This paper examines the developing and complex legal relationship between universities and students, or would-be students, who have mental health problems. Discussion takes account of the wider social and policy contexts, including the extent of mental ill-health among the student population, the market for higher education, and government policies towards universities. It contends that the legal position of students with mental health problems demonstrates that there is a need for the relationship between students and universities to be conceptualised with reference to the citizenship ideal rather than the consumer paradigm with which it has tended to become associated in public policy terms.

Abstract Objective: To assess the relationship between food insecurity, sleep quality, and days with mental and physical health issues among college students. Design: An online survey was administered. Food insecurity was assessed using the 10-item Adult Food Security Survey Module. Sleep was measured using the 19-item Pittsburgh Sleep Quality Index (PSQI). Mental health and physical health were measured using three items from the Healthy Days Core Module. Multivariate logistic regression was conducted to assess the relationship between food insecurity, sleep quality, and days with poor mental and physical health. Setting: Twenty-two higher education institutions. Participants: College students (n=17,686) enrolled at one of 22 participating universities. Results: Compared to food secure students, those classified as food insecure (43.4%) had higher PSQI scores indicating poorer sleep quality (p<.0001) and reported more days with poor mental (p<.0001) and physical (p<.0001) health as well as days when mental and physical health prevented them from completing daily activities (p<.0001). Food insecure students had higher adjusted odds of having poor sleep quality (adjusted odds ratio [AOR]:1.13, 95% confidence interval [CI]: 1.12-1.14), days with poor physical health (AOR: 1.01, 95% CI: 1.01-1.02), days with poor mental health (AOR: 1.03, 95% CI: 1.02-1.03), and days when poor mental or physical health prevented them from completing daily activities (AOR: 1.03, 95% CI: 1.02-1.04). Conclusions: College students report high food insecurity which is associated with poor mental and physical health and sleep quality. Multi-level policy changes and campus wellness programs are needed to prevent food insecurity and improve student health-related outcomes.

AbstractObjectiveThe present study aimed to determine the relationship among food insecurity, social support and mental well-being in sub-Saharan Africa, a region presenting the highest prevalence of severe food insecurity and a critical scarcity of mental health care.DesignFood insecurity was measured using the Food Insecurity Experience Scale (FIES). Social support was assessed using dichotomous indicators of perceived, foreign perceived, received, given, integrative and emotional support. The Negative and Positive Experience Indices (NEI and PEI) were used as indicators of mental well-being. Multilevel mixed-effect linear models were applied to examine the associations between mental well-being and food security status, social support and their interaction, respectively, accounting for random effects at country level and covariates.ParticipantsNationally representative adults surveyed through Gallup World Poll between 2014 and 2016 in thirty-nine sub-Saharan African countries (n 102 235).ResultsThe prevalence of severe food insecurity was 39 %. The prevalence of social support ranged from 30 to 72 % by type. In the pooled analysis using the adjusted model, food insecurity was dose-responsively associated with increased NEI and decreased PEI. Perceived, integrative and emotional support were associated with lower NEI and higher PEI. The differences in NEI and PEI between people with and without social support were the greatest among the most severely food insecure.ConclusionsBoth food insecurity and lack of social support constitute sources of vulnerability to poor mental well-being. Social support appears to modify the relationship between food security and mental well-being among those most affected by food insecurity in sub-Saharan Africa.

Abstract Purpose We aimed to explore the relationship between common mental disorders (CMDs), food insecurity and experiences of domestic violence among pregnant women attending public sector midwife obstetric units and basic antenatal care clinics in Cape Town during the COVID-19 lockdown. Methods Perinatal women, attending 14 healthcare facilities in Cape Town, were enrolled in the study during baseline data collection before the COVID-19 lockdown. During the lockdown period, fieldworkers telephonically contacted the perinatal women who were enrolled in the study and had provided contact details. The following data were collected from those who consented to the study: socio-demographic information, mental health assessment, food insecurity status and experiences of domestic violence. Poisson regression was used to model the associations of a number of risk factors with the occurrence of CMDs. Results Of the 2149 women enrolled in the ASSET study, 885 consented to telephonic interviews. We found that 12.5% of women had probable CMDs and 43% were severely food insecure. Psychological distress increased significantly during the lockdown period, compared to before the COVID-19 outbreak. Using multivariate Poisson regression modelling, we showed that the risk of CMDs was increased in women who were severely food insecure or who experienced psychological or sexual abuse. Conclusions This study provides evidence of the effect of the COVID-19 lockdown on the mental health status of perinatal women living in low-resource settings in Cape Town and highlights how a crisis such as the COVID-19 lockdown amplifies the psycho-social risk factors associated with CMDs in perinatal women.

