Primary bone sarcomas in patients over 40 years of age: A retrospective study using data from the Bone Tumor Registry of Japan

Objectives: Bone tumors are uncommon. Nevertheless, bone is a common site for metastasis. Due to limited data regarding bone tumors in Jordan, this study aims to understand better bone tumor epidemiology and distribution in the Jordanian population. Methods: A part of a retrospective, single-center study, all biopsy reports confirming the diagnosis of a bone tumor between January 2017 and December 2019 were abstracted from Princess Iman Research Center records. The patients’ age, gender, anatomical location, and histopathological type of the tumors were obtained and analyzed. Results: Two hundred eighty-four diagnostic and excisional biopsies were reviewed. Most of the patients (60.2%) were males. The mean age for the patients was 26.74(±16.29) years. Malignant bone tumors were diagnosed in 15.5%. The femur was the most common site of biopsy diagnosed tumor followed by the tibia. Osteochondroma was the most commonly diagnosed benign tumor and accounted for 39.4%, followed by aneurysmal bone cysts (14.8%), and enchondroma (10.2%). The metastatic bone tumors were the most frequent malignant bone pathology (6.7%), while osteosarcoma was the most common primary sarcoma, diagnosed in 5.6% of the sample. Increasing age, pelvic, and spine location of bone tumors were found to be significantly associated with malignancy. Conclusion: In our 284 samples, osteochondromas were the most commonly diagnosed tumors (39.4%), followed by aneurysmal bone cysts (14.8%), and enchondromas (10.2%). Metastatic tumors were the most common malignant bone pathologies, while osteosarcomas were the most common primary bone sarcomas. Femur and tibia were the most common tumor-located sites. Breast and lung deposition were the most common metastasis primaries. Additionally, increasing age, pelvic, and spine-located bone tumors were significantly associated with malignancy.

Objectives: As diagnosis and treatment guidelines for bone sarcomas continue updating, it is important to examine whether, when, and which kinds of patients have had a survival improvement over the last four decades. Methods: This cohort study included 9178 patients with primary bone and joint sarcomas from 1 January 1980 to 31 December 2018 using data from Surveillance, Epidemiology and End Results (SEER)-9 Registries. The follow-up period was extended to November 2020. Patients were divided by decade into four time periods: 1980–1989, 1990–1999, 2000–2009, and 2010–2018. The primary endpoint was bone sarcomas-specific mortality (CSM). The 5-year bone sarcomas-specific survival (CSS) rate was determined stratified by demographic, neoplastic, temporal, economic, and geographic categories. The associations between time periods and CSM were examined using a multivariable Cox regression model, with reported hazard ratio (HR) and 95% confidence interval (CI). Results: The 5-year CSS rate for bone sarcomas was 58.7%, 69.9%, 71.0%, and 69.2%, in the 1980s, 1990s, 2000s, and 2010s, respectively. Older age, male gender, tumor sites at pelvic bones, sacrum, coccyx and associated joints, as well as vertebral column, osteosarcoma and Ewing tumor, and residence in non-metropolitan areas were independently associated with higher CSM risk. After adjusting for the covariates above, patients in the 1990s (HR = 0.74, 95% CI = 0.68–0.82), 2000s (HR = 0.71, 95% CI = 0.65–0.78), and 2010s (HR = 0.68, 95% CI = 0.62–0.76) had significantly lower CSM risks than patients in the 1980s. However, patients in the 2000s and 2010s did not have lower CSM risks than those in the 1990s (both p > 0.05). Conclusions: Although bone sarcomas survival has significantly improved since 1990, it almost halted over the next three decades. Bone sarcomas survival should improve over time, similar to common cancers. New diagnostic and therapeutic strategies such as emerging immune and targeted agents are warranted to overcome this survival stalemate.

Pelvic osteosarcoma has a poor prognosis compared to osteosarcomas in other locations, and the reasons for this remain unknown. Surgical resection of pelvic osteosarcoma is technically demanding and often results in dysfunction and complications. In this study, we investigated the reasons underlying the poor prognosis of pelvic osteosarcoma by comparing it to femoral osteosarcoma using data from the Bone Tumor Registry in Japan. We used propensity score analysis to determine whether surgical resection of pelvic osteosarcoma improved its prognosis. We demonstrated that pelvic osteosarcoma had a poor prognosis because it occurred more often in the elderly, often had larger tumor size, and had metastasis at presentation more often in comparison to femoral osteosarcoma. These three factors were also associated with the non-surgical treatment of pelvic osteosarcoma, which also led to a poor outcome. The overall survival rate was only comparable in pelvic osteosarcoma and femoral osteosarcoma in cases treated with surgical resection. Propensity score analysis revealed that surgical treatment improved the prognosis of pelvic osteosarcoma. As such, we propose that surgical resection should be considered based on tumor stage and patient age in order to improve the prognosis of pelvic osteosarcoma.

