Television, Reading, and Computer Time: Correlates of School-Day Leisure-Time Sedentary Behavior and Relationship With Overweight in Children in the U.S.

Background:The purposes were 1) to determine if different leisure-time sedentary behaviors (LTSB), such as TV/video/video game viewing/playing (TV), reading for pleasure (reading), and nonschool computer usage, were associated with childhood overweight status, and 2) to assess the social-ecological correlates of LTSB.Methods:The analytic sample was 33,117 (16,952 boys and 16,165 girls) participants from the 2003 National Survey of Children’s Health. The cut-point for excessive TV and nonschool computer usage was ≥ 2 hr/day. High quantities of daily reading for pleasure were classified as ≥ 31 min/day. Weighted descriptive characteristics were calculated on the sample (means ± SE or frequency). Logistic regression models were used to determine if the LTSB were associated with overweight status and to examine social-ecological correlates.Results:Over 35% of the sample was overweight. Odds of being overweight were higher in the 2 to 3 hr/day (OR: 1.48, 95% CI: 1.24, 1.76) and ≥ 4 hr/day (OR: 1.52, 95% CI: 1.22, 1.91) daily TV groups compared with none. Reading and nonschool computer usage was not associated with being overweight.Conclusions:TV was associated with overweight classification; however, nonschool computer usage and reading were not. Several individual, family, and community correlates were associated with high volumes of daily TV viewing.

Abstract Objectives Evidence that sedentary behaviors were correlated with unhealthy diet and physical activity were mostly among children and adolescents, but still limited for adults. The aim of this study was to examine the associations between screen-based leisure-time sedentary behaviors (LTSB) including television time and computer time with unhealthy dietary behaviors, and leisure-time physical activity (LTPA) among Chinese adults. Methods Participants (n = 10,108, aged 18–59 years) from a recent cross-sectional study of CHNS (China Health and Nutrition Survey) conducted in 2015 were analyzed. Time spent in LTSB and LTPA were collected from questionnaire. Unhealthy dietary behaviors were collected from three consecutive 24-hour food recalls and the food frequency method. Multivariable logistic regression models were produced to study the odds ratios for unhealthy dietary behaviors and LTPA as the function of the quintile of LTSB. Results Versus the lowest quartile, the OR(95%CI) of the highest quintile of computer time for insufficient vegetable intake (< 300 g/d) was 1.4(1.0,2.0) for male and 1.5(1.1,2.2) for female; the OR(95%CI) for excessive snack intake (≥1 time/day) was 1.6(1.0,2.3) for male and 1.7(1.3,2.4) for female; the OR(95%CI) for excessive western fast food intake (≥1 time/week) was 2.4(1.2,4.4) for male and 2.7(1.4,4.9) for female. The OR(95%CI) of the highest quintile of television time for no LTPA group (0 Mets.h/w) was 1.8(1.3,2.6) for male and 1.5(1.1,2.0) for female; the OR(95%CI) for sufficient LTPA group (≥8 Mets.h/w) was 0.5(0.4,0.8) for male and 0.7(0.5,0.9) for female. The OR(95%CI) of the highest quintile of computer time for no LTPA group (0 Mets.h/w) was 0.4(0.3,0.6) for male and 0.5(0.4,0.7) for female; the OR(95%CI) for sufficient LTPA (≥8 Mets.h/w) group was 2.4(1.7,3.3) for male and was 2.1(1.5,2.9) for female. Conclusions LTSB were associated with unhealthy dietary behaviors and mixed LTPA among Chinese adults. The patterns of “Television time-less LTPA” and “Computer time-unhealthy diet-more LTPA” for Chinese adults were principal associations. Our findings suggest the need for different behavior preventive intervention on different types of screen-based LTSB among Chinese adults. Purpose. To evaluate children’s psychosocial and environmental factors associated with sedentary behavior (SB).Method. The study involved a total of 420 children (mean 9.2 years; 52.9% girls) from the community of Madrid, Spain. SB and physical activity (PA) were objectively measured using accelerometers. TV viewing and potential correlates were assessed by questionnaire. Mixed-model regression analysis, adjusted for clustering within school locations, evaluated the relation of each independent variable with SBs.Results. Girls showed higher levels of SB than boys, whereas boys reported more TV viewing (p<.001in all cases). Regression analysis showed that MVPA levels were negatively related to objective SB measurement in both boys and girls (p<.001). Parent and friend support to PA were negatively associated with SB on weekdays in boys and girls, respectively (p<.05). In the boys’ group, parental professional level was a positive predictor of SB on weekend days (p=.011). Boys with more positive neighborhood perceptions spent less time watching TV (p<.001), whereas mother’s leisure-time PA level was a negative correlate of TV viewing in girls’ group (p<.01).Conclusion. Different psychosocial and environmental correlates of SB were identified. Present findings are promising targets for interventions to improve children’s health.

