The production of two regulators of the inflammatory response, interleukin 10 (IL-10) and tumor necrosis factorα(TNFα), has been found to be deeply deregulated in SLE patients, suggesting that these cytokines may be involved in the pathogenesis of the disease. Genetic polymorphisms at the promoter regions of IL-10 and TNFαgenes have been associated with different constitutive and induced cytokine production. Given that individual steady-state levels of these molecules may deviate an initial immune response towards different forms of lymphocyte activation, functional genetic variants in their promoters could influence the development of SLE. The present review summarizes the information previously reported about the involvement of IL-10 and TNFαgenetic variants on SLE appearance, clinical phenotype, and outcome. We show that, in spite of the heterogeneity of the populations studied, the existing knowledge points towards a relevant role of IL-10 and TNFαgenotypes in SLE.

Numerous genetic variants of the Echinococcus antigen B (AgB) are encountered within a single metacestode. This could be a reflection of gene redundancy or the result of a somatic hypermutation process. We evaluate the complexity of the AgB multigene family by characterizing the upstream promoter regions of the 4 already known genes (EgAgB1-EgAgB4) and evaluating their redundancy in the genome of 3 Echinococcus species (E. granulosus, E. ortleppi and E. multilocularis) using PCR-based approaches. We have ascertained that the number of AgB gene copies is quite variable, both within and between species. The most repetitive gene seems to be AgB3, of which there are more than 110 copies in E. ortleppi. For E. granulosus, we have cloned and characterized 10 distinct upstream promoter regions of AgB3 from a single metacestode. Our sequences suggest that AgB1 and AgB3 are involved in gene conversion. These results are discussed in light of the role of gene redundancy and recombination in parasite evasion mechanisms of host immunity, which at present are known for protozoan organisms, but virtually unknown for multicellular parasites.

Dengue shock syndrome (DSS) is a severe complication of dengue hemorrhagic fever (DHF) and can lead to death, but DSS incidence and effects are difficult to predict via clinical examination. With consideration of this issue, this research determined the role of cytokines in the severity and prognosis of DHF in children. This prospective cohort research involved 234 patients who were aged 18 months to 18 years old, admitted to Tien Giang General Hospital, and diagnosed as having DHF. The cytokine levels of the patients were recorded from admission to discharge. Almost all the patients (>67%) exhibited increased interleukin 10 (IL-10), IL-6, and IL-2, whereas only 0.43% (the lowest) presented with elevated IL-12. Concentrations of IL-2, IL-4, IL-13, and tumor necrosis factor-α were highest in the patients infected with DENV-1, followed by those infected with DENV-3 and DENV-2. IL-12 concentration decreased with increasing days of fever occurrence. No correlation was found between cytokine concentrations in the first day of hospitalization and shock in the DHF patients. The concentration of cytokines during the first day of hospitalization (i.e., patients with a fever lasting less than 72 hours) cannot be used as a guide in determining DSS prognosis.
