A new model to deliver scheduled outpatient care

Here we describe an integrated model for scheduled care (the ‘cluster clinic’). Following a pilot in April 2018, cluster clinics were established across Aberdeen City from April 2019 but not the area surrounding Aberdeen (ie, Aberdeenshire). There were 2360 referrals in 2017/2018 (pre-cluster clinic), and 2615 in 2019/2020 (post-Aberdeen City cluster clinics). The proportions of referrals from City practices seen pre-cluster and post-cluster were 72% and 56%, respectively, and from Shire practices the corresponding proportions were 70% and 65%. The cluster clinic received positive feedback from parents and referring clinicians and was not associated with increased ‘missed diagnoses’ compared with business as usual clinic. The cluster clinic model is a realistic and effective method to deliver integrated scheduled care for children.

The proposed model begins with the Garrity and Sanders model of technologic acceptance and develops an extended nomological network of success factors that draws on motivation and flow theory.

Lean Manufacturing (LM) has always been intended as a means to improve efficiency by reducing operations costs, but the recent focus on sustainability and its three pillars (economic, environmental, and social) raises new issues. In this paper, a new model that links LM with sustainability is proposed and then refined through a cross-sector multiple case study. The results highlight a need to align lean implementation processes with sustainability strategy to avoid negative impacts that LM could have on the components of sustainability.

ObjectivesDiabetic eye disease is a leading cause of blindness but can be mitigated by regular eye assessment. A framework of issues, developed from the literature of barriers to eye assessment, was used to structure an examination of perceptions of a new model of care for diabetic retinopathy from the perspective of staff using the model, and health professionals referring patients to the new service.DesignMultimethod: interviews and focus groups, and a separate survey.SettingA new clinic based on an integrated model of care was established at a hospital in outer metropolitan Sydney, Australia in 2017. Funded jointly by Centre for Eye Health (CFEH) and the hospital, the clinic was equipped and staffed by optometrists who work alongside the ophthalmologists in the existing hospital eye clinic.ParticipantsFive (of seven) hospital staff working in the clinic (ophthalmologists and administrative officers) or referring to it from other departments (endocrinologists); nine optometrists from CFEH who developed or worked in the clinic; 10 community-based optometrists as potential referrers.ResultsThe new clinic was considered to have addressed known barriers to eye assessment, including access, assistance for patients unable/unwilling to organise eye checks and efficient management of human resources. The clinic optimised known drivers of this model of care: providing clear scope of practice and protocols for shared care between optometrists and ophthalmologists, good communication between referrers and eye professionals and a collegial approach promoting interprofessional trust. Remaining areas of concern were few referrals from general practitioners, fewer referrals from hospital endocrinologists than expected and issues with stretched administrative capacity. There were also perceived mismatches between the priorities of hospital management and aims of the clinic.ConclusionsThe new model was considered to have addressed many of the barriers to assessment. While there remain issues with the model, there were also unexpected benefits.

Data mining has various applications for customer relationship management. In this article, we introduce a framework for identifying appropriate data mining techniques for various CRM activities. This article attempts to integrate the data mining and CRM models and to propose a new model of Data mining for CRM. The new model specifies which types of data mining processes are suitable for which stages/processes of CRM. In order to develop an integrated model it is important to understand the existing Data mining and CRM models. Hence the article discusses some of the existing data mining and CRM models and finally proposes an integrated model of data mining for CRM.

We have developed a new model to describe endogenous glucose kinetics during a labeled (hot) intravenous glucose tolerance test (IVGTT) to derive a time profile of endogenous glucose production (EGP). We reanalyzed data from a previously published study (P. Vicini, J. J. Zachwieja, K. E. Yarasheski, D. M. Bier, A. Caumo, and C. Cobelli. Am J Physiol Endocrinol Metab 276: E285–E294, 1999), in which insulin-modified [6,6-2H2]glucose-labeled IVGTTs (0.33 g/kg glucose) were performed in 10 normal subjects. In addition, a second tracer ([U-13C]glucose) was infused in a variable rate to clamp the endogenous glucose tracer-to-tracee ratio (TTR). Our new model describing endogenous glucose kinetics was incorporated into the two-compartment hot minimal-model structure. The model gave estimates of glucose effectiveness [1.54 ± 0.31 (SE) ml·kg−1·min−1], insulin sensitivity (37.74 ± 5.23 104 dl·kg−1·min−1·μU−1·ml), and a new parameter describing the sensitivity of EGP to the inhibitory effect of insulin (IC50 = 0.0195 ± 0.0046 min−1). The model additionally provided an estimate of the time course of EGP showing almost immediate inhibition, followed by a secondary inhibitory effect caused by infusion of insulin, and a large overshoot as EGP returns to its basal value. Our estimates show very good agreement with those obtained via deconvolution and the model-independent TTR clamp technique. These results suggest that the new integrated model can serve as a simple one-step approach to obtain metabolic indexes while also providing a parametric description of EGP.

Quality of service (QoS) has been proofed to be a great significance effect in the utilization of cloud service in higher education institutions (HEI’s), due to having many aspects determine the acceptance level of QoS while providing cloud service, QoS acceptance not given sufficient attention from HEIs and decision-makers. In this research, previous QoS studies are review in this field to identify QoS acceptance variables by review studies for the last 5 years. Thus, with the aim to discover QoS aspects this research intends to develop an integrated model to understand these variables and relationship between QoS acceptance variables, which must be efficiently studied. Hence, the integrated model between TOE (Technological, Organizational, and environmental) and HOT (Human, Organizational, and Technological) theory is presented in this research to be the core contribution. The integrated model helps HEIs to evaluate and explore QoS aspects to increase utilizing of cloud service, improving HEIs efficiency, and help decision-makers to facilitate the acceptance of QoS in HEIs

In this paper, an improved integrated model of electromagnetic scattering for two dimensional fractal sea surface is built. Both geometrical fractal characteristic and permittivity characteristic of sea water are strictly considered, especially for the effects of salinity and temperature on the electromagnetic field scattered by sea water are added on. Finally, the calculated results from the new model are in accord with experimental results in terms of backscattering coefficient of sea water with no more than 2 dB error, which present higher precision than those traditional models. Thus, by using this model, the whole electromagnetic scattering characteristics of sea surface are obtained.

This paper suggests a new model to teach language skills. The aim of this model is to integrate the three most influential theories in the process of language teaching and learning which are behaviorism, cognitivism and constructivism. This model explains how these three theories are integrated with each other in the process of language teaching and learning to complete each other taking into consideration the strengths and weaknesses of each theory. This model is mainly used for teaching language skills which are reading, writing, speaking and listening. Reviewing previous literature, it is clear that there is no integrated model that has tried to link these theories although there are suggestions from scholars to integrate them in a single model because there is no theory that can describe the whole process of learning without the interference of the other theories. This model suggests that teaching any skill can be divided into two phases. The first one is introducing the skill theoretically and the second one is practice. The first phase requires constructivism in order to build the students’ knowledge concerning the skill, and the second one is practice which depends on behaviorism through providing different drills to students. Cognitivism is the link between constructivism and behaviorism. Thus, creating knowledge is the core of constructivism but it is mentally driven as it requires cognitive processes. Also, behaviorism focuses on practice and it did not account for the cognitive processes which are essential especially because practice is associated with the mental activation of all the linguistic knowledge. Therefore, the paper discusses the three theories and the rational for the new model as well as the process of teaching following the suggested model.
