Working in partnership with vulnerable families: the experience of child and family health practitioners

Family circumstances in infancy are persistent and powerful determinants of children’s physical and mental health, influencing inequalities that trace from childhood through to adulthood. While the social factors that perpetuate patterns of inequality are more complex than can be addressed through single interventions, child and family health (CFH) services represent crucial sites where trajectories of inequality can be disrupted. In particular, approaches that foster opportunities for practitioner–parent engagement that challenge traditional hierarchical health care practice, such as the Family Partnership Model (FPM), are recommended as ways of addressing disadvantage. Little is known about how practitioners implement models of working in partnership with families and, consequently, there is a gap in understanding how best to develop and sustain these new CFH practices. This paper reports a research project that investigated the experiences of 25 health professionals working within a FPM framework with vulnerable families. Through discussion of four key themes – redefining expertise, changing practices, establishing new relationships with parents and the complexities of partnership practice – the paper offers first-hand accounts of reframing practices that recognise the needs, skills and expertise of parents and thus contribute to empowerment of families.

This study sought to investigate and analyze the conceptions of professionals from family health teams regarding violence against women and care practice, through participant research, undertaken in a city located in the northwest region of the state of Río Grande do Sul, Brazil. A total of 30 professionals participated, in eight meetings, involving a process of reflection and learning, held in September - December 2012. The data were analyzed using content analysis. The professionals understand that violence against women exists due to the hierarchy and the relationships between men and women, which confers more power on men, a situation which legitimates male practice of violence. The care practice is based on a biological conception of the problem, with a tendency to the social. The care is directed towards the treatment of physical injuries, listening, guidance, denunciation, and referral to specialized health departments and the social service.

This analytical and qualitative study aimed to identify how workers in the Family Health Strategy perceive the risks they are exposed to at work. Thematic analysis and the reference framework of the work process were used to examine the contents of interviews with 48 subjects (community health agents, nurses, nursing auxiliaries and physicians). The workers noticed the following risks: physical and moral violence, typical work accident, emotional exhaustion, lack of problem-solving ability and occupational disease. The results predominantly show the connection with the object / subject of the work and the social environmental characteristics of the related communities. The study adds to the body of knowledge and can systemize collective and individual technological processes for occupational health risk management in primary health care.

Objetivo: identificar qual a conduta assistencial do enfermeiro frente à amamentação cruzada. Metodologia: estudo qualitativo, realizado com seis enfermeiras atuantes da Estratégia de saúde da família por meio de um grupo focal em de Duque de Caxias. Resultados: aponta a carência do aconselhamento na conduta assistencial frente à amamentação cruzada e o silenciamento sobre os motivos da não recomendação da mesma. Conclusão: Recomenda-se mudança nos discursos dos enfermeiros para a busca dos motivos que desencadearam a prática da amamentação cruzada para assim poderem intervir, visto que na maioria das vezes é praticada pela não efetividade de seus direitos.Descritores: Aleitamento materno; Aconselhamento; Enfermagem; Bioética. Objective: identify the nurse’s care practice towards cross-breastfeeding.Methodology: a qualitative study carried out with six nurses working in the Family Health Strategy through a focus group in Duque de Caxias. The family as the unit in the social system has an important role and becomes the first social environment to introduce love, affection, social culture, and religion. When the world is suffering from the COVID-19 pandemic, the families are also affected by some impacts included: family health problems; family economic; family harmonization; family socio-psychology; and socio-culture. The family health is composed of fundamental dimensions: family social climate; family integrity; family functioning; family resistance and family coping. The family coping concept is the capacity of the family to confront, mobilize, and act on stressful events. This article illustrates the aspects of family health and coping strategies in the midst of the COVID-19 in Indonesia. It aims to inform and be the reference for further study related to the family adaptation in the corona crisis. We reviewed many works of literature in the form of articles from various journals. During the COVID-19 pandemic, families must be able to survive. COVID-19 pandemic does not only affect physical but also mental health. The way to deal with the events depend on family characteristics, the severity of the events and family support. There are two types of coping strategies that families in Indonesia usually adopt including reducing expense and increasing income.

Tendo em vista a aproximação da ESF com o social e a preocupação em promover o acesso a saúde voltando-se prioritariamente para os grupos sociais mais vulneráveis pretende-se com este artigo: discorrer sobre a questão social e sua relação com fenômeno da desigualdade social; identificar a contribuição da Estratégia de Saúde da Família no enfrentamento das desigualdades sociais e sua relação com os determinantes sociais da saúde. Trata-se de uma reflexão teórica sobre a questão social e a Estratégia de Saúde da Família como modelo para redução das iniquidades em saúde. Optou-se por uma revisão de literatura, do tipo narrativa em que os artigos selecionados abordaram os temas: determinantes sociais em saúde, questão social e estratégia de saúde da família. Os resultados apontam que a questão social está intimamente relacionada com capital e as relações sociais impostas pelo mesmo e que tal processo é histórico no que diz respeito ao desenvolvimento da sociedade brasileira. Tendo em vista a iniquidade em saúde como uma das faces da questão social, as políticas de saúde apontam a ESF como uma das formas de enfrentar a desigualdade social dado que, nos determinantes sociais encontramos a expressão da questão social, e com uma prática pautada nos princípios da territorialização, do diagnóstico comunitário, da intersetorialidade e da equidade tal modelo pode ser promissor e resolutivo frente as demandas sociais as quais se relacionam com a saúde.   Considering the approximation of the FHS to social issues and the concern with promoting access to health care focusing primarily on the most vulnerable social groups, this article aims to discuss the social issue and its relation to the phenomenon of social inequality; to identify the contribution of the Family Health Strategy in combating social inequalities and its relation to the social determinants of health. This is a theoretical reflection on the social issue and the Family Health Strategy as a model for reducing health inequities. We chose a narrative literature review, in which the selected articles addressed the following themes: social determinants of health, social issue, and family health strategy. The results point out that the social issue is closely related to capital and the social relations imposed by it, and that such a process is historical with respect to the development of Brazilian society. In view of the inequity in health as one of the faces of the social issue, health policies point to the ESF as one of the ways to face social inequality given that, in the social determinants we find the expression of the social issue, and with a practice based on the principles of territorialization, community diagnosis, intersectoriality and equity, such model can be promising and resolutive in the face of social demands which are related to health.

