Mass-forming chronic invasive fungal sinusitis; an uncommon mimicker of head & neck malignancy

Abstract Introduction/Objective Invasive fungal sinusitis is a progressive infection affecting the nasal cavity and associated sinus spaces. Acute forms affect immunocompromised patients with debilitating systemic diseases while chronic forms usually occur in immunocompetent individuals. Methods/Case Report A 26-year-old immunocompetent male with daily Marijuana smoking consulted for acute swelling of the right eye with dull pain, photophobia, loss of vision and pressure of 4 day duration. He reported nasal congestion for approximately one year and over the previous 2 months developed exophthalmos. Imaging demonstrated a 4.9 x 4.3 x 4.1 cm invasive soft tissue mass in the right lamina papyracea with involvement of the paranasal sinus, right orbit, and intracranial space. The mass was presumed to represent a high-grade malignancy. Biopsy was performed and demonstrated granulomatous invasive fungal sinusitis. The patient underwent right orbitotomy and right extranasal ethmoidectomy. He was begun on empiric amphotericin B. After culture and sequencing identification of Curvularia species, voriconazole was added. Follow up imaging studies after two month of treatment still demonstrated residual mass forming lesion with 10-20% decrease in size. The patient has regained his vision after 4 month of treatment and he is expected to complete a six months course. If the mass forming lesion persists, a second round of surgical debridement will be performed. Results (if a Case Study enter NA) N/A Conclusion Curvularia is a dematiaceous fungus most commonly associated with allergic sinusitis, but can rarely become invasive in immunocompetent individuals. Histopathologically large fungal elements are easily recognizable but specific identification requires culture or ribosomal DNA sequencing by PCR. Mass-forming chronic fungal sinusitis overlaps clinically and radiologically with head and neck malignancies. Our report brings attention to this uncommon clinical variant of invasive fungal sinusitis that affects immunocompetent individuals.

AbstractObjective:To demonstrate the use of an optical surface scanner, with associated software, in the assessment of rhinoplasty patients, and to discuss the possible clinical applications of this technology in the future.Design:Case study analysis of pre- and post-operative scans of a patient undergoing septorhinoplasty at Whipps Cross University Hospital, London, UK.Subject:A 21-year-old man undergoing septorhinoplasty underwent pre-operative optical surface scanning of his face. The scans were repeated at one week and one year post-operatively. Software developed at University College London was then used to analyse the scans.Results:The scans clearly showed that the man's dorsal hump had been well reduced and the nose straightened, with a resulting 1600 mm3 gain on the right side and a 1000 mm3 loss on the left side of the nose. Tip projection had also been achieved.Conclusion:This technique allowed objective quantification of facial features and analysis of change. It may well prove useful in the future in predicting change following surgical intervention.

Cystoid Macular Oedema or CME is a painless disorder that affects the central retina or macula. It refers to the accumulation of fluid in the outer plexiform and inner nuclear layer of the retina with the formation of a fluid-filled cyst. The primary symptom of macular oedema is blurry or wavy vision near or in the centre of your field of vision. Materials and Methods: A male patient of 48 yrs. presented in Shalakya OPD of GAMC Bengaluru with symp- toms of diminished vision in his right eye for one year. The patient was diagnosed with cystoid macular oedema in the right eye for which he was given photocoagulation therapy but did not find much relief. So, he approached our OPD. After thorough examination patient was started with Ayurvedic Medicines. Result: The subject showed marked improvement both subjectively and objectively. Discussion: Oedema which is the terminus of the pathology in this condition has to be understood as Ekanga Shopha. Though Kapha is the predominant dosha involved in forming Shopha here, the lakshanas manifesting are that of Vataja Timira. So, in this case study, Kapha Vata Hara followed by Shopha Hara line of treatment is adopted. Keywords: Cystoid macular oedema, Shopha, Vataja Timira, Nasya, Punarnavadi Kashaya.