Objectives. To describe trends and category of disabilities caused by Buruli ulcer disease. Design. This retrospective study was set up to quantify information on the disability trends caused by Buruli ulcer (BU) using data on patients attending BU and chronic ulcer clinics from 2004 to 2009, at Global Evangelical Mission Hospital, Apromase. Methods. Data was retrieved from the WHO BU1 form, case registry book, surgical theatre register, and BU patients' records book of the hospital. Disability was measured as the incapability of patients to perform one or more daily activities due to his/her state of BU disease before treatment. Results. A total of 336 positive BU cases comprising 181 males (53.9%) were recorded of which 113 (33.6%) cases of disabilities were identified. A mean age of 52.5 (±1.32) years was recorded. For the trend of disabilities, the year 2009 recorded the highest (N = 34, 31.0%). The lesions were mostly located at the lower limbs (N = 65, 57.5%) region of the patients. Lesions with diameter >15 cm were the major (59.3%) category of lesions. Conclusion. Trend of disability reveals proportional increase over the years from 2004 to 2009. Contracture at the knee and ankle joints was the commonest disability recorded.

Abstract Background Karyopherin α 2 (KPNA2), a member of the Karyopherin α family, has been observed in several cancers but lack substantial investigation in malignant bone tumors. The purpose of the current study was to evaluate KPNA2 expression level and its utility as a novel diagnostic biomarker in osteosarcomas and their malignant bone tumor mimickers, such as chondrosarcomas and Ewing sarcomas.Method We investigated the expression of KPNA2 protein by immunohistochemistry on paraffin embedded surgical specimens from 217 patients with malignant and benign tumors of bone, including 81 osteosarcomas, 42 chondrosarcomas, 9 Ewing sarcomas, 28 osteoid osteoma, 20 osteochondroma and 37 Chondroblastoma. Immunoreactivity was scored semi quantitatively based on stain extent and intensity.Results Seventy one of 81 (87.7%) osteosarcomas, zero of 42 (0%) chondrosarcomas and one of 9 (11.1%) Ewing sarcomas showed immunoreactivity for KPNA2. Negative KPNA2 expression was observed in all of benign bone tumors. Much more positive expression of KPNA2 was found in osteosarcomas as compared with chondrosarcomas and Ewing sarcomas. The sensitivity and specificity of KPNA2 immuno-expression for osteosarcoma was 87.7% and 100%, respectively. In several subtypes of osteosarcomas, immunohistochemical expression of KNA2 was more frequent in osteoblastic (94.5%), with 39 (70.9%) showing strong-intensity staining. KPNA2 positivity was observed in eleven of 13 (84.6%) chondroblastic, three of 6 (50%) fibroblastic, three of 4 (75%) telangiectatic and two of 3 (66.7%) giant cell-rich osteosarcoma. Stronger-intensity staining was observed in osteoblastic osteosarcoma.Conclusion KPNA2 is most frequently expressed in osteosarcomas, particularly in osteoblastic and chondroblastic tumors, but is rarely positive in chondrosarcomas and Ewing sarcomas. This feature may have diagnostic value since it is very useful for distinguishing between osteosarcomas and other bone sarcomas mimickers. This report supports KPNA2 as a novel marker for the diagnosis of osteosarcoma.

Abstract: Alcohol is an addictive substance that could cause adverse effects to human body. Moreover, it is also related to a variety of criminal acts. North Celebes is one of the provinces with the highest alcohol consumption in Indonesia. This study was aimed to describe the death cases exposed to alcohol that were autopsied at Prof. Dr. R. D Kandou Hospital Manado during 2014-2017. This was a descriptive retrospective study using data of visum et repertum. The results showed that there were 17 death cases exposed to alcohol. The majority were in 2016 as many as 6 cases (35.3%). The most common age group was 26-35 years as many as 7 cases (41.2%). All cases were males (100%). The most cause of death in these cases were due to violence using sharp objects as many as 15 cases (88.2%). Conclusion: Most death cases exposed to alcohol were males aged 26-35 years, and the cause of death was violence using sharp object.Keywords: alcohol, death casesAbstrak: Alkohol adalah zat adiktif yang dapat berdampak buruk bagi tubuh manusia serta terkait juga dengan berbagai tindakan kriminalitas. Sulawesi Utara merupakan salah satu provinsi dengan konsumen alkohol terbanyak di Indonesia. Penelitian ini bertujuan untuk mengetahui gambaran kasus kematian pada korban terpapar alkohol yang diautopsi di RSUP Prof. Dr. R. D Kandou periode 2014-2017. Jenis penelitian ialah deskriptif retrospektif menggunakan data hasil visum et repertum. Jenis penelitian ialah deskriptif retrospektif. Hasil penelitian mendapatkan 17 kasus kematian korban terpapar alkohol. Kasus terbanyak pada tahun 2016 yaitu 6 kasus (35,3%). Kelompok usia terbanyak 26-35 tahun yaitu 7 kasus (41,2%) dan jenis kelamin laki-laki (100%). Penyebab kematian terpapar alkohol terbanyak karena kekerasan benda tajam yaitu 15 kasus (88,2%). Simpulan: Sebagian besar kasus kematian akibat terpapar alkohol berada dalam kelompok usia 26-35 tahun, semua berjenis kelamin laki-laki, dengan jenis penyebab kematian akibat kekerasan benda tajam.Kata kunci: alkohol, kasus kematian
