The plasma membrane (Ca2++Mg2+)ATPase hydrolyzes pseudo-substrates such as p-nitrophenylphosphate. Except when calmodulin is present, Ca2+ ions inhibit the p-nitrophenylphosphatase activity. In this report it is shown that, in the presence of glycerol, Ca2+ strongly stimulates phosphatase activity in a dose-dependent manner. The glycerol- and Ca2+-induced increase in activity is correlated with modifications in the spectral center of mass (average emission wavenumber) of the intrinsic fluorescence of the enzyme. It is concluded that the synergistic effect of glycerol and Ca2+ is related to opposite long-term hydration effects on the substrate binding domain and the Ca2+ binding domain.

AbstractAdenomyosis is a common gynaecological disorder that is associated with infertility and miscarriage. We report a case of adenomyosis presented with urinary retention and infertility. The patient successfully conceived spontaneously following GnRHa followed by progestogen therapy. This treatment option might provide a synergistic effect in improving fertility and pregnancy outcome for women with adenomyosis. This modality has no potential surgical risk; it minimises the long-term side effects of GnRHa and also provides luteal support.

The most frequently used medications for chronic pain are local anesthetics (LA) and glucocorticoids. Common adverse events from LA, such as seizures and cardiotoxicity, are well known. A lesser known side effect is local tissue reaction to the LA concentration. Myotoxicity is one of the common denominators of direct tissue reaction to LA; it results from the disruption of the mitochondria in the muscle cells. All LA produce some degree of myotoxicity; bupivacaine has the greatest effect and procaine the least. If LA is combined with glucocorticoid, muscle breakdown is even more extensive showing a synergistic effect. Myotoxicity depends primarily on LA concentration, is time dependent, and is enhanced by preexisting altered metabolism. It affects mostly young patients. Potential effects of long-term or repeated administration of the combination LA/glucocorticoids medications should always be considered and discussed with patients.
