Multimodal Biomarkers to Discriminate Cognitive State

Multimodal biomarkers based on behavioral, neurophysiological, and cognitive measurements have recently increased in popularity for the detection of cognitive stress and neurologically based disorders. Such conditions significantly and adversely affect human performance and quality of life in a large fraction of the world’s population. Example modalities used in detection of these conditions include speech, facial expression, physiology, eye tracking, gait, and electroencephalography (EEG). Toward the goal of finding simple, noninvasive means to detect, predict, and monitor cognitive stress and neurological conditions, MIT Lincoln Laboratory is developing biomarkers that satisfy three criteria. First, we seek biomarkers that reflect core components of cognitive status, such as work­ing memory capacity, processing speed, attention, and arousal. Second, and as importantly, we seek biomarkers that reflect timing and coordination relations both within components of each modality and across different modalities. This is based on the hypothesis that neural coordination across different parts of the brain is essential in cognition. An example of timing and coordination within a modality is the set of finely timed and synchronized physiological components of speech production, whereas an example of coordination across modalities is the timing and synchrony that occur between speech and facial expression during speaking. Third, we seek multimodal biomarkers that contribute in a complementary fashion under various channel and background conditions. In this chapter, as an illustration of the biomarker approach, we focus on cognitive stress and the particular case of detecting different cognitive load levels. We also briefly show how similar feature-extraction principles can be applied to a neurological condition through the example of major depressive disorder (MDD). MDD is one of several neuropsychiatric disorders where multimodal biomarkers based on principles of timing and coordination are important for detection (Cummins et al., 2015; Helfer et al., 2014; Quatieri & Malyska, 2012; Trevino, Quatieri, & Malyska, 2011; Williamson, Quatieri, Helfer, Ciccarelli, & Mehta, 2014; Williamson et al., 2013, 2015; Yu, Quatieri, Williamson, & Mundt, 2014).

Abstract. Two clients seeking neuropsychological assessment reported anxiety about their cognitive status. We review the cases to increase our understanding of factors contributing to dementia-related anxiety. Case 1 met the criteria for mild neurocognitive disorder; the client’s memory was impaired, and she had a high genetic risk for Alzheimer’s disease. The client reported anxiety about negative perceptions of quality of life among individuals diagnosed with Alzheimer’s disease. Case 2 did not meet the criteria for a neurocognitive disorder. Anxiety about this client’s cognitive status appeared attributable to generalized anxiety disorder, given his anxiety about diverse topics. Both clients reported embarrassment about forgetfulness and social withdrawal. Dementia-related anxiety is believed to be relatively common, to exist on a continuum, to have unique social implications, and to stem from various sources, necessitating differing interventions.

This paper is a summary report on the basic questions of the biopsychosocial approach to Parkinson’s disease. It deals with cognitive, affective and psychological health issues which significantly influence the outcome of the physical rehabilitation. In spite of the unchanged cognitive status, the psychological burden of the changes in the quality of life, the obstruction, the change in the affective tone, and the shrinking ability to fulfil social roles decrease the patient’s quality of life. An interdisciplinary approach is best suited for mitigating these effects. Not only the patient but also his/her family and environment is seriously affected by the disease and its consequences. Treatment and rehabilitation options for increasing or maintaining the quality of life of the affected patients are diverse, and significantly depend on the features of the health care system. The authors believe that the following review emphasizing health psychological principles may contribute to the work of professionals working in clinical and rehabilitational fields and through them may increase the quality of life of patients and their family. Accurate and complete prescriptions of insulin are crucial to prevent medication errors from occurring. Two core components for safe insulin prescriptions are the word ‘units’ being written in full for the dose, and clear documentation of the insulin device alongside the name. A retrospective review of annual audit data was conducted for insulin prescriptions to assess the impact of changes to the prescribing system within a secondary care setting, at five time points over a period of 7 years (2014 to 2020). The review points were based on when changes were made, from standardized paper charts with a dedicated section for insulin prescribing, to a standalone hospital wide electronic prescribing and medicines administration (ePMA) system, and finally an integrated electronic health record system (EHRS). The measured outcomes were compliance with recommended standards for documentation of ‘units’ in full, and inclusion of the insulin device as part of the prescription. Overall, an improvement was seen in both outcomes of interest. Device documentation improved incrementally with each system change—34% for paper charts, 23%–56% for standalone ePMA, and 100% for ePMA integrated within EHRS. Findings highlight that differences in ePMA systems may have varying impact on safe prescribing practices.

