Abstract The WHO African region is characterised by the largest infectious disease burden in the world. We conducted a retrospective descriptive analysis using records of all infectious disease outbreaks formally reported to the WHO in 2018 by Member States of the African region. We analysed the spatio-temporal distribution, the notification delay as well as the morbidity and mortality associated with these outbreaks. In 2018, 96 new disease outbreaks were reported across 36 of the 47 Member States. The most commonly reported disease outbreak was cholera which accounted for 20.8% (n = 20) of all events, followed by measles (n = 11, 11.5%) and Yellow fever (n = 7, 7.3%). About a quarter of the outbreaks (n = 23) were reported following signals detected through media monitoring conducted at the WHO regional office for Africa. The median delay between the disease onset and WHO notification was 16 days (range: 0–184). A total of 107 167 people were directly affected including 1221 deaths (mean case fatality ratio (CFR): 1.14% (95% confidence interval (CI) 1.07%–1.20%)). The highest CFR was observed for diseases targeted for eradication or elimination: 3.45% (95% CI 0.89%–10.45%). The African region remains prone to outbreaks of infectious diseases. It is therefore critical that Member States improve their capacities to rapidly detect, report and respond to public health events.

Jute is the golden fiber of Bangladesh, but its production is declining due to the involvement of higher production and processing costs, where a major portion of the cost is needed for fiber extraction. Labor unavailability and increasing labor cost have led to higher jute fiber production cost. To address these issues, this study looks at the development of a power-operated and cost-effective fiber extraction machine aiming at reducing the production cost. The study was conducted at the Rangpur regional office premises of Practical Action in Bangladesh, and the developed machine was branded as “Aashkol”, which had the following major parts: a feeding tray, a primary extraction roller, a secondary extraction roller, grabbing rollers, fiber collection stand, base frame, protection cover, and a spring-loaded tray under the primary extraction roller. The Aashkol can extract green ribbon from the jute stem, but jute sticks were broken down into smaller pieces (3–6 cm). The performance evaluation of the machine was conducted using different types of jute (Deshi, Kenaf, and Tossa) and compared with another jute extraction machine (KP model, introduced by Karupannya Rangpur Ltd.). The Aashkol-based extraction and improved retting systems were also evaluated and compared with traditional jute extraction systems. The jute stem input capacity (4.99 t h−1) of the Aashkol was 47.6% higher than the KP model (3.38 t h−1). Compared with the traditional system, across jute types, the Aashkol produced a 9% higher fiber yield and saved 46% retting time. Overall, the Aashkol reduced 90% of the labor requirement and saved 11.6 USD t−1 in jute fiber extraction and retting than the traditional method.

Ready-to-eat cereals (RTECs) have become a popular breakfast option claiming to provide important nutrients to children’s diets, despite being a source of excess sugar and, therefore, a health concern. Thus, food reformulation constitutes an important public health strategy that could benefit from inputs provided by nutrient profiling. This study aimed to assess the adequacy of the RTECs for children available in Portuguese supermarkets, applying three nutrient profile models (NPMs)—the nutrient profile model of the World Health Organization’s Regional Office for Europe (WHO-EURO), the profile of the private-sector EU Pledge (EU-Pledge), and the national model developed by the Directorate-General of Health (NPM-PT)—in order to explore the potential for reformulation of the RTECs identified as not adequate and evaluate the impact of RTECs’ reformulation on the nutritional quality of Portuguese children’s diets. In total, 78 RTECs intended for children were assessed and two scenarios—current (not considering reformulation) and alternative (considering reformulation to accomplish the nutrient profile requirements)—were considered to assess the impact of reformulation on nutritional quality. Across all RTECs, only 5.1% could be promoted to children according to the considered NPMs. The most common nutrients requiring reformulation were sugar, saturated fatty acids (SFA), salt, and dietary fiber. The scenarios of reformulation considered could reduce the RTECs average content of total sugars, SFA, and salt by 43%, 8.7%, and 1.1%, respectively, and dietary fiber intake could be increased by 34%. Thus, these results support policies to implement reformulation strategies for developing healthier food products to be promoted to children.

Early on in the COVID-19 pandemic, the WHO Eastern Mediterranean Regional Office recognised the importance of epidemiological modelling to forecast the progression of the COVID-19 pandemic to support decisions guiding the implementation of response measures. We established a modelling support team to facilitate the application of epidemiological modelling analyses in the Eastern Mediterranean Region (EMR) countries. Here, we present an innovative, stepwise approach to participatory modelling of the COVID-19 pandemic that engaged decision-makers and public health professionals from countries throughout all stages of the modelling process. Our approach consisted of first identifying the relevant policy questions, collecting country-specific data and interpreting model findings from a decision-maker’s perspective, as well as communicating model uncertainty. We used a simple modelling methodology that was adaptable to the shortage of epidemiological data, and the limited modelling capacity, in our region. We discuss the benefits of using models to produce rapid decision-making guidance for COVID-19 control in the WHO EMR, as well as challenges that we have experienced regarding conveying uncertainty associated with model results, synthesising and comparing results across multiple modelling approaches, and modelling fragile and conflict-affected states.

Abstract Background Grounded in the last decade's health literacy developments; the work of the European health literacy action networks on measurement and NCDs as well as the newly adopted European Health Literacy Roadmap and resolution agreed upon by their 53 Member States in 2019, the WHO European Region continues its investment in health literacy. A WHO European health literacy action plan is under development to be launched in 2021. The action plan implies concrete actions for Member States on how to develop health literate populations and societies. Methods Based on co-creation principles, the development of the action plan is conducted by stakeholders from a wide range of fields and disciplines. Lead by the WHO European Regional Office, the process includes an initial stakeholder meeting, iterative technical consultations with experts and the wider stakeholder community and Member State consultations. The analytical methods integrate a SWOT-analysis, future scenario thinking and long-sight action planning approaches as well as application of health literacy analytics. Results The results of the initial analytical steps will be presented such as the SWOT analysis, the future scenarios and associated recommendations on how to create health literate populations and societies in Europe. Additionally, the outline of the draft of the European health literacy action plan will be open for discussion and input. Conclusions Through an iterative process, the European health literacy action plan is co-produced by multiple actors through a series of consultations facilitating ownership and accountability. The European health literacy action plan can be an inspiration and a model for other world regions.
