Cost-Effectiveness of a Photovoltaic-Powered Heat Pump Water Heating System vs. Solar Thermal Water Heating

The cost-effectiveness of a photovoltaic (PV) powered heat pump water heater (HPWH) system is compared to that of a traditional solar thermal water heating system. HPWH evaporators are most often located inside the conditioned building space, resulting in a year-round cooling effect in the building. This effect is beneficial during the cooling season but detrimental during the heating season. The significance of this cooling effect was evaluated as part of the life cycle cost (LCC) analysis of the PV-powered HPWH system. Four different locations were considered: Boulder, CO; Miami, FL; Chicago, IL; and Seattle, WA. For the solar thermal analysis, both electric resistance and gas-fired auxiliary water heating scenarios were considered. Life cycle costs for the PV-HPWH system were calculated for the case of a PV system dedicated to providing electricity for the HPWH, and for the case of a previously planned residential PV system being increased in size to accommodate the HPWH. This latter case uses a lower, incremental cost of increasing the size of the PV system. The most notable results of the analysis are summarized below: • In general, the solar thermal system is more cost effective than the PV-HPWH system, even using the incremental cost of increasing the size of a planned PV system. • In locations where there are incentives that apply to PV but not solar thermal systems, as in much of Colorado, the PV-HPWH system will be more cost-effective than solar thermal. • The cooling effect of the HPWH evaporator is a net benefit in Miami, FL, but a net penalty in the other three locations. • The PV-HPWH system becomes more cost-effective than solar thermal with gas auxiliary in Miami when the price of natural gas is increased from $1 to $1.50 per therm. • Increasing the price of gas in the other locations does not make the PV-HPWH system compete against solar thermal because the cooling effect penalty also increases with the price of natural gas.

A photovoltaic-thermal water heating system is a hybrid energy conversion device transforming the incident solar radiation to yield electrical energy and thermal energy. Plane reflectors are found to be a convenient option for enhancing the solar radiation incident on the collector plane. The present work investigates the performance of a photovoltaic-thermal water heater integrated with a plane reflector mounted on the top edge of the collector for the tropical climate of Calicut (11.25°N, 75.78°E). Performance testing of the system has been carried out for the winter season of the location. The variations in thermal and electrical output are studied for various inclination angles of the reflector. The system performance is evaluated on the basis of system energy efficiency and exergetic efficiency. It is observed that there is a significant enhancement in the thermal and electrical output of the system with the integration of the reflector as compared to the system without the reflector. For the present system, the reflector tilt angle in the range of 85–100° has been found to be suitable in terms of enhanced system output and exergetic efficiency for the winter conditions of Calicut.

8043 Background: IXA, CAR, ELO and DARin combination with LEN+DEXhave been found superior in efficacy compared to LEN+DEX in the management of R/R MM. Applying indirect treatment comparisons from a network meta-analysis (NMA), this economic evaluation aimed to estimate the comparative cost-effectiveness and cost-utility of these four triplet regimens in terms of progression-free survival (PFS). Methods: In the absence of direct treatment comparison from a single clinical trial, NMA was used to indirectly estimate the comparative PFS benefit of each regimen. A 2-state Markov model simulating the health outcomes and costs was used to evaluate PFS life years (LY) and quality-adjusted life years (QALY) with the triplet regimens over LEN+DEX and expressed as the incremental cost-effectiveness (ICER) and cost-utility ratios (ICUR). Probability sensitivity analyses were conducted to assess the influence of parameter uncertainty on the model. Results: The NMA revealed that DAR+LEN+DEX was superior to the other triplet therapies, which did not differ statistically amongst them. As detailed in the Table, in our cost-effectiveness analysis, all 4 triplet regimens were associated with increased PFSLY and PFSQALY gained (g) over LEN+DEX at an additional cost. DAR+LEN+DEX emerged the most cost-effective with ICER and ICUR of $667,652/PFSLYg and $813,322/PFSQALYg, respectively. The highest probability of cost-effectiveness occurred at a willingness-to-pay threshold of $1,040,000/QALYg. Conclusions: Our economic analysis shows that all the triplet regimens were more expensive than LEN +DEX only but were also more effective with respect to PFSLY and PFSQALY gained. Relative to the other regimens, the daratumumab regimen was the most cost-effective.[Table: see text]

