We compared the incidence of significant Dupuytren’s disease in men across occupational social classes in England and Wales, using data from the National Morbidity Survey. We found that manual occupational social class was not associated with an increased incidence of Dupuytren’s disease. In fact, the incidence rates of Dupuytren’s disease in the elderly were higher in non-manual than in manual social classes.

An immunohistochemical study was performed on nodules excised from the palmar fascia of patients with Dupuytren’s contracture. In cellular nodules, antibodies to actin (used as a marker for myofibroblasts), desmin, vimentin, Mac 387 (a macrophage marker) and leucocyte common antigen were used. A correlation was demonstrated between the numbers of macrophages and the presence of myofibroblasts. The presence of myofibroblasts is generally considered to indicate the active stage of the disease. Inflammatory cells other than macrophages were largely absent from the nodules, although lymphocytes were frequent in the tissue around the nodules. Microvascular changes were prominent in the nodules and pericyte proliferation was observed around occluded capillaries. Release of growth factors from macrophages may be important in Dupuytren’s contracture, as is the case in other fibrotic diseases. The possible role of macrophages in the aetiology of Dupuytren’s disease is discussed.

Using data from a national survey of South Koreans, this study explores the role of science communication in enhancing three different forms of scientific knowledge ( factual, procedural, and subjective). We first assess learning effects, looking at the extent to which citizens learn science from different channels of communication (interpersonal discussions, traditional newspapers, television, online newspapers, and social media). We then look into the knowledge gap hypothesis, investigating how different communication channels can either widen or narrow the gap in knowledge between social classes. Television was found to function as a “knowledge leveler,” narrowing the gap between highly and less educated South Koreans. The role of online newspapers in science learning is pronounced in our research. Reading newspapers online indicated a positive relationship to all three measures of knowledge. Contrary to the knowledge-leveling effect of television viewing, reading online newspapers was found to increase, rather than decrease, the gap in knowledge. Implications of our findings are discussed in detail.

Abstract Using data from 3 private healthcare claims repositories, we evaluated the incidence of pneumococcal disease among adults with US Advisory Committee on Immunization Practices (ACIP) defined at-risk conditions or rheumatoid arthritis, lupus, Crohn's disease, and neuromuscular disorder/seizures and those with traditional high-risk conditions. We observed that adults with ≥2 concurrent comorbid conditions had pneumococcal disease incidence rates that were as high as or higher than rates observed in those with traditional high-risk conditions.

AbstractBackgroundWhile RSV circulation results in high burden of hospitalization, particularly among infants, young children and the elderly, little is known about the role of different age groups in propagating annual RSV epidemics in the community.MethodsDuring a communicable disease outbreak, some subpopulations may play a disproportionate role during the outbreak's ascent due to increased susceptibility and/or contact rates. Such subpopulations can be identified by considering the proportion that cases in a subpopulation represent among all cases in the population occurring before (Bp) and after the epidemic peak (Ap) to calculate the subpopulation's relative risk, RR=Bp/Ap. We estimated RR for several age groups using data on RSV hospitalizations in the US between 2001-2012 from the Healthcare Cost and Utilization Project (HCUP).ResultsChildren aged 3-4y and 5-6y each had the highest RR estimate for 5/11 seasons in the data, with RSV hospitalization rates in infants being generally higher during seasons when children aged 5-6y had the highest RR estimates. Children aged 2y had the highest RR estimate during one season. RR estimates in infants and individuals aged 11y and older were mostly lower than in children aged 1-10y.ConclusionsThe RR estimates suggest that preschool and young school-age children have the leading relative roles during RSV epidemics. We hope that those results will aid in the design of RSV vaccination policies.

The COVID-19 pandemic is profoundly affecting the minds and behaviors of people worldwide. This study investigated the differences in the need for structure among people from different social classes and the psychological mechanisms underlying this need, as well as the moderating effect of the threat posed by the pandemic. Using data collected from non-student adults in China, we found that the lower an individual’s social class, the lower their need for structure, and this effect was based on the mediating role of perceived control. However, the mediating effect was moderated by pandemic threat, and the above relationship existed only when this threat was low. When the level of pandemic threat was higher, neither the effect of social class nor of perceived control on the need for structure were significant. Specifically, in higher-threat situations, the need for structure among individuals from higher social classes and who had a higher sense of control increased significantly, meaning the mediating effect was no longer significant. This finding showed that under the threat of a pandemic, individuals who have a lower need for structure will still pursue and prefer structure and order. The theoretical and practical implications of the research are also discussed.

Relevance. Dupuytren’s disease (palmar fascial fibromatosis) affects primarily palmar and digital fascia and results in progressive wrist deformity in many patients and often with bilateral involvement. Absence of corresponding data on the patients with severe wrist deformities along with their treatment issues is the ground for targeted research of pathomorphology of advanced disease stages.Purpose of the study — to identify features of ultrastructure of fibromatosis nodules and bands in palmar aponeurosis of patients with Dupuytren’s disease of grade III-IV.Materials and Methods. The authors analyzed medical histories and surgical material of 20 patients aging 42–77 years. Segments from medial portion of pretendinous cord of IV digit were cut for examination under scanning electron microscope (JSM-840, Jeol, Japan).Results. Irrespective of disease history (from 1 to 20 years) fibrbous-fibrillar network and fine cylindrical collagen fibers prevailed in nodules of pretendinous cord which formed semicircular and circular end coils. Empty lacunae, functionally active fibroblasts and close cellular pairs were observed in nodules. Bands differed from nodules by lesser cellularity and less content of fine fibers, orientation of thick fibers mainly along one axis, straightening segments of undulated twisting and separate twisted and tightly interwoven fiber fragments.Conclusion. In Dupuytren’s disease of grade III-IV nodules maintain the role of active contractile centers. Despite relatively small cellularity of pathologically altered tissues there is a potential for progressing, propagation and recurrence of fibromatosis.

Changes in population structure leading to the aging of the population is a challenge in development. In Indonesia, the majority of elderly people are still working and most of them are poor. There are limitations as well as the role of government in protecting the lives of the elderly population to encourage a greater role of the family. Intergenerational Transfer is a solution that can be used to maintain the life of elderly people remain prosperous. Using data from Indonesia Family Life Survey (IFLS) in 2007 and the probit analysis technique, this study provides important results to confirm the position of intergenerational transfers in reducing the level of participation of elderly people in the labor market.
