Abstract Background The deregulation of immune system cell response implies loss of T-cell apoptosis, high rate of proinflammatory cytokines production and subsequent exacerbate activation of TNF-α pathway. The use of biologic antibody decrease inflammation rate and symptoms, but it remains unclear if it has a direct effect on the pathways activation/inactivation on peripheral blood mononuclear cells (PBMCs). The aim of this study is evaluate the role of nuclear magnetic resonance spectroscopy (NMR) applied to the metabolomic study of serum samples isolated from fresh blood from inflammatory bowel disease (IBD) patients under IFX treatment to understand the activated/inactivated pathways of PBMCs. Methods A case–control study was performed. Inclusion criteria were IBD patients under IFX treatment. Blood samples were obtained in Crohn’s disease (CD) and ulcerative colitis (UC) patients before IFX and in healthy controls (CTRL). CD patients were divided into subgroups according to the gut affected, in Ileocolic (IC), ileum and colon. NMR samples of the serum were collected and measured according to Standard Operation Procedures. Three types of NMR spectra were measured for each serum sample (1Hnoepresat, 1Hcpmgpresat and 1HDfilterpresat). The signal in each NMR spectrum was integrated in a series of equidistant little portion of the spectrum called buckets of a constant width of 0.04 ppm, covering the complete 1H NMR spectral window from −5 to 14 ppm. Buckets in regions depleted from signal at the two extremes of the spectrum were discarded as well as those in the proximity of the water peak at ca. 4.7 ppm which was affected by the presaturation. The vectors corresponding to a number of samples of two or more groups can be rapidly analysed using Multivariant Statistical Analysis methods. Results Twenty-two IBD patients (12 CD and nine UC) were included, 10 CTRL were also included. The metabolomic analyses of the NMR spectra of the serum of the different patients and control groups by the fingerprinting and targeting profiling strategies provided OPLS-DA statistical models (Figure 1) that permitted the successful classification of certain groups of samples which are summarised in Table 1. Conclusion The results of this pilot NMR metabolomic study of serum samples of IBD found a series of spectral fingerprints that are able to discriminate between groups of patients CTRL and CD, which underlines its potential use for the diagnosis of the disease.

ContextChronic starvation is characterized by GH resistance, and obesity is characterized by decreased GH secretion. In both extremes, IGF1 levels may be low and androgen levels may be abnormal.ObjectiveTo investigate the determinants of IGF1 and GH across the weight spectrum in women.DesignCross-sectional study.SettingClinical research center.Study participantsIn total, 32 women had participated in the study: 11 women with anorexia nervosa (AN), 11 normal-weight women, and 10 obese women of comparable mean age.InterventionNone.Main outcome measuresPooled hourly overnight serum samples assayed for IGF1, GH, estradiol (E2), testosterone, SHBG, insulin, free fatty acids, and trunk fat.ResultsFree testosterone was higher in obese women and lower in women with AN than in normal-weight women, and was the only independent (and positive) predictor of IGF1 levels, accounting for 14% of the variability (P=0.032) in the group as a whole. This relationship was stronger when obese women were excluded, with free testosterone accounting for 36% of the variability (P=0.003). Trunk fat accounted for 49% of the variability (P<0.0001) of GH, with an additional 7% of the variability attributable to E2 (P=0.042) in the group as a whole, but was not a significant determinant of GH secretion when obese women were excluded.ConclusionsFree testosterone is a significant determinant of IGF1 levels in women across the body weight spectrum. In contrast, GH secretion is differentially regulated at the extremes of the weight spectrum.

