Human Joint Performance and the Roughness of Articular Cartilage

It is known that the surface of articular cartilage is rough and it has been suggested that this is likely to affect the lubrication of human joints. This paper describes the direct measurement of a cartilage surface with a stylus instrument. It is found that the height distribution is Gaussian with an inverse-square power spectrum. It is thus possible to calculate the elastic deflection of the surface under normal walking loads and it is shown that the mean separation of the cartilage surfaces in a human joint varies rather slowly with load. In one particular hip joint at heel strike the real area of contact was calculated to be about 1.3 cm2, the mean gap to be about 60 μm and the trapped volume to be about 80 percent of that when standing.

The treatment of plastic contact developed in this paper is based on three physical observations: that the total volume of metal is not changed by plastic deformation; that the mean indentation pressure is a well-defined material constant applicable to the whole range of likely asperity shapes; and that the displaced material reappears as a uniform rise in the non-contacting surface. An energy-balance argument is used to obtain dimensionless relations between the load, separation, and degree of contact, in terms of the height distribution of the surface. A fourth observation is then added: that the height distributions of many engineering surfaces are, to a good approximation, Gaussian. The relations are worked out in detail for this height distribution and compared with experimental observations. The treatment accurately predicts the behaviour up to extremely high loads; and accounts for the remarkable persistence of asperities on rough surfaces in plastic contact. The argument, and the main supporting experiments, were conceived in terms of the contact of a uniformly loaded nominally flat surface, but the extention to local indentations is quite straightforward. It is shown that for local indentations in homogeneous bodies the real area of contact is always one half of the nominal area. This unexpected result is in fact accurately confirmed by experiment. The treatment also discusses the effect of a hard or soft surface layer on the indented body, and again the predictions are supported by practical measurements.

The statistical analysis of the real area of contact proposed by Greenwood and Williamson is revisited. General and simplified equations for the mean asperity real area of contact, number of contacts, total real area of contact, and mean real pressure as a function of apparent pressure for the case of elastic junctions are presented. The critical value of the mean asperity pressure at which plastic flow starts when a polymer contacts a hard material is derived. Based on this, conditions of elastic and plastic junctions for polymers are defined by a “polymer” plasticity index, Ψp which depends on the complex modulus, Poisson’s ratio, yield strength, and surface topography. Calculations show that most dynamic contacts that occur in a computer-magnetic tape are elastic, and the predictions are supported by experimental evidence. Tape wear in computer applications is small and decreases Ψp by less than 10 percent. The theory presented here can also be applied to rigid and floppy disks.

This study developed an elastic-plastic microcontact model that considers the elliptical contact of surface asperities. In the elastoplastic regime, the relations of the mean contact pressure and contact area of asperity to its contact interference are modeled considering the continuity and smoothness of variables across different modes of deformation. Results obtained from this model are compared with other existing models such as that calculated by the GW, CEB, Zhao and Horng models. The elliptic contact model and circular contact model can deviate considerably in regard to the separation and real area of contact.

A critical examination of the analytical solution presented in the classic paper of Greenwood and Williamson (1966), (GW) on the statistical modeling of nominally flat contacting rough surfaces is undertaken in this study. It is found that using the GW simple exponential distribution to approximate the usually Gaussian height distribution of the asperities is inadequate for most practical cases. Some other exponential type approximations are suggested, which approximate the Gaussian distribution more accurately, and still enable closed form solutions for the real area of contact, the contact load, and the number of contacting asperities. The best-modified exponential approximation is then used in the case of elastic-plastic contacts of Chang et al. (1987) (CEB model), to obtain closed-form solutions, which favorably compare with the numerical results using the Gaussian distribution.

The current work considers the multiscale nature of surface roughness in a new model that predicts the real area of contact and surface separation, both as functions of load. By summing the distance between the two surfaces at all scales, a model of surface separation as a function of dimensionless load is also obtained. A previous rough surface multiscale contact model was based on stacked elastic-plastic spheres. This work uses stacked 3-D sinusoids to represent the asperities in contact at each scale of the surface. The results are also compared to several other existing rough surface contact models and experimental results. It appears that the statistical and multiscale models make different assumptions when considering surface separation. The statistical model does not consider the effect of deformation on the mean surface height, but the multiscale model does (the location of the mean height will change as the surface deforms). When deformation is included and the surface separation becomes zero, the surfaces must also come into complete contact.

The load-bearing human joint is a self-acting dynamically loaded bearing which employs a porous and elastic bearing material (articular cartilage) and a highly non-Newtonian lubricant (synovial fluid). The authors' understanding is that the human joint experiences fluid-film (including elastohydrodynamic), mixed and boundary lubrication in its various operating conditions. It has been recognized that squeeze-film action is capable of providing considerable protection to the cartilage surface once a fluid film is generated (6) (8)§. Furthermore, the possibility of an increasing concentration of hyaluronic acid in synovial fluid during the squeeze-film action due to the porous nature of the cartilage and its surface topography and the known relationship between this concentration and the effective viscosity (7) has led to the concept of ‘boosted lubrication’ as an important feature of joint behaviour (10). A mathematical analysis of the concept of boosted lubrication of human joints is presented in this paper. The predictions of the analysis are shown to be in good agreement with experimental findings (12).

