Relationship between sleep disorders, HIV status and cardiovascular risk: cross-sectional study of long-haul truck drivers from Southern Africa

ObjectivesLong-haul truck drivers (TDs) may have lifestyles that promote cardiovascular disease (CVD), including diet, sleep and activity issues. Most studies conducted among truckers investigated the relationship between poor sleep and cardiometabolic health, but none assessed whether suspected obstructive sleep apnoea (OSA) and shortened sleep were associated with markers of cardiometabolic risk. We determined whether sleep disorders and circadian misalignment were associated with chronic inflammation and CVD risk in TDs from Southern Africa.MethodsParticipants were recruited at roadside wellness centres in Gauteng and Free State Provinces, South Africa. OSA risk was assessed using the Berlin Questionnaire, while sleep duration and sleep quality were assessed using items from the Pittsburgh Sleep Quality Index. Clinical information, neck circumference (NC), metabolic profile, elevated BP, HIV status and C-reactive protein (CRP) were collected. CVD risk was assessed using the Framingham Risk Score (FRS).ResultsOut of 575 participants aged on average 37.7 years, 17.2% were at OSA risk, 72.0% had elevated BP, 9.4% had HIV and 28.0% were obese. Mean sleep duration was 7.4±1.8 hours, and 49.6% reported working night shift at least once a week. Shortened sleep, OSA risk, age, body mass index, NC and years as full-time TD were associated with greater FRS independently of HIV status and night shift. Working night shift was associated with higher CRP levels in HIV+ compared with HIV− participants.ConclusionsCircadian misalignment in HIV, and OSA and short sleep duration in all truckers were associated with increased CVD risk. Truckers should be given careful attention in terms of health management and sleep education.

Background: Sleep disorders can act as risk factors and even aggravate underlying conditions. With prevalence of 17% in general population, hypertension is a leading cause of morbidity and mortality in India. Though hypertension has various well established risk factors like family history, sedentary lifestyle, poor diet, smoking and age, sleep is often an understudied and overlooked factor. Body mass index is another important risk factor for various physical conditions. Associations between sleep and body mass index have been documented in many studies around the world. Although a consensus is yet to be drawn, many studies highlight that BMI related disorders could be predicted by sleep duration and quality. Materials and Methods: Two hundred consecutive hypertensive patients who were attending the OPD for follow-up were included as participants in this study after obtaining an informed consent. A semi structured proforma was designed to elicit the socio demographic profile of the participants. Each participant was assessed for the presence of sleep disorders by sleep-50 questionnaire and quality of sleep by the Pittsburgh Sleep Quality Index (PSQI). Results: Results found that BMI was significantly correlated with sleep quality, sleep duration, and sleep disorder. Hypertension was not significantly correlated to sleep quality or duration but associated to sleep disorder. Conclusion: This study found that body mass index was significantly correlated with sleep variables such as sleep duration, sleep quality, and sleep disorders. Maintaining a healthy BMI could in fact impact the amount and quality of sleep an individual receives.

Introduction: Poor cardiovascular health has been linked to an increased likelihood of cognitive impairment in older adults. Cognitive impairment has also been identified as an emerging co-morbidity of obstructive sleep apnea, a highly prevalent sleep disorder, particularly in patients with neurological conditions. Whether other aspects of sleep, including sleep duration, sleep quality, sleep onset latency, and insomnia are associated with cognition is not established. Objective: The aim of this study was to evaluate whether specific sleep patterns were associated with cognitive function in a diverse population of both younger and older, neurologically healthy women, and to determine whether this association is mediated by cardiovascular disease (CVD) risk factors. Methods: This was a baseline analysis of 392 women (59% racial/ethnic minority, mean age=39±16.53y, range 20-76y) participating in the ongoing American Heart Association Go Red for Women Strategically Focused Research Network population-based study at Columbia University Medical Center (CUMC). Cognitive function was assessed by the validated Montreal Cognitive Assessment (MoCA) screening instrument. Sleep duration, sleep quality, and time to sleep onset were assessed using the Pittsburgh Sleep Quality Index; insomnia was assessed using the Insomnia Severity Index. Blood lipids and glucose were measured in the biomarker core laboratory at CUMC. Multivariable linear regression models were used to evaluate associations between sleep, CVD risk factors, and MoCA scores, adjusted for age, race/ethnicity, education, health insurance, and tested for interactions between age and sleep. Results: The prevalence of abnormal MoCA (score <26) was 38%; mean scores were lower in adults ≥55y vs. <55y (p<0.0001), and racial/ethnic minorities vs. whites (p<0.0001). Average nightly sleep duration was 6.75±1.29 h, and 50% of women had poor sleep quality. In multivariable models testing for interactions, lower MoCA scores were associated with shorter sleep duration (p=0.007), worse quality sleep (p=0.0005), and higher insomnia level (p=0.04). In stratified analyses, associations between MoCA scores and sleep duration, sleep quality, and insomnia persisted among both younger (<55y) and older (≥55y) groups. Lower MoCA scores were also associated with higher triglycerides (p=0.0001) and lower HDL-cholesterol (p=0.0006); formal tests of mediation suggested that the relation between cognition and insomnia was mediated by triglyceride level. Conclusions: Poor sleep patterns were highly prevalent and associated with lower cognitive function, even in younger women in this diverse population. Sleep patterns should be further investigated as a potential mechanism to identify individuals at risk of cognitive decline. Whether the relation is causal or mediated through traditional CVD risk factors deserves further study.

