effect of o-ethoxybenzoic acid are the support to reconsider SuperEBA as a biocompatible retrograde filling material.

In this study, we evaluated the invasion of bacteria into the dentinal tubules of retrofilled extracted human teeth, and the influence of different fluorescently labeled retrograde filling materials on the bacterial invasion and viability, by means of confocal laser scanning microscopy (CLSM). The root apices of extracted teeth were cut, prepared, and filled retrogradely using either intermediate restorative material (IRM), mineral trioxide aggregate (MTA), or Biodentine. The roots were filled with Enterococcus faecalis bacteria from their coronal part for 21 days. Then, 3-mm-long apical segments were cut to get root axial slices, and the bacteria were fluorescently stained and evaluated by CLSM. Bacterial penetration into the dentinal tubules favored the bucco-lingual directions. The filling materials penetrated up to 957 µm into the tubuli, and the bacteria, up to 1480 µm (means: 130 and 167 μm, respectively). Biodentine fillings penetrated less and the associated bacteria penetrated deeper into the tubuli compared to MTA or IRM (p = 0.004). Deeper filling penetration was associated with shallower penetration of both dead and live, or live alone, bacteria (p = 0.015). In conclusion, the current study enables better understanding of the microbiological–pathological course after endodontic surgical procedures. It was found that even with retrograde fillings, bacteria invade deep into the dental tubules, where deeper filling penetration prevents deeper penetration of the bacteria and adversely affects the viability of the bacteria.

Abstract Background The present study compares the apical microleakage of three different root-end filling materials in which the retrograde cavity is prepared by two different burs. Methods Eighty extracted single rooted maxillary and mandibular premolars were taken. Root canal treatment was completed. Apical 3 mm of all the teeth were resected with diamond disk. The tooth were divided into four groups with two subgroups for each group containing 10 tooth (N= 10) as: Group IA (Negative Control and IB (Positive Control); Group IIA and IIB: Prepared with round carbide bur and round diamond bur respectively, filled with GIC; Group IIIA and IIIB: Prepared with round carbide bur and round diamond bur respectively, filled with MTA; Group IVA and IVB: Prepared with round carbide bur and round diamond bur, filled with Biodentine. After applying two coats of nail varnish leaving apical 3 mm (except for negative control group) all teeth were immersed in 2% methylene blue for 3 days and again in 65% nitric acid for next 3 days for extraction of dye. The obtained solution was then transferred to eppendorf tube and centrifuged in microcentrifuges at 14,000 revolution per minutes (RPM) for 5 minutes. Optical density or absorbance of the supernatant solution was measured with UV spectrophotometer at 550 nm. Results The absorbance of the supernatant solution after dye extraction is decreasing in the order of positive control> GIC> MTA> Biodentine> negative control group. The significant difference was observed between GIC and MTA (p=0.0001) and GIC and Biodentine (p=0.0001) with two different burs but statistically non-significant difference was observed between MTA and Biodentine with Carbide bur (p=0.127) and Diamond bur (p=0.496) respectively. Conclusions Within the limitations of the present study, it can be concluded that Biodentine and MTA showed less microleakage as compared to GIC. There is no significant difference between mean microleakage of MTA and Biodentine. However, the mean OD of the Biodentine was least of all evaluated materials. Preparation of the root-end using round carbide bur as well as round diamond burs showed comparable microleakage for all three filling materials.

Aim: Although there are many materials that can be used for retrograde filling in surgical endodontics, none of them can be regarded as an ideal material yet. The purpose of this study was to compare the surface topography of three different root-end filling materials.Methods: 36 extracted single rooted human incisor teeth were cleaned and decoronated to standardized 10 mm root lengths. The root segments were prepared and 2 mm apical resection were performed. The samples were randomly separeted to three groups (Group A: Ca(OH)2, Group B: MTA Angelus, Group C: ProRoot MTA), each comprised of 12 roots. Materials were placed as 2 mm apical barriers and obturated with guttapercha and AH-Plus sealer. Each group dimidiated two subgroups (A1,A2,B1,B2,C1,C2). Groups A1,B1,C1 were stored in normal saline (NS), groups A2,B2,C2 were stored in neutral phosphate buffer saline (NPBS) solution and samples were incubated at 370C for 2 weeks. Stereomicroscope (32X) was used to photograph the root-end filling.Results: All specimens demonstrated white crystals formation and sediment over the root-end filling materials and on the superficial border of the root-end cavities’ wall as a white plague. A2,B2,C2 samples have more crystal sediment on root-end fillings than samples A1,B1,C1. Dissolution and corrosion were observed in groups A1, A2.Conclusions: The results of this study revealed that calcium hydroxide is more resorbable than MTA Angelus and ProRoot MTA. The crystals formation and precipitation were observed in neutral phosphate buffer saline solution was more than normal saline solution for all groups as a hydroxiapatite crystals.
