Diabetes mellitus and risk of pancreatic cancer in China: A meta-analysis based on 26 case-control studies

Abstract Background Results of previous studies were showed that the association between H. pylori infection and the risk of diabetes is still controversies. Therefore, this systematic review and meta-analysis study was designed and implemented aimed to determine the association between H. pylori infection and the risk of diabetes.Methods All case control articles were searched in international databases, including Medline (PubMed), Web of sciences, Scopus, EMBASE, and CINHAL. Search was done from January 1990 to March 2019 without language limitations. Also, logarithm and standard error logarithm odds ratio (OR) were used for meta-analysis.Results A total of 41 studies were included in this meta-analysis. The range of association with odds ratio in case control studies which published between 1990 to 2019 was 0.21 to 6.08. The pooled estimate of the association between H. pylori infection with diabetes was 1.27 (95% CI 1.11 to 1.45, P = 0.0001, I 2 = 86.6%). The effect of H. pylori infection on diabetes mellitus, type 1 and type 2 diabetes was 1.17 (95% CI 0.94 to 1.45), 1.19 (95% CI 0.98 to 1.45), and 1.43 (95% CI 1.11 to 1.85) respectively. Subgroup analysis by the geographical regions showed in Asian population risk of the effect of H. pylori infection on diabetes was higher than other population, but in the American, this was a protective relationship.Conclusion In conclusion, this systematic review & meta-analysis study suggested that H. pylori infection was associated with the risk of diabetes as compared to non- diabetes individual.

AbstractIrisin, an emerging adipokine, has been involved in the pathogenesis of type 2 diabetes mellitus (T2DM). However, previous studies evaluating the association between irisin and diabetic nephropathy (DN) showed inconsistent results. We performed a meta-analysis to evaluate the above association. Matched case-control studies evaluating the difference of serum irisin between T2DM patients with and without DN were identified via systematic search of PubMed, Embase, Cochranes’ Library, China National Knowledge Infrastructure, and WanFang databases from inception to December 5, 2020. A random-effects model or a fixed-effects model was used to pool the results according to the heterogeneity. Overall, thirteen matched case-control studies including 1735 T2DM patients were included. Results of meta-analysis showed that compared to T2DM patients with normoalbuminuria, those with microalbuminuria [10 studies, standard mean difference (SMD): 1.12, 95% confidence interval (CI): 0.48–1.77, p<0.001; I2=94%] and macroalbuminuria (10 studies, SMD: 1.86, 95% CI: 0.93–2.79, p<0.001; I2=97%) had significantly lower serum irisin. Besides, the serum level of irisin was significantly lower in T2DM patients with macroalbuminuria than those with microalbuminuria (10 studies, SMD: 0.91, 95% CI: 0.44–1.38, p<0.001; I2=90%). In addition, patients with estimated glomerular infiltration rate (eGFR)<60 ml/min 1.73 m2 had lower serum irisin compared to those with eGFR≥60 ml/min 1.73 m2 (4 studies, SMD: 0.89, 95% CI: 0.32–1.46, p=0.002; I2=91%). In conclusion, serum irisin may be associated with albuminuria and reduced eGFR in T2DM patients.

Previous reports have suggested a potential association on dietary zinc intake with the risk of pancreatic cancer. Since the associations between different studies were controversial, we therefore conducted a meta-analysis to reassess the relationship between dietary zinc intake and pancreatic cancer risk. A comprehensive search from the databases of PubMed, Embase, Web of Science, and Medline was performed until January 31, 2017. Relative risk (RR) with 95% confidence intervals (CI) derived by using random effect model was used. Sensitivity analysis and publication bias were conducted. Our meta-analysis was based on seven studies involving 1659 cases, including two prospective cohort studies and five case–control studies. The total RR of pancreatic cancer risk for the highest versus the lowest categories of dietary zinc intake was 0.798 (0.621–0.984), with its significant heterogeneity among studies (I2=58.2%, P=0.026). The average Newcastle–Ottawa scale (NOS) score was 7.29, suggesting a high quality. There was no publication bias in the meta-analysis about dietary zinc intake on the risk of pancreatic cancer. Subgroup analyses showed that dietary zinc intake could reduce the risk of pancreatic cancer in case–control studies and among American populations. In conclusion, we found that highest category of dietary zinc intake can significantly reduce the risk of pancreatic cancer, especially among American populations.

The pooled estimate of the association between H. pylori infection with diabetes was OR=1.27 (95% CI 1.11 to 1.45, P = 0.0001, I2 = 86.6%). The effect of H. pylori infection on diabetes mellitus (both types), type 1 and type 2 diabetes was 1.17 (95% CI 0.94 to 1.45), 1.19 (95% CI 0.98 to 1.45), and 1.43 (95% CI 1.11 to 1.85) respectively. Subgroup analysis by the geographical regions showed in Asian population risk of the effect of H. pylori infection on diabetes was slightly higher than other population.Conclusion In overall a positive association between H. pyloriinfection and diabetes mellitus was found.