Purpose This study evaluated the effects of an intergenerational Montessori after-school program on the engagement, affect, and quality of life of older adults with memory concerns and on the attitudes of children toward older adults. Method Eleven older adults were paired with 11 children to participate in a 45-min after-school activity program. Observations of engagement and affect during the interactions were collected 3 times a week for 4 weeks. The older adults' engagement and affect also were observed during 45-min planning/discussion sessions without the children present before their arrival to the program. Results Results revealed significant differences in older adults' engagement and positive affect when the children were present. Significant pre–post improvements in reported quality of life and maintenance of cognitive status were associated with program participation. Children demonstrated more active than passive engagement and more happy than neutral affect during activity sessions. Four of the seven children improved their positive ratings of older adults. Conclusions This program documented success in improving engagement and affect in older adults with mild memory concerns while engaging with children. Future studies with a larger sample of participants with varying degrees of memory impairment are needed to investigate the potential of this promising program.

Observation is a common occurrence within the workplace, and can often manifest as either peer-to-peer monitoring or supervisor-to-peer monitoring. To date, there is a limited body of research that describes changes in performance due to either a positive or negative relationship between supervisors and employees. The present study reports qualitative data on supervisor-to-employee relationships and how the quality of the relationship can alter human performance. The results indicated that relationship with a supervisor was related to perceived performance under direct observation. Women were more likely to report a positive relationship with their supervisor, yet also indicated a negative emotion toward being monitored. These results are important in understanding how supervisor presence influences individual performance when completing job-specific tasks. The implications for future research are discussed.

Abstract Background The ergonomic applied to Dental Medicine aims to develop an ergonomically planned environment, with equipment rationally distributed, to reduce physical and cognitive stress, prevent diseases related to professional exercise, increasing productivity, improving the quality of service provided and reducing fatigue in the working team. However, physical work usually imposes muscle tension on the musculoskeletal system that can culminate in work-related trauma, commonly called Musculoskeletal Disorder. Methods We proposed as an objective, to evaluate how the ergonomic risks in Medical-Dental Clinics, which can cause work-related Musculoskeletal Injuries, influence the quality of life and health of the workers. The checklist was applied based on the legislation in force and, at the same time, a questionnaire was applied for consultation with the workers; identification of the tasks of all workers and their associated risks has been carried out; ergonomic studies of the workstations concerned have been carried out and, in the light of the results obtained, preventive measures have been indicated to address the problems. Results Through the Rula Method, we evaluated the level of performance in relation to the tasks performed and found that most of these corresponded to level 3, which corresponded to values between 5 and 6 and indicated that changes should occur briefly. Conclusion It was concluded that the most outstanding area in need of rapid changes was the surgical area, since it requires more effort on the part of these professionals. It is therefore proposed to implement new measures to improve health and well-being and avoid ergonomically incorrect postures.

Learning motor skills follows a predictable sequence and stages. The main purpose of this study is to examine Gallahue’s motor learning model and compare it with existing featured models. Until now, several different models have been proposed by several theorists to identify and describe stages of motor learning. Fitts, P.M., and Posner, M.I. (1967. Human performance. Belmont: Brooks/Cole Pub. Co.) proposed a three-stage model for motor skill learning based on the learner’s cognitive state during the learning continuum. Gentile (1972–1978) proposed a two-stage model based on the goals of the learner. Gallahue (1972–2012) presented a three-level model with several accompanying sub-stages. In the present study, these models were compared in order to identify the best model to assist learners and practitioners. Analytical examinations showed that Gallahue’s motor learning model incorporates elements from both Fitts and Posner and Gentile models, but also provides specific guidelines and actions for instructors and practitioners along the learning continuum. Therefore, it seems to be more comprehensive and coherent (logical connection or relevant) in terms of functionality than existing models and provides specific cues for maximizing learning and meet the learner’s needs at each stage of learning.

It has been established in literature that “good governance” has major implications for poverty reduction, equity, empowerment, and quality of life. Information and Communication Technology (ICT) is seen as potentially very influential for the cause of good governance. E-governance is seen as means to achieve tenets of “good governance”. E-governance addresses core components of good governance by seeking to improve efficiency and effectiveness of government, relationships with communities, businesses, citizens, and NGO/civil societies for better provision of services, accountability, transparency, and social development. In the beginning sections of the chapter, ideologies behind good governance are discussed because e-government initiatives are presumably embedded in the “good governance” thinking in development. The chapter also focuses on the relevance of e-governance as a means to achieve “good governance.” In rural areas e-governance services are mostly provided through telecentres; hence, the chapter also discusses the role and issues related to telecentres for e-governance service delivery.
