A Preliminary Report on Outcomes of the American Institute for Stuttering Intensive Therapy Program

In this study, we documented the effects of a 3-week, intensive stuttering treatment program on overt and covert symptoms of stuttering and participants’ levels of social anxiety. This study was a preliminary report in which we used a single-group pretest-posttest design to analyze outcomes data with reference to each participant individually and described general group trends. Researchers gave 5 adult participants who stuttered a battery of self-report instruments and collected speech samples before and after treatment. We found that disfluencies lessened considerably for all participants, although most continued to stutter at levels above those of normally fluent speakers. All participants except for 1 were far less negatively affected by stuttering after treatment and greatly reduced their reported levels of social anxiety. Several participants in this sample displayed pathological levels of social anxiety pretreatment. Preliminary data indicate that this intensive program generally achieved its stated goals of facilitating improved stuttering/speech management and psychological/attitudinal management. We need further investigations to document the durability of the gains reported herein, as well as the clinical implications of social anxiety as a comorbid condition in some persons who stutter.

This study explored the effectiveness of storytelling in supporting children with unusual physical traits. Participants were forty-eight children, aged 9 – 12 who, due to various eye diseases, wear eyeglasses. They completed various standardized self-report measures, both before and after participation, in one of the six intervention groups. The measures assessed shyness and social anxiety symptoms, loneliness and social dissatisfaction, perception of negative evaluation, satisfaction with one’s appearance, and anxiety regarding physical appearance. The intervention consisted of six 90-min group sessions and included both individual and group activities. The results lend support to the hypothesis that storytelling can significantly contribute to the emotional well-being of children, with a distinct physical appearance.

ABSTRACT Intensive therapy is an alternative to accelerate the therapeutic process of individuals with cleft lip and palate. The purpose of this study is to describe an intensive speech therapy program and compare the results before and after the program on a child with previously operated right unilateral cleft lip and palate and velopharyngeal insufficiency, using a speech bulb. Sixty therapy sessions were carried out over a 4-week period. Before and after, anamnesis, speech assessment and evaluation of the velopharyngeal function with nasofibroscopy, were performed. The patient presented with adequate resonance and speech intelligibility, reduction of obligatory disorders and compensatory articulations, after intensive therapy with the speech bulb and reduction of velopharyngeal gap, after intensive therapy with and without the speech bulb. Progress was achieved with the intensive speech therapy.

Models of social anxiety emphasize anticipatory processing (AP) and post-event processing (PEP) as repetitive negative thinking (RNT) processes that occur before and after social-evaluative events, respectively. Both AP and PEP have been implicated as maladaptive processes which maintain social anxiety. Accordingly, a common vulnerability, such as poor attentional control, may serve to maintain both. The present research included two separate samples to investigate the relationship between attentional control and AP (Sample 1) and PEP (Sample 2). Participants ( n = 49 for Sample 1; n = 35 for Sample 2) completed self-report measures of social anxiety, AP or PEP, attentional control, and trait anxiety. Poorer total attentional control was associated with social anxiety in both samples. In addition, attentional control total and attentional shifting were negatively associated with PEP (Sample 2) but not with AP (Sample 1). Mediation modeling suggested that trait anxiety mediated the relationship between total attentional control and PEP. The findings suggest that attentional control plays a role in the regulation of emotion, such as anxiety, and RNT processes, such as PEP, but not AP.