Background: The intermetatarsal joint between the fourth and fifth metatarsals (4-5 IM) is important in defining fifth metatarsal fractures. The purpose of the current study was to quantify this joint in order to determine the mean cartilage area, the percentage of the articulation that is cartilage, and to give the clinician data to help understand the joint anatomy as it relates to fifth metatarsal fracture classification. Methods: Twenty cadaver 4-5 IM joints were dissected. Digital images were taken and the articular cartilage was quantified by calibrated digital imaging software. Results: For the lateral fourth proximal intermetatarsal articulation, the mean area of articulation was 188 ± 49 mm2, with 49% of the area composed of articular cartilage. The shape of the articular cartilage had 3 variations: triangular, oval, and square. A triangular variant was the most common (80%, 16 of 20 specimens). For the medial fifth proximal intermetatarsal articulation, the mean area of articulation was 143 ± 30 mm2, with 48% of the joint surface being composed of articular cartilage. The shape of the articular surface was oval or triangular. An oval variant was the most common (75%, 15 of 20 specimens). Conclusion: This study supports the notion that the 4-5 IM joint is not completely articular and has both fibrous and cartilaginous components. Clinical Relevance: The clinical significance of this study is that it quantifies the articular surface area and shape. This information may be useful in understanding fifth metatarsal fracture extension into the articular surface and to inform implant design and also help guide surgeons intraoperatively in order to minimize articular damage.

Background: Osteochondral allograft transplantation is 1 treatment option for focal articular cartilage defects of the knee. Large irregular defects, which can be treated using an oblong allograft or multiple overlapping allografts, increase the procedure’s technical complexity and may provide suboptimal cartilage and subchondral surface matching between donor grafts and recipient sites. Purpose: To quantify and compare cartilage and subchondral surface topography mismatch and cartilage step-off for oblong and overlapping allografts using a 3-dimensional simulation model. Study Design: Controlled laboratory study. Methods: Human cadaveric medial femoral hemicondyles (n = 12) underwent computed tomography and were segmented into cartilage and bone components using 3-dimensional reconstruction and modeling software. Segments were then exported into point-cloud models. Modeled defect sizes of 17 × 30 mm were created on each recipient hemicondyle. There were 2 types of donor allografts from each condyle utilized: overlapping and oblong. Grafts were virtually harvested and implanted to optimally align with the defect to provide minimal cartilage surface topography mismatch. Least mean squares distances were used to measure cartilage and subchondral surface topography mismatch and cartilage step-off. Results: Cartilage and subchondral topography mismatch for the overlapping allograft group was 0.27 ± 0.02 mm and 0.80 ± 0.19 mm, respectively. In comparison, the oblong allograft group had significantly increased cartilage (0.62 ± 0.43 mm; P < .001) and subchondral (1.49 ± 1.10 mm; P < .001) mismatch. Cartilage step-off was also found to be significantly increased in the oblong group compared with the overlapping group ( P < .001). In addition, overlapping allografts more reliably provided a significantly higher percentage of clinically acceptable (0.5- and 1-mm thresholds) cartilage surface topography matching (overlapping: 100% for both 0.5 and 1 mm; oblong: 90% for 1 mm and 56% for 0.5 mm; P < .001) and cartilage step-off (overlapping: 100% for both 0.5 and 1 mm; oblong: 86% for 1 mm and 12% for 0.5 mm; P < .001). Conclusion: This computer simulation study demonstrated improved topography matching and decreased cartilage step-off with overlapping osteochondral allografts compared with oblong osteochondral allografts when using grafts from donors that were not matched to the recipient condyle by size or radius of curvature. These findings suggest that overlapping allografts may be superior in treating large, irregular osteochondral defects involving the femoral condyles with regard to technique. Clinical Relevance: This study suggests that overlapping allografts may provide superior articular cartilage surface topography matching compared with oblong allografts and do so in a more reliable fashion. Surgeons may consider overlapping allografts over oblong allografts because of the increased ease of topography matching during placement.

Abstract. Long-term observations of total nightglow intensity of the atomic oxygen red 630.0 nm line at Abastumani (41.75° N, 42.82° E) in 1957–1993 and measurements of the ionosphere F2 layer parameters from the Tbilisi ionosphere station (41.65° N, 44.75° E) in 1963–1986 have been analyzed. It is shown that a decrease in the long-term trend of the mean annual red 630.0 nm line intensity from the pre-midnight value (+0.770±1.045 R/year) to its minimum negative value (−1.080±0.670 R/year) at the midnight/after midnight is a possible result of the observed lowering of the peak height of the ionosphere F2 layer electron density hmF2 (−0.455±0.343 km/year). A theoretical simulation is carried out using a simple Chapman-type layer (damping in time) for the height distribution of the F2 layer electron density. The estimated values of the lowering in the hmF2, the increase in the red line intensity at pre-midnight and its decrease at midnight/after midnight are close to their observational ones, when a negative trend in the total neutral density of the upper atmosphere and an increase in the mean northward wind (or its possible consequence – a decrease in the southward one) are assumed.

We reasoned that with an optimal aiding horizontal force, the reduction in metabolic rate would reflect the cost of generating propulsive forces during normal walking. Furthermore, the reductions in ankle extensor electromyographic (EMG) activity would indicate the propulsive muscle actions. We applied horizontal forces at the waist, ranging from 15% body weight aiding to 15% body weight impeding, while subjects walked at 1.25 m/s. With an aiding horizontal force of 10% body weight, 1) the net metabolic cost of walking decreased to a minimum of 53% of normal walking, 2) the mean EMG of the medial gastrocnemius (MG) during the propulsive phase decreased to 59% of the normal walking magnitude, and yet 3) the mean EMG of the soleus (Sol) did not decrease significantly. Our data indicate that generating horizontal propulsive forces constitutes nearly half of the metabolic cost of normal walking. Additionally, it appears that the MG plays an important role in forward propulsion, whereas the Sol does not.
