Lactation is a continuation of intrauterine gestation. In both processes, maternal diet plays an active role in the provision of nutrients, maternal nutritional stores and endocrine adaptations serve to buffer the short-term variations in maternal nutritional intake, blood flow plays an overriding role in nutrient transfer to the fetus and newborn infant, and the nutrient demands of the recipient are the highest of any stage in human development. Human milk is remarkable in its variability. Recent data suggest that the variability often improves the nutrient composition as part of a complex adaptation to the infant's specific needs. A comprehensive survey of the literature on lactation and human milk is provided in two review articles.1,2 NUTRIENTS Lipids Milk lipids provide the major fraction of calories in human milk, yet they are the most variable constituent.3 Preceding a nursing, the fluid phase of milk stored within the gland resembles skimmed milk. During the course of a nursing, the contraction of smooth muscle launches the fat droplets. This draught reflex is essential for caloric adequacy for the breast-fed infant.2 Women living under unfavorable socioeconomic conditions have reduced total milk lipid.4-6 There is evidence that supplementing the diets of these women leads to increased milk fat. Under controlled metabolic ward conditions, a high-caloric, high-fat diet can be demonstrated to increase milk fat production.7 The distribution of the spectrum of fatty acids in human milk also is responsive to dietary changes.7-13 Women who are malnourished also produce an excess of 12:0 and 14:0 fatty acids.14

Background: Breast milk is the ideal food source that contains all components an infant needs. Infants are at risk of receiving possible pollutants and high levels of trans fatty acids through breast milk that may affect their cardiovascular health and cognitive development. Statistics indicate that the proportion of children who are breastfed up to one year of age shows an annual increase in Latvia. Objectives: 1. To review studies conducted among the Latvian population regarding breast milk composition. 2. To evaluate the necessary future studies emphasizing the importance of the trans fatty acid content in a mother’s diet and correspondingly in breast milk and taking into consideration the new changes in Latvian legislation setting the maximum trans fat content in foodstuffs. Conclusions: There is very little research done about the human milk composition in Latvia that could be referable to a general population. It is known that the exposure of breast milk to persistent organic pollutants in Latvia corresponds to the lowest levels detected in the European countries and is not a cause of health disorders. Taking into consideration the adverse effect trans fatty acids have on human’s health regardless of the age, consumption should be as low as possible. Latvia has become the latest European Union country to set the maximum amount of trans fats in foodstuffs, yet there is no information about trans fatty acids content in human milk among women in Latvia. This problem needs to be addressed and set in motion. Therefore, it would be a novel idea to study if the TFA content also declines in milk among Latvian women after the introduction of the new legislation.

The human milk fatty acid, including trans fatty acid, composition is predominantly affected by the maternal diet. The aim of this research was to determine the trans fatty acid level in human milk among lactating women in Latvia, and to evaluate how maternal dietary habits affect the trans fatty acid composition of human milk. In total, 70 lactating women participated in this cross-sectional study. A 72-hour food diary and food frequency questionnaire were used to evaluate maternal dietary habits. Different trans fatty acids in human milk samples were determined using gas chromatography (Agilent 6890N, Agilent Technologies Incorporated, the United States). Overall, the dietary intake of trans fatty acids among the participants was 0.54 ± 0.79 g per day. The total trans fatty acid level in the human milk samples was 2.30% ± 0.60%. The composition of trans fatty acids found in human milk was associated with maternal dietary habits. Higher elaidic acid, vaccenic acid and total trans fatty acid levels in human milk were found among participants with a higher milk and dairy product intake. Meat and meat product intake were associated with a higher vaccenic acid and total trans fatty acid levels in human milk. A moderate association was also established between maternal trans fatty acid intake and the total trans fatty acid level in human milk. The obtained correlations indicate that maternal dietary habits during lactation can impact the composition of trans fatty acids found in human milk.
