SUMMARYAfter experimental inoculation of Cytoecetes ondiri, the agent of bovine petechial fever (BPF), multiplication occurred in impala, bushbuck, Thomson's gazelles and wildebeest, as shown by infectivity studies and clinical findings. Similar attempts to infect one eland failed.As a sequel to this, blood and spleen samples were collected from four species of wild ruminants in an area where BPF was endemic. Isolations of C. ondiri were made from three of five bushbuck, but not from any other species.

We report the clinical findings and evolution of seven patients (five men and two women), the majority of them intravenous drug users, with paracoccidioidomycosis associated to acquired immunodeficiency syndrome (AIDS). In four of the patients the paracoccidioidomycosis was restricted to the lung and in the three others was generalized with cutaneous involvement. Only two of them had lived recently in rural area, an indication of the possible reactivation of latent focal infection in the other five patients. The recognition of the role of cell-mediated immunity in host defense against Paracoccidioides brasiliensis leds to the prediction of a growing occurrence of the paracoccidioidomycosis-AIDS association in areas that are endemic for these diseases.

Twenty-one physicians examined records of 43 patients who had attended the hospital because of chest pain. Of these patients, 20 had had coronary heart disease (CHD), 15 had had nonspecific pain, and eight had had pulmonary embolism. The physicians indicated the probability of CHD in each case on the basis of 18 clinical findings, not including ECG, x-ray, or biochemical studies. The trial was repeated five years later, using the same records, by 16 of the same physicians. Diagnostic accuracy was evaluated by ROC curves, and the weight ascribed to each cue was inferred by multiple regression with estimated probability of CHD as the dependent variable. No significant change of areas under the ROC curves with increasing length of clinical experience was observed. Multiple regression was significant in 30 of 37 analyses. The distributions of most physicians' estimates of probabilities had similar shapes five years apart. It is concluded that “experience” does not have a clear role in diagnostic performance based on recorded data and that personal calibration and preferences in estimating probabilities often persist for years.

This chapter reviews the pathophysiology of VTE, including its risk factors and long-term consequences. Diagnostic algorithms that integrate clinical findings, laboratory testing, and imaging are described. The role of risk stratification for identification of high-risk PE patients is highlighted. Options for the management of VTE are reviewed. Finally, practical recommendations for the prevention of VTE are provided.

Abstract Myelin-associated glycoprotein (MAG) is a 100-kDa integral membrane glycoprotein expressed by oligodendrocytes and Schwann cells in the central and peripheral nervous systems, respectively. It is found first in loosely wrapped myelin and then periaxonally after compaction. Clinical findings, structural analysis, and cell assays indicate a role for MAG in adhesion. We propose that the phosphorylation state of MAG modulates its adhesion and that a minimum spatial requirement for the separation of the kinase and phosphatase activities postulated by this model may explain the correlation between axon size and myelination state.

Abstract Background A healthy 25-year-old woman developed COVID-19 disease with clinical characteristics resembling Multisystem Inflammatory Syndrome in Children (MIS-C), a rare form of COVID-19 described primarily in children under 21 years of age. Case presentation The patient presented with 1 week of weakness, dyspnea, and low-grade fevers, followed by mild cough, sore throat, vomiting, diarrhea, and lymph node swelling. She was otherwise healthy, with no prior medical history. Her hospital course was notable for profound acute kidney injury, leukocytosis, hypotension, and cardiac dysfunction requiring ICU admission and vasopressor support. MIS-C-like illness secondary to COVID-19 was suspected due to physical exam findings of conjunctivitis, mucositis, and shock. She improved following IVIG, aspirin, and supportive care, and was discharged on hospital day 5. Conclusion MIS-C-like illness should be considered in adults presenting with atypical clinical findings and concern for COVID-19. Further research is needed to support the role of IVIG and aspirin in this patient population.

Purpose of the Report. There is a paucity of data on correlation of various imaging modalities with clinical findings in craniosynostosis. Moreover, no study has specifically reported the role ofTc99m-ECD SPECT in a large number of subjects with craniosynostosis.Materials and Methods. We prospectively analyzed a cohort of 85 patients with craniosynostosis from year 2007 to 2012. All patients underwent evaluation withTc99m-ECD SPECT and the results were correlated with radiological and surgical findings.Results.Tc99m-ECD SPECT revealed regional perfusion abnormalities in the cerebral hemisphere corresponding to the fused sutures preoperatively that disappeared postoperatively in all the cases. Corresponding to this, the mean mental performance quotient (MPQ) increased significantlyP<0.05postoperatively only in those children with absent perfusion defect postoperatively.Conclusions. Our study suggests that early surgery and release of craniosynostosis in patients with preoperative perfusion defects (absent onTc99m-ECD SPECT study) are beneficial, as theylead to improved MPQ after surgery.

Introduction. Indolent T-lymphoblastic proliferation (iT-LBP) is a rare nonmalignant entity that presents as a proliferation of T-lymphoblasts. We report a first such case with a recurrent laryngeal obstruction presentation that was successfully controlled with Sirolimus. Case presentation. This is the case of a 29-year-old female who presented with a recurrent significant lymphoid hyperplasia in the adenoid and tongue base region as well as a right cervical lymph node. After repeated adenoidectomies and tonsillectomies, and based on pathological and clinical findings she was diagnosed with iT-LBP. Trials of radiotherapy and immunotherapy with cyclosporine and rituximab all failed to control the progression of the disease. Sirolimus was finally able to restrict the growth and improve her symptoms. Conclusion. While It-LBP does not usually require treatment, it is important to report cases in which treatment was crucial for the survival of the patient, and the effective role of Sirolimus in doing so, without any major adverse effects.
