SUMMARYThe first recognized outbreak of haemorrhagic colitis due toEscherichia coliO 157. H7 in the United Kingdom affected at least 24 persons living in East Anglia over a 2-week period. Theillnesses were characterized by severe abdominal pain and bloody diarrhoea of short duration. Eleven patients were admitted to hospital and there was one death. Patients were mainly adult women who had not eaten out of the home in the 2 weeks before onset. Unlike previously reported outbreaks hamburgers were not the vehicle of infection, and a case-control study suggested that handling vegetables, and particularly potatoes, was the important risk factor.

A 10-year invasive pneumococcal disease (IPD) enhanced surveillance project in the Oxfordshire region of the UK between 1996 and 2005 identified a total of 2691 Streptococcus pneumoniae isolates from all ages that provided a comprehensive description of pneumococcal epidemiology. All isolates were serotyped and those from children under 5 years of age were genotyped and a matched case–control study using adults hospitalized between 1995 and 2000 was performed to estimate the effectiveness of the pneumococcal polysaccharide vaccine in the local population. Fifty-one serotypes were isolated, with different age distributions. The overall incidence of IPD was 9.2 cases per 100 000 population per annum [95 % confidence interval (CI), 8.6–9.9] and that of meningitis was 0.7 per 100 000 population per annum (95 % CI 0.5–0.9). After adjusting for age, serotype 1 was found to be less likely to be associated with meningitis versus other IPD, compared with the most common serotype 14, whereas serotype 12F was more likely to cause meningitis than other IPD. There were significant temporal changes in IPD incidence of four serotypes, with decreases in serotypes 1, 12F and 14 and increases in serotype 8. A possible novel variant (from serotype 6A to 6B) was found using multilocus sequence typing analysis. From the matched case–control study of adults, the pneumococcal polysaccharide vaccine effectiveness was estimated to be 43 % (2–68 %), which did not change significantly after adjustment for pre-existing co-morbidities. The data provide a baseline against which the impact of the pneumococcal conjugate vaccine introduced in the UK in 2006 could be measured.

Abstract Background The role of therapeutic intervention, particularly antibiotics, for Shiga toxin-producing Escherichia coli (STEC) related infection is controversial. Methods We performed a population based matched case-control study to assess the association between treatment (antibiotics, antidiarrheal agents and probiotics) for STEC related infections and HUS development. We identified all STEC HUS patients as cases and matched five non-HUS patients as controls using the data from the National Epidemiological Surveillance of Infectious Diseases (NESID) between January 1, 2017, and December 31, 2018. Further medical information was obtained by standardized questionnaires answered by physicians who registered each patient. We used multivariate conditional logistic regression model to evaluate the association between exposures (use of antibiotics, use of antidiarrheal agents, days between disease onset and fosfomycin administration [within two or three days]) and the development of HUS, by matched odds ratios (OR) and 95% confidence intervals (CI). Covariates we used were sex, age group, area code, presence of diarrhea and other factors. We also performed subgroup analyses using age (adults and children) as a stratification factor. Results 7,760 STEC related patients were registered in the NESID. We selected patients who had a record of HUS diagnosis (n=182) and matched controls without HUS (n=910). After collecting standardized paper-based questionnaires, we enrolled 90 HUS patients and 371 non-HUS patients for analysis. In the main analysis, matched OR of fosfomycin was 0.75(0.47-1.20) in all ages, 1.41(0.51-3.88) in adults and 0.58(0.34-1.01) in children. Matched OR of antidiarrheal agents was 2.07(1.07-4.03) in all ages, 1.84(0.32-10.53) in adults, 2.65(1.21-5.82) in children. Matched OR of probiotics was 0.86(0.46-1.61) in all ages, 0.76(0.21-2.71) in adults, 1.00(0.48-2.09) in children. There was no significant association between the timing of fosfomycin use in the first two or five days of illness and HUS development in any age group. Conclusion Our results suggest that fosfomycin might decrease the risk of HUS in children younger than 15 years of age with STEC confirmed bacterial gastroenteritis. Background: Adolescent pregnancy, pregnancy in women aged between 13-19 years, is considered a major health problem. Objective: This study was performed to determine the maternal, perinatal, and neonatal outcomes of adolescent pregnancies. Materials and Methods: The present hospital-based case-control study was performed on pregnant women who were referred to Fatemieh Hospital in Hamadan City for delivery. The case group was pregnant women whose gestational age was less than 19 years (adolescent pregnancy) and the control group included pregnant women whose gestational age was at 19-35 years (adult women pregnancy). Data were analyzed using Stata software version 14. The significant level was considered less than 0.05. Results: The unwanted pregnancy was significantly higher in adolescent women (45% vs. 28.5%, P=0.002). 18% of adolescent women had the experience of spouse violence compared to 4.25% in the control group (P<0.001). Accessing prenatal care and supplements were significantly higher in adult women (P<0.05). The adult mothers had significantly higher cesarean section delivery compared adolescent group (54.91% vs. 33%, P<0.001). Also, there was a significant association between postpartum hemorrhages and the study group (22% in cases compared to 13.75% in the control group, P=0.04). The Apgar score 1 minute was significantly higher in adult mothers (8.05±2.08 vs. 7.39±2.05, P<0.001). Conclusion: In conclusion, adolescent mothers in the west of Iran had a higher risk of inadequate prenatal care, low Apgar score and postpartum hemorrhage. Therefore, society should focus on public health policies that can improve the adequacy of prenatal care among adolescents.
