Despite the rapid technological advance, the expansion of prenatal ultrasound diagnosis, as well as the accumulation of experience by both domestic and foreign experts, prenatal recognition of true knots of the umbilical cord causes significant difficulties. Three cases of successful ultrasound diagnosis of true knots of the umbilical cord and brief review of the literature are presented.

The complex method of antenatal diagnostics of the cord entanglement round the body of the fetus is elaborated. It includes echography, color Doppler mapping, dopplerometry and actocardiography. On the basis of the data received the system of prognosing and estimating the severity of feta l hypoxia in the end of the third trimester pregnancy is suggested, that allows to choose the correct tactics of pregnancy and delivery management in different rates ofcord entanglement. The use o f this method made it possible to rise the effectiveness of antenatal diagnostics of this gestational complication from 23,5% to 79,4%, to decrease the frequency of postnatal asphyxia to 46,1% and, therefore, to avoid intra- and postnatal loss.

Abstract. CRH-binding protein was present in the amniotic fluid and in the umbilical cord plasma after 15 weeks and 24 weeks of pregnancy, respectively. The size of the CRH-binding protein was similar to that in the peripheral blood from normal subjects. The level of the binding of CRH-binding protein in the umbilical cord plasma during the third trimester of pregnancy was also similar to that in the peripheral blood of neonates and normal adult subjects. The binding of CRH-binding protein was temporarily decreased at 40 weeks of pregnancy. These results indicate that fetal CRH-binding protein seems to be produced at least in the second trimester of pregnancy.

The aim of the work is to study the activity of lipoperoxidation processes and antioxidant protection system in the umbilical cord blood of newborns from CMV-seropositive women with exacerbation of infection in the third trimester of pregnancy. Lipid peroxidation activity and antioxidant status in 30 newborns from CMV-seropositive women with exacerbation of infection in the third trimester of pregnancy and in 25 from CMV-seronegative women were studied. Erythrocytes were used to determine the products of oxidative modification of lipids; antioxidant status was evaluated in the blood plasma of the umbilical vein. The content of TBK-active products was determined by fluorometric method, lysophosphatidylcholine by thin-layer chromatography, arachidonic acid by gas-liquid chromatography. Antioxidant status was assessed by α-tocopherol content and superoxide dismutase activity. Registration of optical densities and fluorescence was carried out using a spectrofluorometer. The study found an increase in the content of red blood cells of the umbilical cord of newborns from CMV-seropositive women with an exacerbation of infection in the third trimester of pregnancy of TAC-active products in 2.1 times (p˂0.001), of lysophosphatidylcholine in 1.9 times (p˂0.001) and of arachidonic acid in 1.5 times (p˂0.001). The antioxidant status of umbilical cord blood was characterized by 1.5 times decrease in α-tocopherol (p<0.01) and 1.3 times decrease in superoxide dismutase activity (p˂0.001) compared with similar indices in the group of newborns from CMV-seronegative women. The results of the study show that the exacerbation of cytomegalovirus infection in the third trimester of pregnancy is accompanied by the development of oxidative stress and tension in the system of antioxidant protection in the blood of the umbilical cord of newborns due to increased consumption of α-tocopherol and suppression of superoxide dismutase activity.

Background. Endothelial cells are the site of productive replication, hematogenous spread and persistence for a variety of viruses, including cytomegalovirus, which play a critical role in the development of vascular complications associated with cytomegalovirus infection due to developing endothelial dysfunction.Aim: to reveal the role of inflammatory mediators (tumor necrosis factor alpha, interleukin-1β, interleukin-8) in the formation of umbilical cord vascular endothelial dysfunction in reactivation of latent cytomegalovirus infection in the third trimester of pregnancy.Material and methods. The standard method of solid-phase (“sandwich” variant) enzyme immunoassay was carried out to study pro-inflammatory cytokines (tumor necrosis factor alpha, interleukin-1β, -8), endothelin-1, nitrite anion in the blood of the umbilical cord of newborns from mothers who come through reactivation of latent cytomegalovirus infection in the third trimester of pregnancy. The work includes examination data of 78 newborns born at 38–40 weeks of gestation. Among them: 45 newborns were born by CMV-seropositive women with reactivation of latent cytomegalovirus infection in the third trimester of pregnancy (main group) and 33 – by CMV-seronegative women (control group). Umbilical vein blood serum was chosen as the material for the study.Results. In the blood of the umbilical vein of newborns from mothers with reactivation of latent cytomegalovirus infection in the third trimester of pregnancy, a high level of pro-inflammatory cytokines was detected: tumor necrosis factor alpha, interleukin-1β, interleukin-8 (p < 0.001) with a simultaneous increase in the content of endothelin-1 and nitrite anion (p < 0.001), compared with similar indicators for healthy newborns.Conclusion. Reactivation of latent cytomegalovirus infection in the third trimester of pregnancy is associated with the formation of a systemic fetal inflammatory response determined by a high concentration of inflammatory mediators (tumor necrosis factor alpha, interleukin-1β, interleukin-8) and an increase in vasoactive compounds (endothelin-1 and nitrite-anion) leading to the formation of dysfunction of the vascular endothelium of the umbilical cord.