Introduction. Rheumatoid arthritis (RA) is a chronic progressive inflammatory disease that causes joint destruction. The condition imposes a significant economic burden on patients and societies. The present study is aimed at evaluating the cost-effectiveness of Infliximab, Adalimumab, and Etanercept in treating rheumatoid arthritis in Iran. Methods. This is a cost-effectiveness study of economic evaluation in which the Markov model was used. The study was carried out on 154 patients with rheumatoid arthritis in Fars province taking Infliximab, Adalimumab, and Etanercept. The patients were selected through sampling. In this study, the cost data were collected from a community perspective, and the outcomes were the mean reductions in DAS-28 and QALY. The cost data collection form and the EQ-5D questionnaire were also used to collect the required data. The results were presented in the form of an incremental cost-effectiveness ratio, and the sensitivity analysis was used to measure the robustness of the study results. The TreeAge Pro and Excel softwares were used to analyze the collected data. Results. The results showed that the mean costs and the QALY rates in the Infliximab, Adalimumab, and Etanercept arms were $ 79,518.33 and 12.34, $ 91,695.59 and 13.25, and $ 87,440.92 and 11.79, respectively. The one-way sensitivity analysis confirmed the robustness of the results. In addition, the results of the probabilistic sensitivity analysis (PSA) indicated that on the cost-effectiveness acceptability curve, Infliximab was in the acceptance area and below the threshold in 77% of simulations. The scatter plot was in the mentioned area in 81% and 91% of simulations compared with Adalimumab and Etanercept, respectively, implying lower costs and higher effectiveness than the other two alternatives. Therefore, the strategy was more cost-effective. Conclusion. According to the results of this study, Infliximab was more cost-effective than the other two medications. Therefore, it is recommended that physicians use this medication as the priority in treating rheumatoid arthritis. It is also suggested that health policymakers consider the present study results in preparing treatment guidelines for RA.

In Italy in 2020, only 15.5% of school building heritage was retrofitted from an energy and environmental point of view. In this paper, the cost-optimal method was applied to two different school buildings belonging to the same Italian cold climate zone but characterized by different structural and technological solutions. The research aims at defining the cost-effective redevelopment solution among several ones proposed to apply to this building type. At the same time, this paper provides a critical analysis of the methodology applied, highlighting deficiencies related to a not proper evaluation of environmentally friendly retrofitting measures. In a cost-effective context, the main results show that the intervention on the heating system is more convenient than the retrofitting of the envelope. The energy saving is equal to about 35% for both considered schools. Among the different proposed requalification configurations, the adoption of PV (photovoltaic) electric generation is included. In this regard, an optimization procedure was implemented in a generative design environment to maximize energy production with reference to different design parameters. As a result, a solution with south oriented PV modules with a tilt angle of 42° and arranged in 0.7 m spaced rows proved to be the most effective.

Background: Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder that results from regurgitation of acid from the stomach into the esophagus. Treatment available for GERD includes lifestyle changes, antacids, histamine-2 receptor antagonists (H2RAs), proton pump inhibitors (PPIs), and anti-reflux surgery. Aim: The aim of this review is to assess the cost-effectiveness of the use of PPIs in the long-term management of patients with GERD. Method: We searched in PubMed to identify related original articles with close consideration based on inclusion and exclusion criteria to choose the best studies for this narrative review. The first section compares the cost-effectiveness of PPIs with H2RAs in long-term heartburn management. The other sections shall only discuss the cost-effectiveness of PPIs in 5 different strategies, namely, continuous (step-up, step-down, and maintenance), on-demand, and intermittent therapies. Results: Of 55 articles published, 10 studies published from 2000 to 2015 were included. Overall, PPIs are more effective in relieving heartburn in comparison with ranitidine. The use of PPIs in managing heartburn in long-term consumption of nonsteroidal anti-inflammatory drug (NSAID) has higher cost compared with H2RA. However, if the decision-maker is willing to pay more than US$174 788.60 per extra quality-adjusted life year (QALY), then the optimal strategy is traditional NSAID (tNSAID) and PPIs. The probability of being cost-effective was also highest for NSAID and PPI co-therapy users. On-demand PPI treatment strategy showed dominant with an incremental cost-effectiveness ratio of US$2197 per QALY gained and was most effective and cost saving compared with all the other treatments. The average cost-effectiveness ratio was lower for rabeprazole therapy than for ranitidine therapy. Conclusion: Our review revealed that long-term treatment with PPIs is effective but costly. To achieve long-term cost-effective approach, we recommend on-demand approach to treat heartburn symptoms, but if the symptoms persist, treatment with continuous step-down therapy should be applied.


A completed study of a solar hot water heating system installed in a school showed an annual average efficiency of 15%, the low efficiency largely caused by the unfavourable pattern of use in schools. Field studies, in 80 existing and 12 new houses, of a simple domestic hot water system have been initiated to ascertain the influence of the occupants on the actual performance of solar collector systems. The development of testing methods of solar collectors and solar water heating systems is being undertaken in close collaboration with the B.S.I. and the E.E.C. Solar space heating is being investigated in two experimental low energy house laboratories, one using conventional solar collectors with interseasonal heat storage and the other a heat pump with an air solar collector. Studies of the cost-effectiveness of solar collector applications to buildings in the U.K. show that they are far less cost-effective than other means of conserving energy in buildings.
