We present an approach to simultaneously reasoning about a video clip and an entire natural-language sentence. The compositional nature of language is exploited to construct models which represent the meanings of entire sentences composed out of the meanings of the words in those sentences mediated by a grammar that encodes the predicate-argument relations. We demonstrate that these models faithfully represent the meanings of sentences and are sensitive to how the roles played by participants (nouns), their characteristics (adjectives), the actions performed (verbs), the manner of such actions (adverbs), and changing spatial relations between participants (prepositions) affect the meaning of a sentence and how it is grounded in video. We exploit this methodology in three ways. In the first, a video clip along with a sentence are taken as input and the participants in the event described by the sentence are highlighted, even when the clip depicts multiple similar simultaneous events. In the second, a video clip is taken as input without a sentence and a sentence is generated that describes an event in that clip. In the third, a corpus of video clips is paired with sentences which describe some of the events in those clips and the meanings of the words in those sentences are learned. We learn these meanings without needing to specify which attribute of the video clips each word in a given sentence refers to. The learned meaning representations are shown to be intelligible to humans.

Following the first two editions of her book, which she wrote solo, Celli has joined forces with colleague N D Young for the third edition. Celli and Young have prepared this book with the goal of giving instructors in postsecondary education “a primer on learning styles” (p.11). In a review of the second edition, Kovac (1999) highlighted the importance for educators of understanding the learning styles of their students to better present course content. Fifteen years later this statement remains true. With Kovac’s own teaching background in chemistry, he noted that it was challenging to address the differing learning preferences of all students in his classes. He pointed to the practical nature of Celli’s book in addressing these needs and offering pedagogic solutions. This third edition appears to have altered little, with an entry-level overview of learning-style theory, followed by an explanation of Celli and Young’s own learning-style theory. Celli and Young set out to address the struggle of every educator who seeks to improve pedagogic practice in the classroom through careful thought, planning and innovative implementation.

Forty-two children of normal intelligence with congenital heart disease between the ages of 4 and 8 years were tested for general intelligence, perceptual motor function, and gross and fine motor coordination. Three groups of children were tested: (1) those with cyanotic heart disease, (2) those with noncyanotic heart disease and a history of congestive heart failures, and (3) those with noncyanotic heart disease and a benign medical history. The first group had lower I.Q. scores, did less well in perceptual motor tasks, and had poorer gross motor coordination than children in the third group. Children of the second group occupied an intermediate position.

Abstract Patients with known coagulation deficiencies, either congenital or acquired, may bleed spontaneously with trauma or with surgical intervention. In contrast, however, are the unchallenged patients who bleed in a variety of clinical settings that demand rapid diagnosis so that appropriate therapy can be instituted. In the first section Dr. Louis M. Aledort demonstrates a series of vignettes of actual cases who presented with unexpected bleeding or a screening laboratory abnormality prior to a needed surgical intervention. Settings include dental, oral surgical, obstetrical, surgical and gynecological. The differential diagnoses of these cases are discussed. In the second section Dr. David Green also uses vignettes to demonstrate how the laboratory is used to differentiate the various clinical entities. The choice and priority of required tests indicated by the settings, history, site and type of bleeding, and the syllogisms used to define the abnormality are stressed. In the third section, Dr. Jerome Teitel reviews in detail the therapeutic armamentarium available to the clinician and presents algorithms for the management of these bleeding disorders.

Oral and maxillofacial surgery (OMFS) is a specialty widening in its scope. An objective analysis of the referral pattern can provide essential information to improve healthcare. This four-year retrospective study was implemented in Khartoum Teaching Dental Hospital. Data (age, sex, diagnosis, and type of treatment) were collected from patient records. Disease frequency, as well as the effect of sex and age, were analyzed for each group. The frequency of treatment types was also assessed. Data were collected from a total of 3,478 patients over the four-year study period. There was a male predominance with the third decade of life being the most common age group. Pathological diseases were the most common (37%) reason for referral, followed by trauma (31%). Temporomandibular joint (TMJ) disorders and dentoalveolar extraction were the least frequently observed. Open reduction and internal fixation (ORIF) was the most commonly performed procedure (28%). These data represent the epidemiology of oral and maxillofacial diseases in Sudan. Given that the third decade of life is the most represented age group, it is beneficial to learn the long-term consequences of these diseases in these young patients and to use modern surgical techniques to improve their lives.

