Client Safety and Telepractice in a Clinic or Home Setting

Telepractice is an exciting addition to the arsenal of speech-language pathologists for delivering services. Efficacy data continues to emerge proving the benefit of using available technology to provide assessment and treatment for persons with a variety of communication disorders, ages, and gender. In addition to providing assessment and treatment using telepractice technology, several professional issues have arisen which must be addressed before implementation of this service delivery system. Licensure and reimbursement have been at the forefront, as well they should. However, client safety is another issue that should also be addressed. Providing speech pathology services in a safe environment is a concept which may not have been considered before technological advances made it possible to provide services to someone while not being physically present.

Rates of anxiety disorders are significantly elevated among sexual and gender minorities. In this chapter, the minority stress model is discussed as a framework for conceptualizing anxiety among sexual and gender minorities, and the authors review the literature on the relationships between specific minority stressors and symptoms. The authors examine prevalence rates of anxiety disorders among sexual minorities and gender minorities, separately and in comparison to heterosexual and cisgender individuals. Also reviewed is the literature on anxiety disorders among sexual and gender minorities with a racial or ethnic minority status. Current assessment and treatment approaches are identified and reviewed. Finally, limitations to the current literature base are discussed and recommendations are provided for future studies.

The story collection known in the West as The Arabian Nights or One Thousand and One Nights, is famous, among other things, for its erotic playfulness. This eroticism was (and is) one of the key reasons for its continuous popularity after Antoine Galland’s French translation in 1704. The Arabian Nights includes, besides traditional, heterosexual acts, play, and desires, examples of homoerotic playfulness—even though we must tread lightly when using such Western concepts with an oriental text body such as this one. The homoerotic playfulness of The Arabian Nights is the subject of this article. By making use of a text-immanent analysis of two of the Nights’ stories—of Qamar and Budûr and of Alî Shâr and Zumurrud—the author of this article focuses on the reversal of common gender roles, acts of cross-dressing, and, of course, homoerotic play. He will argue that these stories provide a narrative safe environment in which the reader is encouraged to “experiment” with non-normative sexual and gender orientations, leaving the dominant status quo effectively and ultimately unchallenged, thus preventing the (self-proclaimed) defenders of that status quo from feeling threatened enough to actively counter-act the experiment.

Telepractice in speech-language pathology shows the potential to mitigate the current shortage of speech-language pathologists (SLPs) available to serve a growing number of persons with communication disorders.  Since a majority of American Speech-Language-Hearing Association (ASHA) certified SLPs work in schools and the population of communicatively impaired clients in schools continues to grow, research into the use of telepractice in the educational setting is warranted.  This article reports upon the perspectives of SLPs regarding the use of telepractice in school settings. In-depth qualitative interviews were conducted with five SLPs experienced in the delivery of telepractice. Four major themes emerged: barriers, benefits, reasons for acceptance and use of telepractice, and suggestions to resolve telepractice professional issues.

Practitioners working with gender non-conforming children and youth ascribe to general guidelines based on the World Professional Association for Transgender Health Standards of Care for the Health of Transsexual, Transgender, and Gender Nonconforming People (2012). These guidelines inform clinical practice and assessment and emphasize the need for gender affirming care, but they do not include strict treatment criteria. Consequently, there are multiple perspectives and approaches in the field regarding effective assessment and treatment of gender diverse and transgender clients. Given the ongoing debate around best practices, the current exploratory research study investigates the perspectives and satisfaction of transgender youth and their parents actively seeking out gender health assessments (e.g., hormone readiness assessments). Twenty-five parents and 22 youth who were accessing gender health services through a community outpatient clinic completed a questionnaire about the gender health assessment process. Survey data was analyzed using descriptive statistics, and portions analyzed using thematic analysis. Similar responsepatterns were found between groups and themes emerged surrounding the need for an individualized approach to care. This study aims to increase clinical understanding of the experiences of those seeking gender health assessment services to inform and improve practices to better serve this community.

BACKGROUND Given the interactive media characteristics and intrinsically motivating appeal, virtual serious games are often praised for their potential for assessment and treatment. OBJECTIVE This study aims to validate and develop normative data for a virtual serious game (Deusto-e-motion1.0) for the evaluation of emotional facial expression recognition and social skills, both of which are components of the theory of mind. METHODS A total of 1236 children took part in the study. The children were classified by age (8-12 years old), gender (males=639, females=597), and educational level (between the third and sixth years of Primary Education). A total of 10 schools from the Basque Country and 20 trained evaluators participated in this study. RESULTS Differences were found in Deusto-e-motion1.0 scores between groups of children depending on age and gender. Moreover, there was a moderately significant correlation between the emotional recognition scores of Deusto-e-motion1.0 and those of the Feel facial recognition test. CONCLUSIONS Deusto-e-motion1.0 shows concurrent validity with instruments that assess emotional recognition. Results support the adequacy of Deusto-e-motion1.0 in assessing components of the theory of mind in children.