In order to identify the molecular effects of erythropoietin on the lipid composition and properties of the membrane of red blood cells of the umbilical cord, 35 newborn children born from cytomegalovirus seropositive women with exacerbation of infection in the third trimester of pregnancy (main group) were examined. The control group consisted of 30 newborns born from cytomegalovirus seronegative women. Plasma and red blood cells of the umbilical cord became the material for the study. In the membrane of red blood cells of the umbilical cord by spectrophotometry there were studied the products that react with thiobarbituric acid; membrane microviscosity was studied by lateral diffusion of hydrophobic fluorescent probe of pyrenes; lysophosphatidylcholine content by a thin-layer chromatography; the content of arachidonic, oleic and α-linolenic fatty acids by gas chromatography; ionized calcium content by analyzer of blood gases and electrolytes; erythropoietin in umbilical cord blood plasma by enzyme immunoassay. The number of erythrocytes was determined by an automatic hematological analyzer. The assessment of erythrocytes morphology and echinocytes counting was performed by light microscopy with further cytophotometrical data processing. In the umbilical cord blood of newborns born from cytomegalovirus seropositive women with exacerbation of infection in the third trimester of pregnancy, there was found out an increase in the level of erythropoietin by 20% (p<0.05), the number of erythrocytes by 21% (p<0.01), of products reacting with thiobarbituric acid by 60% (p<0.001), of lysophosphatidylcholine by 42% (p<0.001), of arachidonic acid by 31% (p<0.001)<0.001), of ionized calcium by 14% (p<0.05), of the amount of echinocytes by 91% (p<0.001); with a decrease in the content of oleic and α-linolenic fatty acids by 40% (p<0.001), there was an increase in the ratio of fluorescence of excimers and monomers (Fe/Fm) by 30% (p<0.001), the number of red blood cells by 21% (p<0.01). The results obtained suggested in the newborns from seropositive women to cytomegalovirus with exacerbation of the infection in the third trimester of pregnancy the development of ineffective erythropoiesis, enhancing oxidative damage of red blood cells, leading to echinocytosis and eryptosis

The purpose of the work is to study the processes of peroxidation of lipids and their influence on the structure of the endothelium of the umbilical artery of newborns from mothers who had experienced exacerbation of cytomegalovirus (CMV) infection in the third trimester of pregnancy. For the object of the study there were taken 45 umbilical cord samples during childbirth from women who had acute CMV infection in the third trimester of pregnancy (main group), and 25 umbilical cord samples from healthy women (control group). Clinical diagnosis of primary CMV infection was established by the presence of IgM antibodies to CMV and low-avid IgG in the peripheral blood (avidity index <50%), as well as CMV DNA detected by polymerase chain reaction in blood or urine; exacerbation of chronic CMV infection by the presence of IgM to CMV, high-avid IgG (avidity index >65%), and CMV DNA in scrapes taken from buccal epithelium. Lipid peroxides in the endothelium of the umbilical artery were detected by the Winkler-Schulze method. Morphological examination of the umbilical cord was performed by histochemical methods. The results of histochemical studies indicate that the exacerbation of CMV infection in the third trimester of pregnancy is associated with oxidative stress that causes the depletion of the protective systems and oxidant potential, which leads to the development of significant oxidative destruction of the endothelium of the umbilical cord artery of newborns. The revealed morphohistochemical changes in umbilical cord tissues in CMV infection can affect the functional state of this organ and be a morphological substrate for the appearance of its dysfunction, mainly the basis for the violation of fetoplacental blood circulation causing the development of hypoxia.
