Background Consistent with the increasing prevalence of obesity in the United States and many countries worldwide, anesthesiologists are now presented with a greater number of adult and pediatric patients who are significantly overweight. This prospective study was designed to examine the relation between age-adjusted body mass index, preoperative comorbidities, and perioperative outcome in children. Methods Children aged 2-18 yr undergoing noncardiac elective procedures were classified as overweight or obese based on their age- and sex-adjusted body mass index. Information was elicited regarding patient demographics, presence of comorbidities, and anesthetic technique. Data regarding the incidence and severity of perioperative adverse events were collected prospectively. Results Two thousand twenty-five children comprised the sample (1,380 normal weight, 351 overweight, and 294 obese). Obese children had a significantly higher prevalence of comorbidities than nonobese children, including asthma, hypertension, sleep apnea, and type II diabetes. Furthermore, obese children had a higher incidence of difficult mask ventilation, airway obstruction, major oxygen desaturation (>10% of baseline), and overall critical respiratory adverse events. Logistic regression analysis revealed several risk factors for adverse events, including procedures involving the airway, obesity, age younger than 10 yr, and a history of obstructive sleep apnea. Conclusions These results suggest that children presenting for elective surgical procedures who are obese have a greater prevalence of preexisting comorbid medical conditions and an increased incidence of perioperative adverse respiratory events compared with normal-weight children. Identification and awareness of risk factors for perioperative complications will be important in optimizing the anesthetic management of these children.

The purpose of this study was to investigate gross and fine motor skill in overweight and obese children compared with normal-weight peers. According to international cut-off points for Body Mass Index (BMI) from Cole et al. (2000), all 117 participants (5–10 year) were classified as being normal-weight, overweight, or obese. Level of motor skill was assessed using the Movement Assessment Battery for Children (MABC). Scores for balance (p < .01) and ball skills (p < .05) were significantly better in normal-weight and overweight children as compared with their obese counterparts. A similar trend was found for manual dexterity (p < .10). This study demonstrates that general motor skill level is lower in obese children than in normal-weight and overweight peers.

According to the data published by Haslam and James, about 10% of the world populations aged up to 18 areoverweight or obese [1]. In Europe, there are about 20% children with excessive body mass, 5% of whom sufferfrom obesity [2,3]. Childhood obesity is an ongoing epidemic in the United States [4,5]. The most recent data fromthe US indicate that 16.9% of children and adolescents are obese, defined as a body mass index (BMI) for age >95thpercentile [6,7] and there is evidence that the prevalence of obesity among children will reach 30% by 2030 [8].Childhood obesity is a risk factor for greater morbidity later in life, including diabetes, coronary artery disease andincreased mortality [4,5,9,10].

- Body dissatisfaction is a central aspect for self-evaluation; pressures to maintain an ideal physique can result in disordered eating habits. In young women, body concern and anxiety related to some parts of it can be expressed through their desire to lose weight. The aim of the study is to analyze the role of some risk factors, such as Body Mass Index (BMI), low self-esteem and sociocultural influences in affecting body dissatisfaction, expressed in terms of distance from an ideal and body concern. Participants are 187 adolescent females aged between 14 and 16. Risk factors taken into consideration seem to have a different effect on the two aspects characterizing dissatisfaction. BMI, self-esteem and internalization of a thin ideal seem to influence the distance perceived from an ideal body; pressure, besides BMI and self-esteem, affect weight and body shape concern. Body shape concern seems to be influenced even by internalization and awareness. ABSTRACT Background: Body mass index (BMI) is frequently used to evaluate risk of disease, but can be misleading because it does not account for different types of tissue mass (e.g., bone, muscle, fat). The purpose of this study was to classify adults in the United States according to cardiovascular fitness (CVF), BMI, and body fat using the National Health and Nutrition Examination Survey (NHANES) data. Methods: The three most current NHANES datasets (6,648 records) were included. Counts, means, and 95% confidence intervals (CI) determined the distribution of CVF across percent of body fat and BMI categories. Results: According to BMI, approximately 42.3% of participants were classified as either underweight or normal weight, and 24.9% were classified as obese. According to percent of body fat, 13.5% of subjects were classified as lean, while 68.4% of subjects were in the high percent body fat group. In regard to BMI, 9.9% and 6.7% of the overweight and obese populations, respectively, were classified in the highest third of CVF. According to adiposity, 6.6% and 21.0% of the moderate and high percent body fat population fell into the same category, respectively. Conclusion: Two-thirds of the population ranked below the 35th percentile for body fat (high percent body fat), with more of these individuals in the low CVF category than any other. The largest categorization for BMI was the normal-weight category. This supports that BMI may be misleading, and that utilizing percent body fat and CVF may provide a better indication of health.