Background: Cardiovascular Disease (CVD) remains the leading cause of death in the US. Although Physical Activity (PA) has been inversely associated with the risk of CVD, few studies have examined whether sedentary behaviors modify such association. Our goal was to examine associations of leisure time PA with risk of CVD and mortality and the role of sedentary behavior as potential effect modifier among US veterans. Methods: We analyzed self-reported data on leisure time PA, television watching, and time spent on the computer among 438,364 participants of the Veterans Affairs Million Veteran Program from 2011 to 2018. We calculated metabolic equivalent of task-hours per week (MET-h/week) for each person and used electronic health record data to ascertain CVD. Results: Mean age was 64.6 ± 12.6 years and 92% were men. During a mean follow up of 3.3 years, we observed 22,942 new cases of CVD and 48,325 deaths. There was an inverse relation of leisure time PA with CVD and total mortality [HR: 0.96 (0.95-0.97) and 0.91 (0.90-0.92) per 2 MET-h/week increment for CVD and total mortality, respectively]. The associations of PA with both incident CVD and mortality were stronger in participants who spent more time watching television or on computer (all p values for interaction < 0.01). No interaction of PA with time spent on video game was observed (p>0.05). Conclusions: Leisure time PA is inversely associated with risk of CVD and mortality among US veterans and such relations were stronger in participants who spent more time watching television or on computer.

Background:The primary and secondary purposes were to examine social-ecological correlates of excessive TV viewing (>2hr/day) in American children 1) between race/ethnic groups and 2) between boys and girls.Methods:Children (n = 48,505) aged 6 to 18 years from the 2007 National Survey of Children’s Health were included. Social-ecological correlates included individual-, family-, and community-level variables. Logistic regression analyses were used for race/ethnicity [Non-Hispanic White (NHW), Non-Hispanic Black (NHB), Hispanic, other] and sex groups (boys, girls), to determine associated correlates.Results:By race/ethnicity, 16.6%, 37.8%, and 24.5% of NHW, NHB, and Hispanic exceeded recommendations. For boys and girls, 23.7% and 18.2% viewed excessive TV. Having a TV in the bedroom and higher poverty level were associated with excessive TV in all groups. Differences by race/ethnicity were age, sex, TV in the bedroom, extracurricular activities, physical activity, adequate sleep, family structure, family meals, knowing child’s friends, parent/ child communication, and neighborhood characteristics. Differences by sex were family structure, parent/ child communication, family meals, and neighborhood elements.Conclusion:Social-ecological correlates and associated prevalence of excessive TV viewing differed across subgroups. These specific correlates can be targeted in tailored interventions.

This study calculated the exposure-response rates of social-ecological correlates of practicing regular (>150 min/week) leisure-time physical activity (PA) in 393,648 adults from the 27 Brazilian state capitals who participated in a national survey between 2006 and 2016. Regular PA encouraging factors were inputted into an exposure-response model. Growth rates for the odds ratio and prevalence of regular PA were calculated for each increase of one encouraging factor. Regular PA was reported by 22% of the participants (25% of men and 20% of women). More than 40% of men and 30% of women with higher intra-personal encouraging conditions reported practicing regular PA. There was a 3% (ages 18–32 years) to 5% (ages 46–60 years) increase in regular PA practice in men for each increase in an encouraging climate factor (temperature from 21 °C to 31 °C, humidity from 65% to 85%, 2430 to 3250 h of sun/year, and from 1560 to 1910 mm of rain/year). Encouraging intra-personal factors and favorable climate conditions had larger effects on regular PA practice than the built environment and socio-political conditions; the latter two had independent effects, but did not have a cumulative effect on PA.

Background:The purpose of this study was to examine the association between parent and adolescent levels of physical activity, sedentary behaviors, and sleep among a group of overweight and obese adolescents.Methods:Baseline data of parent–adolescent pairs who enrolled in a lifestyle modification intervention were analyzed for this study (n = 176). Participants completed questionnaires about their screen time (TV, video game, and computer time), wore an accelerometer for 8 days, and completed a sleep diary for 1 week. In total, 98 parent–adolescent dyads provided valid data for the analyses. Multivariable regression analyses were conducted to assess the relationship between parent and adolescent’s moderate-to-vigorous activity (MVPA), step counts, sedentary behaviors, and sleep duration. Analyses were split by weekday, weekday evening and weekend.Results:Parent–adolescent MVPA was significantly associated on weekdays (b = 0.18; SE = 0.08; β = 0.26), weekday evenings (b = 0.21; SE = 0.08; β = 0.28), and weekends (b = 0.29; SE = 0.12; β = 0.27). This study found associations between parent–child video game time on weekends (b = 1.10; SE = 0.49; β = 0.24) and computer time on weekdays (b = 0.42; SE = 0.19; β = 0.23). Adolescent sleep was associated with parental sleep on weekdays only (b = 0.38; SE = 0.09; β = 0.46).Conclusions:The findings warrant further investigation into the direction and mechanism of the relationship between parent and adolescent weight related behaviors.