Abstract Objectives: The prevalence of student food insecurity at Memorial University of Newfoundland (MUN) is relatively high (58.0%) compared to the national population (12.7%). We explored the relationship between food security status, perceived health, and student experience among MUN students. Methods: Through an online survey of returning MUN students at the St. John’s campus, we assessed food security using Statistics Canada’s Canadian Household Food Security Survey Module (HFSSM), and self-reported physical health, mental health, and stress. We used logistic regression to compare health and stress ratings between students of different food security levels. We thematically coded open-ended responses to describe students’ experiences related to food insecurity. Results: Among the 967 study eligible students, 39.9% were considered food insecure, 28.2% were moderately food insecure, and 11.7% were severely food insecure. After controlling for significant predictors, students who were moderately or severely food insecure were 1.72 [95% CI:(1.20,2.48)] and 2.81 [95% CI:(1.79,4.42)] times as likely to rate their physical health as ‘fair’ or ‘poor’ than food secure students, and 1.66 [95% CI:( 1.22,2.27)] and 4.23 [95% CI: (2.71-6.60] times as likely to rate their mental health as ‘fair’ or ‘poor’ than food secure students, respectively. Conclusion: Food security level experienced by MUN students was closely related to their perceived physical and mental health. As food security level worsened among participants, their self-reported physical and mental health also worsened. Health professionals working with university student populations should screen for food security and consider its relationship to students’ health.

Abstract Objectives The aim of the study was to evaluate whether maternal mental health and infant's diet mediate the association between Household Food Insecurity (HFI) and Early Child Development (ECD) using Structural Equation Modeling (SEM). We hypothesized a priori that both maternal mental health and infant's diet would mediate the association between HFI and ECD. Methods A cross-sectional study was carried out with 474 mother-infant dyads at Primary Health Centers in the Federal District, Brazil. Mothers answered a questionnaire that evaluated variables through validated instruments. SEM analyses were conducted including the following observed variables: HFI, ECD, consumption of a healthy and diverse diet, presence of a partner in the household, wanted pregnancy and current breastfeeding. Latent variables were included to evaluate mental health (created from maternal depression, trait anxiety and state anxiety), poverty (created from maternal educational level, household per capita income and participation in the Brazilian cash transfer program) and infant health (created from preterm birth, hospitalization after birth and Neonatal Intensive Care Unit needed after birth). Results Poor maternal mental health mediated the relationship between HFI and ECD (β = –0.05), but a healthy and diverse diet was not a mediator in this relationship. Poverty was related to lower ECD scores directly (β = 0.21) and indirectly (β = 0.02). Not having a partner (β = 0.05) and having an unwanted pregnancy (β = 0.02) predicted indirectly lower ECD scores. Conclusions Poor maternal mental health mediates the relationship between HFI and ECD, and ECD is influenced by poverty, single motherhood and unwanted pregnancy through different pathways. Therefore, it is necessary that public policies and interventions that aim to promote an optimal ECD also approach these aspects. Longitudinal cohort studies are needed to determine causality and identify promising approaches to improve parenting skills and ECD outcomes in the context of HFI. Abstract Background COVID-19 pandemic has been affecting population’s health and economies, in which food insecurity appears as important concern, particularly considering its associations with mental health and adverse behaviours. This study explored the relationship between food insecurity and mental health features and behaviours in adults from Portugal. Methods A cross-sectional study using data from an online survey was performed, including Portugal’s residents aged ≥18 years. Data on sociodemographics, behaviours, mental health features and food security status were collected. Food security status was evaluated with the United States Household Food Security Survey Module: Six-item Short Form. Chi-square test or Fisher's exact test, as appropriate, were used to explore the relationship between food security status and categorical variables. Results Food insecurity prevalence was 6.8%; showed to be significantly higher among less educated individuals (≤12 years of schooling) (p&lt;0.001), those who were employed and remained since the beginning of the pandemic (p&lt;0.001) and among those reporting need to be careful about expenses (p&lt;0.001). Comparing to individuals who were food secure, food insecure participants were more likely to smoke (p=0.009), to report depressive symptoms (p&lt;0.001) and anxiety (p&lt;0.001), measured by the Hospital Anxiety and Depression Scale. Conclusions From the present study, it can be concluded that the COVID-19 pandemic can affect food security in different subpopulations. Key messages The findings of this study have several implications for health policy and reinforce the need for investment in contexts of populations vulnerable to food insecurity