There are several factors that can cause problems in the land sector in Indonesia, one of which is inheritance. This study aims to determine: 1) the legal position of the heirs of foreign citizens in the object of inheritance in the form of land rights obtained from inheritance with Indonesian citizenship; and 2) The validity of the control of land rights by foreign countries on objects originating from the inheritance of Indonesian citizens. The research method uses a normative juridical approach to the law (statute approach), conceptual approach (conceptual approach), and case study approach (case study). The results of the study are heirs who have changed their status to foreign citizens can also be proven by their lineage or have blood relations so that if they acquire property in the form of land originating from inheritance while those who control the property are foreign nationals, if possible, do so to discuss the object. through an exchange, grant, or auction within a maximum period of 1 (one) year, the status will automatically switch. If the land has been turned into state land, then the owner is considered to have relinquished his rights but the owner is still given the opportunity by law to be able to apply for the Right to Use the land.Keywords: Inheritance Law; Legality Of Ownership Of Land Rights; Foreign Nationalilty.Terdapat beberapa faktor yang dapat menimbulkan permasalahan di bidang pertanahan di Indonesia, salah satunya adalah pewarisan. Penelitian ini bertujuan untuk mengetahui: 1) Kedudukan hukum ahli waris warga negara asing dalam pewarisan obyek waris berupa hak atas tanah yang diperoleh dari pewarisan berkewarganegaraan Indonesia; dan 2) Keabsahan penguasaan Hak Atas Tanah yang dilakukan oleh warga negara asing atas obyek yang berasal dari Pewarisan berkewarganegaraan Indonesia. Metode penelitian menggunakan yuridis normatif dengan pendekatan perundang-undangan (statute approach), pendekatan konseptual (conceptual approach), dan pendekatan studi kasus (case study). Hasil penelitian yaitu ahli waris yang telah berubah status kewarganegaraan menjadi warga negara asing juga dapat menjadi ahli waris yang dibuktikan dengan adanya garis keturunan atau memiliki hubungan darah sehingga jika memperoleh harta berupa tanah yang berasal dari pewarisan sedangkan yang menguasai harta tersebut sudah menjadi warga negara asing maka sebaiknya dilakukan peralihan terhadap obyek tersebut melalui jual beli, tukar menukar, hibah, atau lelang dalam jangka waktu maksimal 1 (satu) tahun karena jika lebih dari jangka waktu tersebut maka status tanah akan beralih secara hukum menjadi tanah negara. Jika tanah tersebut telah beralih menjadi tanah negara maka si pemilik dianggap telah melepaskan haknya tetapi si pemilik masih diberi kesempatan oleh undang-undang untuk dapat mengajukan permohonan Hak Pakai atas tanah.Kata Kunci: Hukum Waris; Keabsahan Kepemilikan Hak Atas Tanah; Warga Negara Asing.

Abstract BackgroundPrimitive myxoid mesenchymal tumor of infancy (PMMTI) is a rare tumor and its molecular mechanism and prognosis remain unclear. We here report a case of PMMTI, and review the reported literature of PMMTI.Case presentationA one-year-old boy found a lump in his right groin. The size of the mass was 1 cm×1 cm. Computed tomography showed a round, slightly dense shadow of 38 mm×32 mm in the right groin. The tumor was removed for pathological examination, and the size of the tumor was 6.0 cm×5.0 cm×3.0 cm. Tumor cells are diffusely distributed, mainly spindle cells. The blood vessels were abundant, many adipocytes can be observed in some areas, and focal lymphocyte infiltration can be seen in a few areas. Immunophenotype was as follows: histone H3 at lysine 27 (H3K27me3), B-cell lymphoma 6 corepressor (BCOR) and Bcl-2 were positive, cluster of differentiation 99 (CD99) showed diffuse reactivity, BCL6 was scattered positive, Ki-67 proliferation index was 20%. Dual-color fluorescence in situ hybridization (FISH) showed red and green signal nuclei caused by the deletion of ETV6 gene rearrangement. The patient underwent extensive surgical resection during the recurrence period and was in good condition 7 months later.ConclusionsPMMTI is at least a low-grade malignancy. BCOR, BCL2, BCL6, and CD99 can be used as special IHC markers, and most PMMTI cases exhibit BCOR ITD change. H3K27me3 may be used as a new IHC marker for PMMTI.

The introduction of Computational Thinking (CT) in Italian compulsory schools is on the way, and there is a general need for new methodologies to support teachers’ work. A one-year long learning path supporting the development of CT skills in primary school students through game making activities was defined and tested in a case study with one grade 5 class. All students in the class were actively involved regardless of their personal interest, their participation was generally high, their skills increased along the project, and the main objectives were reached. Nevertheless, results suggest that a longer time span is needed for students to master deeply the new concepts and tools, class organization proved to be crucial and this confirms the need for an adequate teachers’ training before introducing CT in classroom activities. Students demonstrated to be on the right path to approach CT through game making, but they still need to be guided in taking the player’s perspective and making their games easy to be understood and played. At the end of the case study, the learning path was revised and is now in use in a three-year long project in a primary school.