Schizophrenia before the age of 18 years is usually divided into two categories. Early-onset schizophrenia (EOS) presents between the ages of 13 and 17 years, whereas very-early-onset schizophrenia (VEOS) presents at or before the age of 12 years. Previous studies have found that neurodevelopmental difficulties in social, motor, and linguistic domains are commonly observed in VEOS/EOS patients. Recent research has also shown a high prevalence of neurodevelopmental disorders (e.g., intellectual disability, communication disorders, autism spectrum disorder, neurodevelopmental motor disorders) in VEOS/EOS patients, indicating genetic overlap between these conditions. These findings lend support to the neurodevelopmental continuum model, which holds that childhood neurodevelopmental disorders and difficulties and psychiatric disorders (e.g., schizophrenia) fall on an etiological and neurodevelopmental continuum, and should not be considered discrete entities. Based on this literature, in this study we focused on the overlap between neurodevelopmental disorders and schizophrenia investigating, in a large sample (N = 230) of VEOS/EOS children and adolescents, the clinical differences, at the onset of psychosis, between VEOS/EOS with neurodevelopmental disorder or neurodevelopmental difficulties and VEOS/EOS with no diagnosed neurodevelopmental disorder or neurodevelopmental difficulties. The findings showed that, in children and adolescents with a neurodevelopmental disorder or neurodevelopmental difficulties, psychosis onset occurred at an earlier age, was associated with more severe functional impairment (e.g., global, social, role), and was characterized by positive symptoms (e.g., grandiose ideas, perceptual abnormalities, disorganized communication) and disorganized symptoms (e.g., odd behavior or appearance, bizarre thinking). Instead, in children and adolescents without a neurodevelopmental disorder or neurodevelopmental difficulties, psychosis onset was mainly characterized by negative symptomatology (e.g., social anhedonia, avolition, expression of emotion, experience of emotions and self, ideational richness). Given these differences, the presence of a neurodevelopmental disorder or neurodevelopmental difficulties should be carefully investigated and integrated early into the assessment and treatment plan for VEOS/EOS patients.

With technological possibilities in healthcare steadily increasing, more tools for digital cognitive rehabilitation become available. Acceptance of such technological advances is crucial for successful implementation. Therefore, we examined technology acceptance specifically for this form of rehabilitation in a sample of healthcare providers involved in cognitive rehabilitation. An adjusted version of the Technology Acceptance Model (TAM) questionnaire was used, including the subscales for perceived usefulness, perceived ease of use, subjective norm (toward use), and intention to use, which all contribute to actual use of a specific technology. Results indicate a generally favorable attitude toward the use of digital cognitive rehabilitation and positive responses toward the TAM constructs. Only for subjective norm, a neutral mean response was found, indicating that this could pose a potential obstacle toward implementation. Potential differences between subgroups of different age, gender, and professional background were assessed. Age and gender did not affect the attitude toward digital cognitive rehabilitation. Occupational therapists showed lower scores than healthcare psychologists and physiatrists with regard to perceived usefulness, possibly linked to a difference in operational and managerial tasks. The findings of his study stimulate further implementation of digital cognitive rehabilitation, where the role of subjective norms should be specifically considered.

Disorders of development include a range of disorders first evidenced in childhood. Although most disorders have their origins in childhood, a few fully express themselves before early adulthood. This chapter describes the nature, assessment, and treatment of the more common disorders that are revealed in a clinically significant way during a child’s developing years. The disorders of development affect a range of functioning from single skills deficits to more pervasive problems that negatively impact a child’s ability to function. Included is coverage of several disorders usually diagnosed first in infancy, childhood, or adolescence, including the neurodevelopmental disorders (e.g., attention-deficit/ hyperactivity disorder, autism spectrum disorder, communication disorders, intellectual disability, and specific learning disorder) and the disruptive, impulse control, and conduct disorders (e.g., oppositional defiant disorder, conduct disorder). Recommendations for future research on the potential for advancing knowledge regarding spectrums within some of these disorders as well as recommendations for treatment are outlined.