Background and objectives: Role of salivary zinc to dental caries and body weight has not been studied extensively in children. The aim of the present study was to evaluate the relationship between salivary zinc and caries in overweight/obese Indian children. Study design: One hundred and sixty children aged 8–12 years of both genders were divided into two groups of eighty each based on their body mass index into normal weight and overweight/obese. Each child was assessed for their caries experience in primary and permanent dentition. Unstimulated salivary samples were collected from each child to estimate zinc levels by atomic absorption spectrophotometer. Data was subjected to statistical analysis. Results: The mean caries score among the overweight/obese children was 2.2±3.9 and 0.7±1.5 in the primary and permanent dentition respectively compared to the 2.0±2.6 and 0.2±0.5 respectively among normal weight children. Mean salivary zinc levels in overweight/obese children were 0.36±0.27 ppm compared to normal weight children of 0.81±0.46 ppm. Conclusion: Overweight/obese children demonstrated significantly lower salivary zinc levels and higher caries experience in permanent dentition. Salivary zinc levels showed a positive but weak association to caries in permanent dentition in both groups. Salivary zinc levels showed a negative non-significant correlation to caries in primary dentition among the children with higher BMI.

BACKGROUND: Obstructive sleep apnea (OSA) is a common diagnosis in clinical practice. Excessive daytime sleepiness may be a warning for possible OSA.OBJECTIVES: To assess the prevalence of excessive daytime sleepiness as measured by the Epworth Sleepiness Scale (ESS) in a rural community population; potential risk factors for OSA were also assessed.METHODS: In 2010, a baseline respiratory health questionnaire within the Saskatchewan Rural Health Study was mailed to 11,982 households in Saskatchewan. A total of 7597 adults within the 4624 (42%) respondent households completed the ESS questionnaire. Participants were categorized according to normal or high (>10) ESS scores. Data obtained included respiratory symptoms, doctor-diagnosed sleep apnea, snoring, hypertension, smoking and demographics. Body mass index was calculated. Multivariable logistic regression analysis examined associations between high ESS scores and possible risk factors. Generalized estimating equations accounted for the two-tiered sampling procedure of the study design.RESULTS: The mean age of respondents was 55.0 years and 49.2% were male. The prevalence of ESS>10 and ‘doctor diagnosed’ OSA were 15.9% and 6.0%, respectively. Approximately 23% of respondents reported loud snoring and 30% had a body mass index >30 kg/m2. Of those with ‘doctor-diagnosed’ OSA, 37.7% reported ESS>10 (P<0.0001) and 47.7% reported loud snoring (P<0.0001). Risk of having an ESS>10 score increased with age, male sex, obesity, lower socioeconomic status, marriage, loud snoring and doctor-diagnosed sinus trouble.CONCLUSIONS: High levels of excessive daytime sleepiness in this particular rural population are common and men >55 years of age are at highest risk. Examination of reasons for residual sleepiness and snoring in persons with and without sleep apnea is warranted.

Abstract Introduction Studies increasingly find a prospective association between adolescent weight status and adverse physical health outcomes. However, less is known about the long-term consequences of adolescent body weight on the adoption of health-risk behaviors. This study sought to determine whether adolescent body mass index (BMI) was associated with cigarette smoking in adulthood. Methods Six thousand six hundred eighty-three nonsmoking adolescents were interviewed at baseline (1996, age 11–20) and at follow-up (2008, age 24–32) as part of the National Longitudinal Survey of Adolescent Health. Logistic and fractional regression models tested the association between adolescent BMI and smoking status and frequency in adulthood. Respondent weight and height were measured at baseline and converted into age and sex-specific BMI z-scores. Being a smoker was defined as smoking at least once in the past 30 days, while smoking frequency was defined as the proportion of days smoked over the past 30 days. Both outcomes were measured at follow-up. Results Adolescent BMI was positively associated with the transition from nonsmoking to smoking 12 years later for women but not men. Adolescent BMI was also positively associated with smoking frequency among women smokers. Both associations persisted after adjusting for established risk factors and were robust to sensitivity analyses. Conclusions Adolescent BMI was strongly associated with increased cigarette smoking behavior in adulthood for women, even after adjusting for important risk factors. Implications Using a large, nationally representative sample, this study found that adolescent BMI was positively associated with smoking behavior during adulthood for women but not men. These results have potentially important public health implications for future smoking rates in the United States, as the prevalence of overweight and obesity among US adolescent females continues to grow. Thus, it may be important for smoking prevention interventions to prioritize overweight adolescent females moving forward.