Abstract Introduction Circadian misalignment is commonly cited as a culprit of daytime sleep disturbances in night shift workers; however, the specific impact and magnitude that circadian misalignment has on daytime sleep has not been well-characterized in larger samples of night shift workers. Methods Participants included fixed-night shift workers (n=52, ages 18–50) who completed an 8-hour daytime polysomnography (PSG) in the lab following a night shift. Measures of sleep disturbances included: difficulty falling asleep (sleep onset latency [SOL], latency to persistent sleep [LPS]), difficulty staying asleep (sleep efficiency [SE], wake after sleep onset [WASO]), and sleep duration (total sleep time [TST]). Melatonin samples were collected hourly for 24 hours under dim light (&lt;10 lux) and used to determine dim light melatonin offset (DLMOff). Circadian misalignment (CM) was calculated as the time difference between bedtime and DLMOff (higher values represented sleeping after DLMOff), and correlated with PSG sleep variables. Results CM was significantly associated with difficulty staying asleep (WASO: r=0.48, p&lt;0.001; SE: r=-0.45, p&lt;0.001), and sleep duration (TST: r=-0.38, p&lt;0.01). Specifically, every 3 hours of CM on average added 19.2 minutes of WASO and reduced TST by 15 minutes. In contrast, CM was not significantly correlated with sleep onset difficulties (SOL: r=-0.27; LPS: r=-0.02). Conclusion These data suggest that circadian misalignment in shift workers may be a better predictor of difficulties staying asleep and sleep duration during the day relative to difficulties falling asleep. Because longer work hours (10–12 hours) are common in night shift worker, it may be that sleep initiation difficulties associated with circadian misalignment is masked by elevated fatigue or an increased homeostatic drive from prolonged wakefulness. These results may help guide decisions about the magnitude of phase shifts required (e.g., with light therapy) for the desired improvement in daytime sleep. OBJETIVO: Organizar conteúdos acerca da influência do exercício aeróbico de intensidade moderada na qualidade do sono. MÉTODOS: Caracterizado como um estudo de revisão narrativa, foi realizada uma revisão bibliográfica, buscando artigos em bases de dados online (PUBMED, Scielo e Google Acadêmico) publicados acerca do tema.RESULTADOS: Os estudos encontrados mostraram que o exercício aeróbico de intensidade moderada proporcionou aos indivíduos uma melhora significativa na duração do sono, diminuiu os distúrbios do sono, melhorou a latência e a eficiência do sono.CONCLUSÃO: Diante de tais achados pode-se considerar que o exercício aeróbico de intensidade moderada pode fazer parte do tratamento dos distúrbios do sono e através de um programa adequado de atividades físicas, o indivíduo pode alcançar uma melhora significativa na qualidade do sono.ABSTRACT. The influence of moderate intensity aerobic exercise on sleep quality: a narrative review.OBJECTIVE: To organize content about the influence of moderate-intensity aerobic exercise on sleep quality.METHODS: Characterized as a narrative review study, a bibliographic review was carried out, looking for articles in online databases (PUBMED, Scielo and Google Scholar) published about the topic. RESULTS: The studies found showed that aerobic exercise of moderate intensity provided individuals with a significant improvement in sleep duration, decreased sleep disorders, improved sleep latency and efficiency.CONCLUSION: In view of these findings, it can be considered that aerobic exercise of moderate intensity can be part of the treatment of sleep disorders and through an adequate program of physical activities, the individual can achieve a significant improvement in sleep quality.

AbstractNight shift workers are often associated with circadian misalignment and physical discomfort, which may lead to burnout and decreased work performance. Moreover, the irregular work hours can lead to significant negative health outcomes such as poor eating habits, smoking, and being sedentary more often. This paper uses commercial wearable sensors to explore correlates and differences in the level of physical activity, sleep, and circadian misalignment indicators among day shift nurses and night shift nurses. We identify which self-reported assessments of affect, life satisfaction, and sleep quality, are associated with physiological and behavioral signals captured by wearable sensors. The results using data collected from 113 nurses in a large hospital setting, over a period of 10 weeks, indicate that night shift nurses are more sedentary, and report lower levels of life satisfaction than day-shift nurses. Moreover, night shift nurses report poorer sleep quality, which may be correlated with challenges in their attempts to fall asleep on off-days.

Introduction: Sleep disorders among shift workers are common problems due to the disturbed circadian rhythm. The Bergen Shift Work Sleep Questionnaire assesses discrete sleep problems related to work shifts (day, evening and night shifts) and rest days. Aim: The aim of the study was to develop the Hungarian version of this questionnaire and to compare the sleep quality of nurses in different work schedules. Method: 326 nurses working in shifts filled in the questionnaire. The authors made convergent and discriminant validation of the questionnaire with the Athens Insomnia Scale and the Perceived Stress Questionnaire. Results: The questionnaire based on psychometric characteristics was suitable to assess sleep disorders associated with shift work in a Hungarian sample. The frequency of discrete symptoms significantly (p<0.001) differed with the shifts. Nurses experienced the worst sleep quality and daytime fatigue after the night shift. Nurses working in irregular shift system had worse sleep quality than nurses working in regular and flexible shift system (p<0.001). Conclusions: The sleep disorder of nurses working in shifts should be assessed with the Hungarian version of the Bergen Shift Work Sleep Questionnaire on a nationally representative sample, and the least burdensome shift system could be established.