Purpose The aim of the study was to assess how the use of a mild-gain hearing aid can affect hearing handicap, motivation, and attitudes toward hearing aids for middle-age, normal-hearing adults who do and do not self-report trouble hearing in background noise. Method A total of 20 participants (45–60 years of age) with clinically normal-hearing thresholds (< 25 dB HL) were enrolled in this study. Ten self-reported difficulty hearing in background noise, and 10 did not self-report difficulty hearing in background noise. All participants were fit with mild-gain hearing aids, bilaterally, and were asked to wear them for 2 weeks. Hearing handicap, attitudes toward hearing aids and hearing loss, and motivation to address hearing problems were evaluated before and after participants wore the hearing aids. Participants were also asked if they would consider purchasing a hearing aid before and after 2 weeks of hearing aid use. Results After wearing the hearing aids for 2 weeks, hearing handicap scores decreased for the participants who self-reported difficulty hearing in background noise. No changes in hearing handicap scores were observed for the participants who did not self-report trouble hearing in background noise. The participants who self-reported difficulty hearing in background noise also reported greater personal distress from their hearing problems, were more motivated to address their hearing problems, and had higher levels of hearing handicap compared to the participants who did not self-report trouble hearing in background noise. Only 20% (2/10) of the participants who self-reported trouble hearing in background noise reported that they would consider purchasing a hearing aid after 2 weeks of hearing aid use. Conclusions The use of mild-gain hearing aids has the potential to reduce hearing handicap for normal-hearing, middle-age adults who self-report difficulty hearing in background noise. However, this may not be the most appropriate treatment option for their current hearing problems given that only 20% of these participants would consider purchasing a hearing aid after wearing hearing aids for 2 weeks.

Abstract The purpose of this study was to test the hypothesis that adults who stutter will exhibit significant improvements after attending a residential, 3-week intensive program that focuses on avoidance reduction and stuttering modification therapy. Preliminary analyses focused on four measures: (a) SSI-3, (b) speech rate, (c) S-24 Scale, and (d) OASES. Results indicated significant improvements on all of the measures.

Abstract. The current study examined the validity of two self-report measures of social anxiety constructed using social comparative referent points. It was hypothesized that these comparison measures would be both reliable and valid. Results indicated that two different comparative versions – one invoking injunctive norms and another invoking descriptive norms – showed good reliability, excellent internal consistency, and acceptable convergent and discriminant validity. The comparative measures also predicted positive functioning, some aspects of social quality of life, and social anxiety as measured by an independent self-report. These findings suggest that adding a comparative reference point to instructions on social anxiety measures may aid in the assessment of social anxiety.

Abstract Background There is a strong research imperative to investigate effective treatment options for adolescents and adults with autism spectrum disorder (ASD). Elevated social anxiety, difficulties with social functioning and poor mental health have all been identified as core treatment targets for this group. While theoretical models posit a strong bidirectionality between social anxiety and ASD social functioning deficits, few interventions have targeted both domains concurrently. Of the two group interventions previously conducted with adolescents and adults with ASD, significant results have only been observed in either social anxiety or social functioning, and have not generalised to changes in overall mood. The aim of this study was to evaluate the potential benefit, tolerability and acceptability of a group cognitive-behaviour therapy (CBT) intervention in young adults with ASD. Primary treatment outcomes were social anxiety symptoms and social functioning difficulties; secondary outcomes were self-reported mood and overall distress. Method Ten groups of participants completed an eight-week, modified group CBT intervention targeting both social anxiety and social functioning, that included social skills training, exposure tasks and behavioural experiment components. Seventy-eight adolescents and young adults with ASD, without intellectual impairment, aged between 16 and 38 (M = 22.77; SD = 5.31), were recruited from the community, Headspace centres and the Autism Clinic for Translational Research at the Brain and Mind Centre, University of Sydney. Outcomes (social anxiety, social functioning and mood) were measured pre- and post-intervention via self-report questionnaires (administered either online or through the return of hard-copy booklets), and participants were invited to provide anonymous feedback on the intervention (at the mid-point and end of the intervention). Results Participants demonstrated statistically significant improvements on all outcome measures in response to the intervention. Specifically, social anxiety symptoms decreased (p < .001), and specific subdomains of social functioning improved post-intervention, particularly in social motivation (p = .032) and restricted interests and repetitive behaviours (p = .025). Self-reported symptom improvements also generalised to mood (depression, anxiety and stress; p < .05). All improvements demonstrated small effect sizes. Participant feedback was positive and indicated strong satisfaction with the program. Limitations The absence of a control group and follow-up measures, reliance on self-report instruments as outcome measures and the exclusion of those with intellectual disability represent significant limitations to this study. Conclusions These findings indicate that a group CBT intervention appears to be a beneficial intervention for self-reported social anxiety, social functioning and overall mental health in adolescents and young adults with ASD. The stand-alone nature of the intervention combined with positive participant feedback indicates it was well tolerated, has potential clinical utility and warrants further study in a randomised-controlled, follow-up design.
