Objectives: To study clinical characteristics and treatment outcome of epistaxis in head and neck trauma. Methods: A prospective descriptive study of 71 cases of epistaxis managed at Hue Center Hospital and Hue Univesity Hospital from April 2015 to June 2016. Results: Most of bleeding times were at night (59.2%). Unilateral bleeding was seen in almost 72.9% cases. Anterior nasal bleeding was noted in majority of the patients (70.4%), anterior and posterior nasal was 18.3%, posterior nasal was 11.3%. There were three stage: mild (77.5%), moderate (15.5%), severe (7%). Anterior nasal packing (70.4%) were the most common methods, Posterior nasal packing were 25.4%, Local cauterization were 1.4%, Constriction of the blood vessels were 1.4%, Embolization procedure were 1.4%. Complication rate was 8.4% include: fever (5%), pressure necrosis (1.7%), scars (1.7%). The rate of good recovery after treatment was 91.7%, partial recovery was 8.3%. The overall mean of hospital stay was 6,33 ± 5,61 days (range 1 to 36 days). Key words: Epistaxis

Skin hyperpigmentation disorders arise due to excessive production of the macromolecular pigment melanin catalyzed by the enzyme tyrosinase. Recently, the therapeutic use of curcumin for inhibiting tyrosinase activity and production of melanin have been recognized, but poor stability and solubility have limited its use, which has inspired synthesis of curcumin analogs. Here, we investigated four novel chemically modified curcumin (CMC) derivatives (CMC2.14, CMC2.5, CMC2.23 and CMC2.24) and compared them to the parent compound curcumin (PC) for inhibition of in vitro tyrosinase activity using two substrates for monophenolase and diphenolase activities of the enzyme and for diminution of cellular melanogenesis. Enzyme kinetics were analyzed using Lineweaver-Burk and Dixon plots and nonlinear curve-fitting to determine the mechanism for tyrosinase inhibition. Copper chelating activity, using pyrocatechol violet dye indicator assay, and antioxidant activity, using a DPPH radical scavenging assay, were also conducted. Next, the capacity of these derivatives to inhibit tyrosinase-catalyzed melanogenesis was studied in B16F10 mouse melanoma cells and the mechanisms of inhibition were elucidated. Inhibition mechanisms were studied by measuring intracellular tyrosinase activity, cell-free and intracellular α-glucosidase enzyme activity, and effects on MITF protein level and cAMP maturation factor. Our results showed that CMC2.24 showed the greatest efficacy as a tyrosinase inhibitor of all the CMCs and was better than PC as well as a popular tyrosinase inhibitor-kojic acid. Both CMC2.24 and CMC2.23 inhibited tyrosinase enzyme activity by a mixed mode of inhibition with a predominant competitive mode. In addition, CMC2.24 as well as CMC2.23 showed a comparable robust efficacy in inhibiting melanogenesis in cultured melanocytes. Furthermore, after removal of CMC2.24 or CMC2.23 from the medium, we could demonstrate a partial recovery of the suppressed intracellular tyrosinase activity in the melanocytes. Our results provide a proof-of-principle for the novel use of the CMCs that shows them to be far superior to the parent compound, curcumin, for skin depigmentation.

Mangroves provide multiple ecosystem services and are essential for mitigating global warming owing to their capacity to store large carbon (C) stocks. Due to widespread mangrove degradation, actions have been implemented to restore them worldwide. An important representative case in Colombia is the Ciénaga Grande de Santa Marta’s restoration plan. This management intervention focused on restoring the natural hydrological functioning after massive mangrove mortality (~25,000 ha) due to soil hyper-salinization after river water input from the Magdalena River was eliminated. A partial recovery occurred during subsequent years, and hydrological management is still being implemented today. To understand how the degradation and subsequent management have affected mangrove C stocks, we compared C stocks in stands with different intervention levels reflected in their current forest structure. We found that the total C stock (398–1160 Mg C ha−1) was within the range measured in other neotropical mangroves without vegetation deterioration. The aboveground C was significantly higher in the stands where hydraulic connectivity was restored. By contrast, the belowground C was higher in the stands with low hydraulic connectivity due to channel clogging and a lack of sufficient maintenance. Our results show that hydrological management measures influenced above- and belowground C stocks, even at a 2 m depth. In addition, a strong indirect relationship useful for estimating carbon content from organic matter content was found.

Abstract Background Takotsubo syndrome is a frequent entity; however, it has never been described after a mitral valve surgery. Case summary  We present the case of a 79-year-old woman, with background of atrial fibrillation and a left atrial appendage closure device, who was admitted for elective mitral valve replacement, because of asymptomatic severe primary mitral regurgitation. Biologic mitral valve was implanted without incidences, but in the postoperative, she developed cardiogenic shock. Electrocardiogram (ECG) showed inverted T waves in precordial leads and an echocardiography showed severe left ventricular (LV) dysfunction with mid to distal diffuse hypokinesis, and better contractility in basal segments. Troponin levels were mildly elevated. With the suspicion of a postoperative acute coronary syndrome, a coronary angiography was performed and showed no significant coronary lesions. The haemodynamic situation was compromised for the next 48 h, in which vasoactive support and intra-aortic balloon counterpulsation were implemented. After 48 h, the haemodynamic situation suddenly improved. The ECG was normalized, and a control echocardiogram showed partial recovery of the LV function with resolution of regional wall motion abnormalities. The patient could be discharged at 1 week. The clinical picture was interpreted as a stress cardiomyopathy after mitral valve surgery. Discussion  Takotsubo syndrome is a threatening condition; complications in acute phase could lead to a fatal outcome. Mitral valve surgery has to be considered as a trigger for this entity, after excluding coronary involvement, specially of left circumflex artery.

Abstract INTRODUCTION There is a paucity of studies assessing the use of magnetic resonance-guided laser interstitial thermal therapy (LITT) in the elderly population. METHODS Geriatric patients (≥65 years) treated with LITT for intracranial tumors at a single institution from January 2011 to November 2019 were retrospectively identified. We grouped patients into two cohorts: 65-74 years (group 1) and 75 years or older (group 2). Baseline characteristics, operative parameters, postoperative course, and morbidity were recorded. RESULTS There were 55 patients who underwent 64 distinct LITT procedures. The majority of tumors (62.5%) treated were recurrent brain metastasis/radiation necrosis. The median hospital length of stay was 1 day, with no significant difference between age groups. Hospital stay was significantly longer in patients who presented with a neurological symptom and in those who experienced a postoperative complication. The majority of patients (68.3%) were discharged to their preoperative accommodation. Rate of discharge to home was not significantly different between age groups. Those discharged to rehabilitation facilities were more likely to have presented with a neurological symptom. Nine patients (14.1%) were found to have acute neurological complications, with nearly all patients showing complete or partial recovery at follow-up. The 30-day postoperative mortality rate was 1.6% (n = 1). The complication and 30-day postoperative mortality rate were not significantly different between age groups. CONCLUSIONS LITT can be considered as a minimally invasive and safe neurosurgical procedure for treatment of intracranial tumors in geriatric patients. Careful preoperative preparation and postoperative care is essential as LITT is not without risk. Appropriate patient selection for cranial surgery is essential as neurosurgeons treat an increasing number of elderly patients, but advanced age alone should not exclude patients from LITT.
