Introduction. Work of locomotive crew members (engine drivers and engine driver assistants) in railway transport is associated with exposure to complex of occupational hazards including psychoemotional stress and shiftwork (with night shifts) along with noise and vibration.Objective. To study influence of shift work (with irregular alternation of day and night shifts) on biorhythms disturbances in locomotive crew workers.Materials and methods. Polls, simulators of occupational activities with training complex «Cabin of engine driver EP1M», daily monitoring of blood pressure, statistic data processing by SPSS–20 software.Results. Findings are that 953 locomotive crew workers (94%) experienced sensations of temporary disorientation, slight dizziness, sleepiness, interpreted as monotony and sensor deprivation. 61% of the examinees demonstrated frustration signs, in 30% of the cases there were certain manifestations of mental ill-being — irritability, fatigue, weariness, absent-mindedness, anxiety; 27.2% of the testees experiences various sleep disorders; 13.8% of the workers complained of «light sleep», 2% of those — restless sleep, 0.6% — sleeplessness. In occupationally related biorhythms disturbance, the body undergoes occupational adaptation, with partial transformation for atypical schedule of sleep and wakefulness.Results. To preserve health, efforts should be aimed at adaptation to shifted occupational activities with safe adaptogenic medications, low-caloric diet for individuals working at irregular shifts, create conditions for adequate physical activity of the workers between shifts. Necessities also are individual norms for blood pressure parameters in engine drivers with night shiftwork, specification of antihypertensive treatment for patients with irregular shifted work.

To simulate and monitor the evolution of posttraumatic sepsis in mice, we combined a two-hit model of trauma/hemorrhage (TH) followed by polymicrobial sepsis with repetitive blood sampling. Anesthetized mice underwent femur fracture/sublethal hemorrhage and cecal ligation and puncture (CLP) 48 h later. To monitor outcome-dependent changes in circulating cells/biomarkers, mice were sampled daily (facial vein) for 7 days and retrospectively divided into either dead (DIE) or surviving (SUR) by post-CLP day 7. Prior to CLP, AST was 3-fold higher in DIE, while all other post-TH changes were similar between groups. There was a significant post-CLP intergroup separation. In SUR, RBC and Hb were lower, platelets and neutrophils higher, and lymphocytes mixed compared to DIE. In DIE, all organ function markers except glucose (decrease) were few folds higher compared to SUR. In summary, the combination of daily monitoring with an adequate two-hit model simulates the ICU setting, allows insight into outcome-based responses, and can identify biomarkers indicative of death in the acute posttraumatic sepsis in mice.

With new advances in technology, remote monitoring of heart failure (HF) patients has become increasingly prevalent and has the potential to greatly enhance the outcome of care. Many studies have focused on implementing systems for the management of HF by analyzing physiological signals for the early detection of HF decompensation. This paper reviews recent literature exploring significant physiological variables, compares their reliability in predicting HF-related events, and examines the findings according to the monitored variables used such as body weight, bio-impedance, blood pressure, heart rate, and respiration rate. The reviewed studies identified correlations between the monitored variables and the number of alarms, HF-related events, and/or readmission rates. It was observed that the most promising results came from studies that used a combination of multiple parameters, compared to using an individual variable. The main challenges discussed include inaccurate data collection leading to contradictory outcomes from different studies, compliance with daily monitoring, and consideration of additional factors such as physical activity and diet. The findings demonstrate the need for a shared remote monitoring platform which can lead to a significant reduction of false alarms and help in collecting reliable data from the patients for clinical use especially for the prevention of cardiac events.

Currently, there are many consequences of the new coronavirus infection. We shall study the actual long-term consequences of this disease for population health in the coming years. It is necessary to consider concomitant chronic diseases in patients who are particularly dangerous in the post-COVID period. Vibration disease is one of the leading occupational pathologies in the Russian Federation. Symptoms of vibration disease are vegetative-vascular disorders, cold, cyanosis, paresthesia, violation of regional blood circulation of the extremities. As is known from numerous sources, the defeat of the cardiovascular system (CVS) against the background of COVID-19 infection occurs through angiotensin-converting enzyme (ACE-2) receptors, which presents in large numbers in the endothelium of veins and arteries. The study aims to research hemodynamics features in patients with vibration disease after a new coronavirus infection. In the clinic of occupational pathology, the researchers examined 28 patients with a previously established diagnosis of VD after a coronavirus infection. We did a Holter ECG, ultra-sound examination of the arteries and veins of the upper extremities. Arrhythmias in the form of supraventricular extrasystoles, paroxysms of atrial fibrillation, positional sinus tachycardia occur in patients with VB after a new coronavirus infection on an ECG and daily monitoring of the ECG by Holter. As is known, cardiac arrhythmias are essential in the development of acute and chronic vascular pathology, characterized by a decrease in arterial blood filling and changes. Ultrasound of the vessels of the upper extremities revealed moderate expansion of the radial and ulnar veins, insufficiency of the valve apparatus during functional tests, increased venous outflow. There was also an increase in the indicators of peripheral vascular resistance, which indicates violations of the tonic properties of the vessels of the upper extremities and violations of vasodilation. Scientists identified violations of the tonic properties of blood vessels in patients with vibration disease in the post-COVID period on ultrasound, arrhythmias in the form of supraventricular extrasys-toles, paroxysms of atrial fibrillation, positional sinus tachycardia on daily ECG monitoring.