BACKGROUND No study has comprehensively investigated the association between the usage of typical screen-based electronic media devices and sleep quality in a Chinese population with individuals in a wide range of ages. OBJECTIVE This study aimed to understand the characteristics of television (TV) viewing, computer usage, and mobile phone usage in a representative Chinese population in Macau and to examine their roles in predicting the variations in sleep quality. METHODS This cross-sectional study was an analysis of 1500 Macau residents aged 15 to 90 years based on a community-based health needs assessment study entitled, “Healthy Living, Longer Lives.” Data collection was conducted in 7 districts of Macau from 2017 to 2018 through face-to-face interviews. The durations of daily TV viewing, computer usage, and mobile phone usage were recorded in a self-administered questionnaire. The Chinese version of the Pittsburgh Sleep Quality Index (PSQI) was used to assess the sleep quality. RESULTS The prevalence of TV, computer, and mobile phone usage was 78.4% (1176/1500), 51.6% (769/1490), and 85.5% (1276/1492), respectively. The average daily hours of usage were 1.75 (1.62), 1.53 (2.26), and 2.85 (2.47) hours, respectively. Females spent more time watching TV (<i>P</i>=.03) and using mobile phones (<i>P</i>=.02) and less time on the computer (<i>P</i>=.04) as compared to males. Older adults were more likely to watch TV while young people spent more time using the computer and mobile phones (<i>P</i> for all trends&lt;.001). The mean PSQI global score was 4.79 (2.80) among the participants. Females exhibited significantly higher PSQI scores than males (5.04 vs 4.49, respectively; <i>P</i>&lt;.001). No linear association was observed between the PSQI score and the amount of time spent on the 3 electronic devices (<i>P</i>=.58 for PSQI-TV, <i>P</i>=.05 for PSQI-computer, and <i>P</i>=.52 for PSQI-mobile phone). Curve estimation showed significant quadratic curvilinear associations in PSQI-TV (<i>P</i>=.003) and PSQI-computer (<i>P</i>&lt;.001) among all the participants and in PSQI-mobile phone among youths (age, 15-24 years; <i>P</i>=.04). After adjustment of the gender, age, body mass index, demographics, and lifestyle factors, more than 3 hours of TV viewing and 4 hours of computer usage or mobile phone usage was associated with 85% (95% CI 1.04-1.87; <i>P</i>=.008), 72% (95% CI 1.01-2.92; <i>P</i>=.045), and 53% (95% CI 1.06-2.22; <i>P</i>=.03) greater odds of having poor sleep quality (PSQI score&gt;5), respectively. CONCLUSIONS The mobile phone was the most popular screen-based electronic device used in the Macau population, especially among young people. “J” shape associations were observed between sleep quality and the duration of TV viewing, computer usage, and mobile phone usage, indicating that the extreme use of screen-based electronic devices predicted poorer sleep status, whereas moderate use would be acceptable.

Background: Sedentary behaviors (SB) are activities associated with prolonged time periods of sitting, reclining, or laying down during waking hours. While the relation between SB and physical activity is complex, the common consensus is that SB is not the absence of physical activity and consists of its own determinants posing distinct health outcomes. These behaviors are of significant public health importance as the majority of Americans spend much of their days in SB and due to the increased risks of morbidity and mortality associated with SB. Adverse health outcomes associated with SB include cardiovascular disease, obesity, metabolic syndrome, hypertension and mortality. Television-viewing time and total sitting time have both been used widely to assess time spent in SB and therefore we hypothesize that TV-viewing time and total hours sitting will have high concordance and can be used interchangeably to represent sedentary behaviors. Methods: Using a sample (n = 2858) from the Images of a Healthy Worksite study, a group-randomized control trial involving nutrition and physical activity, the current study assessed how two different tools measured time spent in SB. Tertiles were created based upon the distribution of hours sitting and hours spent TV-vewing. Weighted Kappa statistics were used to measure concordance between hours of TV-viewing and total hours of time spent sitting for the entire sample and for subgroup analyses. Results: Weighted Kappa statistics for tertiles of hours sitting and tv hours were 0.0046, indicating little agreement on the television and the sitting items. Kappa w statistics for BMI categories also showed poor agreement (obese Kappa w = 0.02, overweight Kappa w = 0.002, and healthy subjects Kappa w = 0.006. The Kappa w statistics for males and females were -0.006 and 0.02, respectively. Kappa w statistics for the intervention group (Kappa w = 0.007) and for the control group (Kappa w = 0.0005) also showed little agreement. Conclusions: These results suggest that although commonly used, using television viewing time and total time spent sitting as interchangeable markers of SB, is not a valid assumption. We propose that total time spent sitting and hours spent television-viewing represent different domains within the construct of sedentary behavior. It is important for future researchers to use measures of sedentary behavior that capture the numerous domains involved in measuring SB to allow for the most sensitive measurement of this high-risk behavior.