La anomalía de Ebstein es una cardiopatía congénita de frecuencia inusual, por lo que la presentación de este caso es de interés médico investigativo. Esta patología consiste en una implantación baja de la válvula tricúspide. Se describe el caso de recién nacido de 27 horas de vida, que debutó clínicamente con cianosis  generalizada,  soplo  holosistólico  más  frémito  y  acidosis  metabólica descompensada. Se confirmó el  diagnóstico  mediante  ecocardiograma,  se  derivó  al  tercer  nivel de atención y a pesar del esfuerzo de mantener hemodinámicamente estable al paciente, fallece a las 72 horas de vida. Se concluye destacando la importancia del control prenatal a nivel ecográfico y postnatal mediante la identificación de hallazgos clínicos exhaustivos, con especial atención en la  oximetría  de  pulso,  herramienta  útil,  sencilla  y  económica  de  gran  importancia  para  detección precoz de malformaciones congénitas cardiovasculares, que requieran tratamiento especializado en un nivel de atención acorde a las expresiones clínicas de la enfermedad, para incrementar la supervivencia del neonato, mejorar la calidad de atención y disminuir los índices de mortalidad. ABSTRACT: Ebstein’s anomaly is a congenital heart disease of unusual frequency, so the presentation of this case is of investigative medical interest. This pathology consists of a low implantation of the tricuspid valve. We describe the case of a newborn of 27 hours lifetime that debuted clinically with general- ized  cyanosis,  holosystolic  murmur  more  frightened  and  decompensated  metabolic  acidosis,  the diagnosis  is  confirmed  by  echocardiogram,  it  is  derived  to  the  third  level  of  care  and  despite  the effort to maintain the patient hemodynamically stable, he died in 72 hours’ time. We conclude by highlighting the importance of prenatal control at the ultrasound and postnatal level by identifying comprehensive clinical findings with special attention to pulse oximetry, a useful, simple and eco- nomical tool of great importance for the early detection of congenital cardiovascular malformations requiring  specialized  treatment  in  a  level  of  attention  according  to  the  clinical  expressions  of  the disease, to increase the survival of the neonate, to improve the quality of attention and to decrease the mortality rates. Objective: construction and validation in appearance and content of the competence frameworks and of the Entrustable Professional Activities to develop skills in the training of nurses to assist the airway of adult patients in urgency and emergency situations. Method: a descriptive and methodological study developed in four phases: in the first, a workshop was held, composed of experts, for the construction of the competence frameworks; in the second, the material was validated using the Snowball Technique and the Delphi Technique, in the third, content analysis and calculation of the Content Validation Index were conducted; and in the fourth phase, the Entrustable Professional Activities were built, validated in simulated workshops. Results: the competence frameworks were built and validated, with a resulting CVI≥0.85 in all the items. The Entrustable Professional Activities were validated by experts regarding their applicability; of these, 44% stated they were applicable in simulated environments, 100% that they were useful content and with appropriate language, 22% suggested the insertion of new items to assess competence, 11% reported the difficulty of assessing competence individually in the clinical settings, and 11% of the experts referred to the need for prior training of the teacher/facilitator to use it. Conclusion: the study resulted in the construction of competence frameworks and six Entrustable Professional Activities relating them to the domains of essential competences in the training of nurses to assist the airway of adult patients in urgency and emergency situations. The participation of experts in the construction and validation of this material was essential to guarantee the theoretical and practical relevance of the result.
