The contribution of a special library project to a computerized problem-oriented medical information system (PROMIS) is discussed. Medical information displays developed by the PROMIS medical staff are accessible to the health care provider via touch screen cathode terminals. Under PROMIS, members of the library project developed two information services, one concerned with the initial building of the medical displays and the other with the updating of this information. Information from 88 medical journals is disseminated to physicians involved in the building of the medical displays. Articles meeting predetermined selection criteria are abstracted and the abstracts are made available by direct selective dissemination or via a problem-oriented abstract file. The updating service involves comparing the information contained in the selected articles with the computerized medical displays on the given topic. Discrepancies are brought to the attention of PROMIS medical staff members who evaluate the information and make appropriate changes in the displays. Thus a feedback loop is maintained which assures the completeness, accuracy, and currency of the computerized medical information. The development of this library project and its interface with the computerized health care system thus attempts to deal with the problems in the generation, validation, dissemination, and application of medical literature.

The eHealth is a rapidly evolving and changing field. The different models and protocols of medical information system (MIS) are now available. The MIS and its everyday usage is extremely beneficial for the healthcare sector, but its introduction in everyday practice is slow. Cloud computing is perspective field and it differs from the conventional distributed computing by its focus on large-scale sharing, innovative applications, and high-performance orientation. This paper defines “cloud computing” as flexible, secure, coordinated resource for data sharing among dynamic collections of individuals, institutions, and etc. It should be emphasized, that cloud computing can be reviewed as a useful tool for sharing of computing resources and services. The author expects that this technology has huge potential in health care and can improve health care services, as well as provide benefits for health care research. It is proposed that the solution is the widespread and routine implementation of the medical information system. The present article will discuss the application of cloud computing for the medical information system practical usage. The goal in the information age must be to create knowledge from medical information with less time managing medical information and data. The availability of adaptable technology and possibility of improvement of the infrastructure conditions is the basis for medical informatics applications. The routine application of MIS by healthcare organization holds the potential to improve, develop and realize medical service in the effective and comprehensive mode.

Objective: The objective of the study was to determine the effect of depression on the utilization of health care resources, after adjusting for age and comorbidity from data obtained on routine clinical practice. Method: The study is an observational cohort of 15,186 patients followed over a one-year period beginning December 1993. Comprehensive demographic, clinical, and utilization data were available from the computerized medical information system generated database of a general internal medicine practice in an urban academic medical center. Results: Four point seven percent of patients carried a provider-coded diagnosis of depression. With regards to utilization of health care resources, even after controlling for age and comorbidity, depressed patients had more primary care visits (5.3 vs. 2.9 visits, p < .001), higher rates of referral to specialists (1.1 vs. 0.5, p < .002), and radiologic tests (0.9 vs. 0.4 tests, p < .001). They had higher total outpatient charges ($1,324 vs. $701, p < .001) and total charges ($2,808 vs. $1,891, p < .001). Depressed patients also had longer length of stay when hospitalized (14.1 vs. 9.5 days, p < .002). Conclusions: Patients diagnosed as depressed had significantly higher resource utilization of all types, even after controlling for the higher burden of comorbid medical illness associated with depression.

Big data analysis has enhanced its demand nowadays in various sectors of health-care including pharmacovigilance. The exact definition of big data is not known to many people though it is routinely used by them. Big data refer to immense and voluminous computerized medical information which are obtained from electronic health records, administrative data, registries related to disease, drug monitoring, etc. This data are usually collected from doctors and pharmacists in a health-care facility. Analysis of big data in pharmacovigilance is useful for early raising of safety alerts, line listing them for signal detection of drugs and vaccines, and also for their validation. The present paper is intended to discuss big data analytics in pharmacovigilance focusing on global prospect and domestic country-India.

Охрана здоровья граждан – одна из ключевых тем развития государства. О необходимости выстроить систему здравоохранения на новой технологической базе заявил в Послании Федеральному Собранию 21.04.2021 Президент РФ Владимир Путин. Здравоохранение представляет собой достаточно сложную и динамическую систему, которая требует постоянной оценки ситуации и быстрого принятия решений. Непрерывная оценка эпидемиологической ситуации и мониторинг здоровья конкретного гражданина невозможен без качественно организованного обмена информацией и единой системы данных, внедренных в повседневную работу медицинских учреждений. На данный момент существует большое количество медицинских информационных систем (МИС), однако, многие из них содержат избыточный функционал и специфичный формат хранения данных. Адаптировать существующие МИС под нужды конкретных регионов довольно проблематично как в трудо- и времязатратах, так и финансовом плане. Была поставлена задача разработать медицинскую информационную систему Астрал.Мед, направленную на решение актуальных задач в сфере оказания первичной медико-санитарной помощи. Одной из задач разработки является возможность передачи и приема различной медицинской информации в уже существующие МИС с целью, в конечном итоге, отправки этих данных в Единую государственную информационную систему в сфере здравоохранения. В данном случае должно реализовываться единое хранилище всех данных пациентов внезависимости от медицинского учреждения, в котором они обслуживаются, и от информационной системы в данном учреждении. Реализация системы Астрал.Мед состоит из: разработки модуля интерфейсной части; разработки базы данных и способа взаимодействия с ней; модуля обработки запросов от специалистов медицинского учреждения и модуля интеграции со сторонними МИС (что является одной из самых основных и сложных задач из-за возможной несовместимости форматов хранения данных в различных информационных системах). Protection of the health of citizens is one of the key topics in the development of the state. The need to build a health care system on a new technological base was announced by the President of the Russian Federation Vladimir Putin in his Address to the Federal Assembly on April 21, 2021. Health care is a rather complex and dynamic system that requires constant assessment of the situation and quick decision-making. A continuous assessment of the epidemiological situation and monitoring of the health of a particular citizen is impossible without a well-organized exchange of information and a unified data system introduced into the daily work of medical institutions. At the moment, there are a large number of medical information systems (MIS), however, many of them contain redundant functionality and a specific data storage format. It is quite problematic to adapt existing IIAs to the needs of specific regions, both in terms of labor and time costs, and financially. The task was to develop a medical information system Astral.Med, aimed at solving urgent problems in the field of primary health care. One of the development tasks is the possibility of transferring and receiving various medical information to the already existing MIS with the aim, ultimately, of sending this data to the Unified State Information System in the field of health care. In this case, a single repository of all patient data should be implemented, regardless of the medical institution in which they are served and the information system in this institution. Implementation of the Ast-Ral.Med system consists of: development of the interface module; developing a database and a way to interact with it; a module for processing requests from specialists of a medical institution and a module for integration with third-party MIS (which is one of the most basic and complex tasks due to the possible incompatibility of data storage formats in various information systems).

The aim to provide valid, relevant, permanent, timely information to all health care institutions, as well as citizens of Ukraine in the framework of public health management processes for quality medical care. Sociological method - allows you to study the social structure and its impact on health. Systems analysis as a scientific method of cognition, which makes it possible to establish structural connections between system elements. Medical information system is a type of information system that differs in a set of methodological techniques, techniques and management algorithms designed to collect, store, process and transmit information in health care facilities. An single information system ensures the provision of reliable information in the right amount, in the right place, at the right time for members of the health care system. One of the important factors in the implementation of health care reform is the electronic health care system (E-Health). E-Health consists of two interconnected parts, one of which - the central database - will be controlled by the state. Institutions will have access to it through the second part, which is called privately developed medical information systems. The National Health Service of Ukraine ensures the functioning of the electronic health care system and awebsite containing information on the electronic health care system. Health care informatization today is an integral, perhaps the main, component of any health care reform in today’s world. Creating a single information space has many advantages.
