Our results show that negative returns to health outcomes set in at around 50 work hours per week, and that the negative effects of working long hours manifest earlier for women than men. Increased work hours are associated with higher incomes and better access to medical care. However, increased work hours also generate greater physical and mental stress, which may cause health problems. We examine these questions empirically with data from the 2006 wave of the National Longitudinal Survey of Youth (NLSY), using two-stage least squares to account for endogeneity of work hours and income in the health outcomes model. Abstract Introduction Regular sleep-wake patterns aid in the maintenance of optimal physical and mental health, by helping to align environmental, behavioural, and physiological rhythms. The distribution of sleep regularity across the population has not been well documented. Furthermore, researchers currently lack tools to easily quantify sleep regularity. Method We have described sleep regularity in 86 624 UK Biobank participants (age (M±SD) = 62.45±7.84; 56.2% female) using data from wrist-worn accelerometers. Regularity was measured using the Sleep Regularity Index (SRI), which quantifies day-to-day similarity in sleep-wake patterns, and which is linked to cardio-metabolic and mental health outcomes. We developed an R package to calculate SRI from accelerometer data, which works in conjunction with GGIR (a validated accelerometer processing tool) to identify sleep-wake state, including naps and broken sleep. Results The SRI distribution had M±SD = 78.02±11.53, and median = 80.49. The least regular quintile (SRI<70.2) had standard deviation of sleep onset = 2.23h, offset = 2.14h, and duration = 1.95h, compared with onset = 0.78h, offset = 0.85h, and duration = 0.95h in the most regular quintile (SRI>87.3). Approximately 14% of participants exhibited large day-to-day shifts in sleep timing (>3h) at least once per week. Discussion This is the largest description of sleep regularity to-date. The norms established here provide a reference for researchers and clinicians intending to quantify sleep regularity with the SRI. We have combined methods described here into an open-source R package to calculate SRI from accelerometer or sleep diary data, available for download via GitHub.

In 2017, there were over 65 million displaced persons at the global level, with approximately 23 million of these people living as refugees around the world. In this same year, the US resettled 53,716 refugees, with the State of Utah receiving 954 of those refugees. Refugees, in general, often face health-related challenges upon resettlement. Since the health of aging refugee men and women is of growing concern, host communities face significant challenges in accommodating the health needs of a diverse refugee population. This study, a review of physical and mental health data from the Utah Department of Health, was undertaken in an effort to ascertain the prevalence of health conditions among refugee men and women 60 years and older arriving in Utah. Findings include information on diseases correlated with increasing age, such as hypertension, decreases in vision, arthritis, and low back pain, which are common among this population of refugees aged 60 years and older. Overall, most of the health conditions assessed affect women and men with a similar prevalence. Some notable exceptions are a history of torture and violence, and a propensity for tobacco use. When dealing with refugee men older than 60 years, providers should consider the psychological ramifications of having endured such atrocities, as well as introduction to evidence-based tobacco cessation programs. When working with refugee women of the same age, an increase in the prevalence of musculoskeletal pain and urinary tract infections should be considered.
