Screening and adjuvant postoperative therapies have increased survival among women with breast cancer. These tools are seldom applied in elderly patients, although the usually reported incidence of breast cancer is close to 50% in women 65 years or older, reaching 47% after 70 years in the updated Surveillance, Epidemiology, and End Results (SEER) database. Elderly breast cancer patients, even if in good medical health, were frequently excluded from adjuvant clinical trials. Women age 70 years who are fit actually have a median life expectancy of 15.5 years, ie, half of them will live much longer and will remain exposed for enough time to the potentially preventable risks of a relapse and specific death. In the last few years, a new concern about this issue has developed. Treatment now faces two major end points, as in younger women: to improve disease-free survival in the early stages, and to palliate symptoms in advanced disease. However, in both settings, the absolute benefit of treatment is critical because protecting quality of life and all its related aspects (especially functional status and independence), is crucial in older persons who have more limited life expectancy. Furthermore, the new hormonal compounds (aromatase inhibitors) and chemotherapeutic drugs (capecitabine, liposomal doxorubicin), are potentially less toxic than and equally as effective as older more established therapies. These new treatments bring new challenges including higher cost, and defining their benefit in elderly breast cancer must include an analysis of the cost/benefit ratio. These issues emphasize the urgent need to develop and support clinical trials for this older population of breast cancer patients both in the adjuvant and metastatic settings, a move that will take us from a prejudiced, age-based medicine to an evidence-based medicine.

Increased survival by blacks and Hispanics is causing a widening of the sex imbalance of the elderly population much like we have observed in the general population. These demographic trends point toward greater widowhood among minority women and continuing high rates of poverty. In addition, we can expect increased rates of disability in minority elderly women, increased dependency, worsening intergenerational relationships, and higher rates of institutionalization.

This paper investigated depression-free life expectancy in the elderly population of Ceará, the Northeast and Brazil. We used data from the 2019 National Health Survey (PNS) and the projections of the Mortality Tables provided by the Brazilian Institute of Geography and Statistics (IBGE) in 2018. Sullivan's method was used to estimate depression-free life expectancy at age 60 years. The results show that, among the elderly in the analyzed regions, women are at a disadvantage in the number of years they will live with depression compared to men. Which suggests that women live longer with depression compared to men. Among the populations analyzed, it was also found that the one in the Northeast showed the smallest difference in the prevalence of depression in the elderly between the sexes, with 11.6% in women and 3.7% in men. In general, the Northeast has the lowest prevalence of depression among the elderly, when compared to the other two regions.

Abstract Background: Prolonged healthy life expectancy, which is duration without the requirement of any kind of help for activities of daily living (ADL), is essential to ensure a long life with a good quality of living in the community. Further, local residents should understand their health conditions and live consciously to prolong healthy life expectancy. The development of a simple general health indicator is necessary. Both occlusal force and flow-medicated dilation (FMD) which reflects endothelial function are useful tools for understanding the general condition of the elderly. However, few studies have investigated the relationship between occlusal force and endothelial function. In the present study we examined this relationship, occlusal force measurement can be a good indicator of the general condition of the elderly. Methods: In 38 community-dwelling women(aged 76.7 ±5.7 years), we measured occlusal force,　grip strength, endothelial function evaluated by FMD, advanced glycation end products (AGEs). In this study we investigate we investigated the relationship between occlusal force, measurement items, and factors independently related to endothelial dysfunction (FMD<7％). Results: There were significantly correlation between occlusal force and grip strength (r=0.54, p<0.01). Degree of FMD significantly associated with occlusal force (r=0.60, p<0.01) and grip strength (r=0.35, p<0.05) or amount of increased AGEs (r=-0.37, p<0.05). Occlusal force was independently associated with degree of FMD after adjusting for age, AGEs, and grip strength (p < 0.05).Conclusion: There was a significant relationship between occlusal force and FMD. Occlusal force can be an important indicator of endothelial function in community-dwelling elderly. This study may help understanding general health of elderly in community.
