Unique Case

Emphysematous osteomyelitis is a rare but potentially fatal condition that must be considered whenever intraosseous gas is identified on imaging. The organisms implicated in most cases of emphysematous osteomyelitis are anaerobes or members of theEnterobacteriaceaefamily. Significant comorbidities, such as malignancy and diabetes mellitus, frequently predispose to this condition, and high mortality rates have been reported. The radiologist must be aware of the implications of identifying intraosseous gas in order to facilitate early diagnosis and expedite management. We report a unique case of a 58-year-old male with diabetes mellitus who presented with emphysematous osteomyelitis of the midfoot and necrotising fasciitis of the ipsilateral distal lower limb. Specimen cultures in this case revealed a pure growth of Group GStreptococcus.

AbstractAcute hypoperfusion of the bowel without mechanical occlusion of the arterial or venous circulation is referred to as nonocclusive mesenteric ischemia (NOMI) and is responsible for approximately one fourth of all arterial causes of mesenteric ischemia. Patients present with nonspecific symptoms, making clinical diagnosis difficult. Prognosis is poor with high mortality rates. Mortality rates have improved during the past two decades from 80 to 50% due to early diagnosis and prompt initiation of treatment. In this review, we discuss the etiology, pathophysiology, and symptoms of NOMI as well as radiological findings and management along with comprehensive review of the literature.

Penile ossification is an exceedingly rare condition, with only a handful of histologically confirmed cases reported in the literature.  The most common condition leading to penile ossification is Peyronie’s disease.  Other conditions such as gout, ERSD, diabetes mellitus, hyperparathyroidism, and local trauma have also been associated with penile ossification.  We report a unique case of near-complete penile ossification of the corporal bodies with histologic confirmation on pathologic review.  Our report summarizes the literature regarding this rare entity.

High mortality and disability resulting from a stroke throughout the world is the cause of fi nding new opportunities to improve early diagnosis and optimize therapeutic approaches. The article presents a review of the literature devoted to the study of glial fi brillary acidic protein (GFAP) as a biomarker in the acute period of stroke, which showed that its use is possible not only to diff erentiate the ischemic and hemorrhagic process, but also to determine the degree of destruction of brain tissue, as in the fi rst hours of a stroke, and in the dynamics. In addition, the GFAP analysis can be useful for monitoring the course of the disease, control the eff ectiveness of therapy, as well as to predict outcomes.

Mucormycosis is an opportunistic, invasive fungal disease, which is most commonly found in patients with preexisting morbidity such as diabetes mellitus and prolonged steroidal therapy. During this COVID-19 pandemic, diabetic patient’s food and their lifestyle, as well as their access to medicine and regular medical care becomes disrupted. This could result in metabolic decompensation, resulting in hyperglycemia, and causes secondary bacterial and fungal infections such as Rhinocerebral mucormycosis and Staphylococcus sepsis. The most common cause of this condition is the extraction of maxillary posterior tooth, which accounts for more than 80% of all cases. In this article, five cases of mucormycosis have been documented after dental extractions in maxilla. Importance of understanding this disease, early diagnosis and appropriate aggressive treatment is emphasized in this article for a successful outcome modality associated with the surgeries are explained.

Summary: A 49-year-old man was diagnosed with pre-fibrotic myelofibrosis after acute left lower-limb ischemia requiring amputation and portal vein thrombosis. After surgery he developed heparin-induced thrombocytopenia (HIT) with venous thromboembolism, successfully treated with argatroban followed by dabigatran. Our systematic review of the literature supports the use of dabigatran for suspected HIT.

The absence of hallucal sesamoids is an uncommon and incidental entity, bilateral absence is rare, and few cases of symptomatic patients have been reported in literature. Most patients present with metatarsalgia limited to plantar aspect of the great toe, with aggravation of pain on walking and relief at rest. We present a unique case of ipsilateral symptomatic hypoplastic medial hallucal sesamoid and asymptomatic absence of medial sesamoid at the contralateral foot.