Fragestellung: Erwachsene mit Anorexia nervosa (AN) zeigen vor und nach Gewichtsrehabilitation Beeinträchtigungen kognitiver Funktionen. Im Bereich der kindlichen und früh-adoleszenten AN besteht ein großer Bedarf an strukturierten Untersuchungen der kognitiven Funktionen. Bisherige Studien weisen methodische Inkonsistenzen bezüglich der Testauswahl und der Operationalisierung kognitiver Funktionen auf, die die Interpretierbarkeit und Vergleichbarkeit der Befunde deutlich einschränken. Um diese Inkonsistenzen zu verringern, wurde eine neuropsychologische Testbatterie, das sogenannte „Ravello Profil“ entwickelt, das bisher jedoch nicht für den deutschen Sprachraum zur Verfügung steht. Die vorliegende Arbeit stellt daher eine für den deutschen Sprachraum adaptierte Version des Ravello Profils vor und überprüft dessen Anwendbarkeit im Kindes- und Jugendalter. Methodik: Das Ravello Profil wurde für den deutschen Sprachraum adaptiert. Anhand von drei Fallbeispielen wurde die Durchführbarkeit des Ravello Profils bei Kindern und Jugendlichen mit AN überprüft. Ergebnisse und Schlussfolgerungen: Die Fallbeispiele verdeutlichen die Anwendbarkeit des adaptierten Ravello Profils bei Kindern und Jugendlichen mit AN. Das Ravello Profil ermöglicht somit auch im deutschen Sprachraum methodisch konsistente Untersuchungen kognitiver Funktionen bei Kindern, Jugendlichen und Erwachsenen mit AN. Mithilfe des Ravello Profils kann die Rolle kognitiver Funktionen bei der Entstehung einer AN über eine umfassende Altersspanne systematisch untersucht werden.

Objective: This study examines retrospective correlates of nonshared family environment prior to onset of disease, by means of multiple familial informants, among anorexia and bulimia nervosa patients. Methods: A total of 332 participants was included (anorexia nervosa, restrictive type (AN-R): n = 41 plus families); bulimic patients (anorexia nervosa, binge-purging type; bulimia nervosa: n = 59 plus families). The EATAET Lifetime Diagnostic Interview was used to establish the diagnosis; the Subjective Family Image Test was used to derive emotional connectedness (EC) and individual autonomy (IA). Results: Bulimic and AN-R patients perceived significantly lower EC prior to onset of disease compared to their healthy sisters. Bulimic patients perceived significantly lower EC prior to onset of disease compared to AN-R patients and compared to their mothers and fathers. A low family sum – sister pairs sum comparison – of EC had a significant influence on the risk of developing bulimia nervosa. Contrary to expectations, AN-R patients did not perceive significantly lower levels of IA compared to their sisters, prior to onset of disease. Findings of low IA in currently ill AN-R patients may represent a disease consequence, not a risk factor. Conclusions: Developmental child psychiatrists should direct their attention to disturbances of EC, which may be present prior to the onset of the disease.

Objective: Acute anorexia nervosa (AN) leads to reduced gray (GM) and white matter (WM) volume in the brain, which however improves again upon restoration of weight. Yet little is known about the extent and clinical correlates of these brain changes, nor do we know much about the time-course and completeness of their recovery. Methods: We conducted a meta-analysis and a qualitative review of all magnetic resonance imaging studies involving volume analyses of the brain in both acute and recovered AN. Results: We identified structural neuroimaging studies with a total of 214 acute AN patients and 177 weight-recovered AN patients. In acute AN, GM was reduced by 5.6% and WM by 3.8% compared to healthy controls (HC). Short-term weight recovery 2–5 months after admission resulted in restitution of about half of the GM aberrations and almost full WM recovery. After 2–8 years of remission GM and WM were nearly normalized, and differences to HC (GM: –1.0%, WM: –0.7%) were no longer significant, although small residual changes could not be ruled out. In the qualitative review some studies found GM volume loss to be associated with cognitive deficits and clinical prognosis. Conclusions: GM and WM were strongly reduced in acute AN. The completeness of brain volume rehabilitation remained equivocal.

Zusammenfassung. Die gravierende körperliche, psychische und psychosoziale Morbidität infolge von Anorexia nervosa wird von Betroffenen häufig trotz ausgeprägter Einschränkungen als weniger schwerwiegend wahrgenommen als von ihrem Umfeld. Ärzte und andere Fachpersonen im Gesundheitswesen sind daher regelmässig mit der Schwierigkeit konfrontiert, dass dringend notwendige medizinische Massnahmen von Betroffenen als unnötig oder sogar bedrohlich angesehen und abgelehnt werden. Obwohl Patientinnen und Patienten mit Anorexia nervosa sich in der Regel eine Verbesserung ihres Zustands wünschen, sind sie meist nur als Reaktion auf hohen äusseren Druck in der Lage, eine Behandlung, die auf eine Normalisierung des Essverhaltens und eine Gewichtszunahme abzielt, zuzulassen. Zur Behandlung der Patientinnen und Patienten ist in Anbetracht dieser Ausgangslage ein interdisziplinäres Team erforderlich, das Erfahrung mit diesen Behandlungen hat. Eine enge Zusammenarbeit ist nötig, um einen tragenden Behandlungsrahmen gewährleisten zu können.