Abstract Objective: To know the social support received in the pregnancy and postpartum period in the perception of postpartum adolescents. Method: Qualitative research, carried out between May and August 2016 through semi-structured interviews and speaking map with 11 postpartum adolescents, and who had had their children at a teaching hospital in the interior of the state of Rio Grande do Sul, Brazil. Data were analyzed by means of thematic content analysis. Results: The social support received by the postpartum adolescents had the predominance of instrumental support, followed by information and emotional support, which were provided mainly by family members. They also reported lack of social support, which reflected in alienation and disappointment. Conclusion: The need for monitoring was identified so as to meet the demands of adolescents in their routine, which implies sensitive actions by the Family Health Strategy units, a model that seeks care comprehensiveness.

Estudo realizado com objetivo de descrever as concepções dos enfermeiros que atuam no Programa Saúde da Família - PSF do município de Cabedelo sobre o processo de trabalho de enfermagem, as características desse processo, e, identificar os aspectos do processo saúde-doença que estes enfermeiros abordam com maior e menor freqüência, no cotidiano de sua prática profissional. Teve referencial teórico nos modelos de assistência à saúde implantados no Brasil, na estratégia do PSF, e, no processo de Trabalho em saúde mental. O referencial metodológico teve inspiração no Materialismo Histórico e Dialético. O material empírico foi coletado por meio de entrevista e analisado através da técnica de análise do discurso. A análise permitiu identificar os saberes e as práticas que orientam o trabalho investigado, auxiliando os sujeitos da investigação a desenvolver ações de enfermagem capazes de subsidiar uma aproximação com a superação das contradições reveladas, para adensar a construção de uma práxis, requerida na atualidade, que responda às necessidades de atenção à saúde mental, no sentido da inclusão social da pessoa com doença mental. O PSF é uma estratégia para a efetivação do Sistema Único de Saúde - SUS e como tal, um instrumento que pode ser empregado no sentido da municipalização das ações de saúde mental, aproximando essa área específica com o SUS e permitindo o avanço do movimento pela Reforma Psiquiátrica na perspectiva de uma de suas diretrizes, a desinstitucionalização da doença e da pessoa com doença mental. The nurse’s praxis in the family health program in attention to mental health care Abstract Study carried out with the objective to describe the conceptions of the nurses who act in the “Family Health Program”PSF of the city of Cabedelo about the process of nursing work, the characteristics of this process, and, to identifythe aspects of the process health-illness that these nurses approach with greater and minor frequency in daily of the practical professional. It had theoretical referencial in the models of assistance to health introduced in Brazil, in the strategy of the PSFand in the process of work in Mental Health, the metodological referencial had inspiration the Materialism Historical and Dialectical. The empirical material was collected by means of interviews and analyzed through the technique of analysis of the speech. The analysis allowed to identify the knowledge and the practical ones that guide the investigated work, assisting the citizens of the inquiry to develop actions of nursing capable to subsidize and approach with the overcoming of the d i s c l o s e d direction of the social inclusion of the person with insanity. The PSF is a strategy for the accomplishment of the only System of Health -SUS and such as a tool that can be used in the direction of gathering the actions of mental health approaching this specific area with SUS and allowing the advance of the movement for the Psychiatric Reform in the perspective of one of its guidelines, the desinstitutionalization of the illness and the person with insanity.

PurposeThis study was conducted to investigate the Social Physique Anxiety (SPA) level in adolescent and young women with polycystic ovarian syndrome (PCOS).Design/methodology/approachThis is a descriptive study. It was conducted in Turkey. The study was conducted from January 1, 2017 to December 31, 2017. It was completed with a total of 150 young women – 75 of them had PCOS and 75 of them were healthy. Study data were collected using demographic information form and Social Physique Anxiety Scale (SPAS).FindingsThe study was found that the adolescent and young women with PCOS had higher subscales and total scores for total SPAS at a statistically significant level than healthy adolescent and young women (p < 0.05).Practice implicationThe findings of this study may be used for planning and implementing interventions in health-care practice such as screening and early treatment of SPA of women with PCOS.Originality/valueIt was found that PCOS in adolescent and young women increased the SPA level. In line with these results, health professionals must be particularly aware of the potential psychosocial and health needs of adolescent and young women with PCOS.
